Human-in-the-loop (HITL) infrastructure automation is a workflow in which software prepares or performs infrastructure work while a person reviews, approves, rejects, or takes control at selected points. For infrastructure as code (IaC), the clearest example is a proposed-change plan reviewed before it is applied. In agentic systems, a person can approve consequential actions, but that approval should complement—not replace—restricted permissions and technical controls.
The phrase describes a design pattern, not a single standardized product or protocol. A Terraform plan review is an established IaC workflow; an agent reasoning and acting across tools is a newer and broader use of automation.

How does human-in-the-loop infrastructure automation work?
A typical IaC workflow creates a review point between preparing a change and carrying it out. The software produces evidence of the intended change; a reviewer checks whether that evidence matches the request and whether the change is authorized.

- Prepare: An engineer changes the infrastructure configuration.
- Preview and validate: Automation generates a plan, runs relevant checks, and evaluates applicable policies.
- Review: The right person sees a readable plan and enough context to judge its impact.
- Authorize or reject: The reviewer approves the change or sends it back for revision.
- Apply and record: The authorized change is carried out, and the result is recorded for later investigation.

What does a reviewer actually approve?
A Terraform plan previews proposed resource creation, updates, and deletions. It gives reviewers something concrete to compare with the author’s stated intent: which resources are changing, how they are changing, and whether any unexpected destructive action appears. The plan is reviewable evidence of intended change, not a guarantee that the change is harmless or that every operational risk has been eliminated.
Terraform also supports saved plans for automation. When a saved plan is passed to terraform apply, Terraform applies that plan without asking for a new interactive approval. The workflow must therefore control the plan artifact and who is allowed to apply it; otherwise, the reviewed plan and the executed change may not remain aligned. See Terraform apply documentation.

Should a human approve every Terraform apply?
Not necessarily. Put human review at consequential boundaries—especially changes with a large blast radius, difficult recovery, or effects that policy alone cannot adequately judge. Routine, low-risk work may be handled through automated checks and narrowly scoped permissions where the team has confidence in the process.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteReview quality matters more than approval count. A reviewer needs a readable plan, policy results, and context about the request. Requiring a person to click approve for every action can overload reviewers and make approvals reflexive rather than meaningful. AWS recommends human final decisions for high-consequence actions while warning against approval overload in its guidance on security principles for agentic AI systems.

How do you add an approval gate safely?
Design the gate as one part of a control system, not as a substitute for authorization. Evaluate the workflow against these questions:
- Impact: How serious is an error, and how wide is the operation’s blast radius?
- Review evidence: Can the reviewer understand the plan, relevant policy results, and requested outcome?
- Artifact integrity: Is the plan that was approved exactly the artifact that will execute?
- Identity and separation of duties: Are the author, approver, and executor identifiable, and are their rights appropriately scoped?
- Auditability: Can the team later determine what was requested, reviewed, authorized, and applied?
- Operational behavior: What happens if a reviewer rejects the change, does not respond, or the apply fails?
- Reviewer workload: Are people asked to decide only when judgment is useful?
NIST warns that overuse of approval requests can lead to consent fatigue, while AWS recommends deterministic controls outside an agent’s reasoning. As NIST puts it, “It’s tempting to ask the human for access approval to support accountability and non-repudiation for agentic actions, but relying too heavily on HITL mechanisms introduces a severe risk of consent fatigue.” Read NIST’s guidance on identity for agentic AI.

What changes when an AI agent can act on infrastructure?
An agent may plan and take actions across several tools, making its potential effects harder to bound than a single, reviewable IaC plan. A person can be asked to decide on genuinely consequential actions, but the agent should not be trusted to enforce its own limits. AWS states: “Organizations should enforce security through deterministic, infrastructure-level controls external to the agent’s reasoning loop, not through the agent’s own reasoning, internal guardrails, or prompt-based instructions.”

What should happen on rejection or timeout?
An approval workflow is incomplete unless it defines the non-approval path. Rejection should stop the action and provide a clear route to revise or escalate it. A timeout should not silently become approval; the system should follow a defined fail-safe outcome and notify the responsible people. Logs should capture the request, reviewer interaction, decision, and result.
AWS Nova Act documents binary or multiple-choice approval and live UI takeover patterns for autonomous web workflows. Its documentation describes timeouts, rejection handling, supervisor notifications, and interaction logs. The capability is implemented in its SDK rather than provided as a managed AWS service; the documentation describes deploying a Human Intervention Service package in an AWS environment or building a custom interface. This is an illustration of intervention design for web automation, not an IaC approval product. See Nova Act human intervention documentation.
How should teams tune the amount of autonomy?
Start with review where the consequences are high, then use ongoing evaluation to decide whether a particular class of actions can safely proceed with less intervention. Keep durable technical constraints in place wherever the potential consequences justify them; reducing approval prompts should not mean granting an agent broader access. AWS recommends expanding autonomy deliberately based on evaluation rather than assuming that automation is safe simply because it has worked before.
