What Happens When a Pilot Seeks Mental Health Care? FAA Disclosure and Certification Explained

The FAA encourages appropriate mental health care, but pilots must accurately report requested visits, conditions, and medications. Learn how AME review, FAA deferrals, records, and antidepressant authorization work.

Seeking mental health care does not automatically disqualify a pilot from holding an FAA medical certificate. The FAA encourages appropriate counseling, but pilots must answer the medical application accurately and disclose the conditions and medications requested during the examination. The outcome depends on the individual’s symptoms, diagnosis, treatment, records, and the applicable FAA standards.

What does a pilot need to disclose?

FAA guidance says applicants must report health-professional visits from the previous three years, medications they are taking, and other medical history requested on the application. At the examination, pilots must disclose existing physical and psychological conditions and medications. An AME may ask follow-up questions based on the application and examination. See the FAA’s Pilot Mental Fitness guidance.

  • Answer each application question truthfully and completely as it applies to your history.
  • Be prepared to discuss the care you received, relevant symptoms or diagnoses, and any medication.
  • Do not omit a visit or change treatment to try to preserve certification. Discuss treatment decisions with your treating clinician.

What happens at the medical exam?

The FAA’s usual applicant process starts in MedXPress and continues with an examination by an FAA-designated aviation medical examiner (AME). An AME may issue a certificate when the applicable standards are met and the required documentation is available. Some cases require further information or an FAA decision rather than an immediate AME decision.

  1. Complete the MedXPress application. Bring its confirmation number to the appointment.
  2. Attend the AME examination. Bring relevant records and be ready to answer questions about your health history and treatment.
  3. Receive the AME’s disposition. The AME may issue if the requirements are met, or defer the case to the FAA when the condition or documentation requires FAA review.
  4. Respond to an FAA request. If the FAA needs more information, it generally sends a letter describing what to provide. Follow the instructions in that letter.

The FAA outlines this process in How do I get a Medical Certificate and what to expect during the AME examination.

Does counseling differ from treatment with medication?

The FAA says counseling or therapy is encouraged when medically appropriate. A counseling visit is not, by itself, the same as a disqualifying diagnosis. Medication can add a distinct aeromedical review because the FAA generally treats psychotropic medication use as disqualifying, subject to specific exceptions.

Situation What FAA guidance says What may happen in certification
Counseling or therapy without medication The FAA encourages counseling when medically appropriate. Applicants still report the visits and other information requested on the application. The AME considers the applicant’s history and circumstances; counseling alone does not establish a particular outcome.
Treatment involving medication Psychotropic medication use is generally disqualifying for aeromedical certification, with a conditional antidepressant pathway for selected cases. The medication, diagnosis, stability, symptoms, and records can affect whether the AME can act or must defer the case for FAA review.

The counseling guidance appears in the FAA’s Therapy, Psychotherapy and Counseling Information: Pilots / ATCS, updated May 27, 2026. The medication policy is described in the FAA’s AME Guide: Item 47, Psychiatric Conditions.

How does the FAA antidepressant pathway work?

For some applicants, FAA policy allows case-by-case consideration for special issuance or special consideration while taking a qualifying antidepressant. This is not routine AME approval or a guarantee of authorization. The FAA describes possible consideration for certain diagnoses when the applicant has been clinically stable for at least three continuous months on a stable dose, without aeromedically significant side effects or increased symptoms, and is taking a conditionally acceptable single medication.

The FAA lists exclusion criteria that include psychosis, suicidal ideation, electroconvulsive therapy, concurrent use of multiple SSRIs, and a multi-agent psychiatric drug protocol. Applicants who meet the conditions and want to continue medication are directed to a HIMS AME for further evaluation. The FAA’s Use of Antidepressant Medications page links to the current medication list and applicant instructions. The list can change, and formulations matter, so check the FAA’s current materials rather than relying on an older list or a drug name alone.

If applying for initial certification while taking an antidepressant

The FAA’s initial-certification handout, updated August 26, 2026, instructs applicants in this pathway not to fly under 14 CFR 61.53 until they have FAA authorization. It directs them to contact a HIMS AME, gather applicable treatment records, and document three months on the same dose without symptoms or side effects. The HIMS AME determines whether a submission is appropriate. These instructions are specific to this pathway; apply the no-fly direction that governs your own circumstances. See the FAA’s Airman Information – SSRI INITIAL Certification.

If considering stopping medication

FAA guidance describes a separate route to reapply for regular issuance after being off medication for 60 days and providing a favorable report from the treating physician. That is a certification condition, not medical advice to discontinue treatment. Do not stop or alter medication without direction from the treating clinician.

Which records may be needed?

Documentation depends on the condition and the FAA’s request. For some conditions, the FAA requires a detailed clinical progress note from a visit no more than 90 days before the AME examination. The FAA cautions that an After Visit Summary or patient-portal notes may not contain all required information; in most cases it requests the actual clinical progress note rather than a new physician letter. Check the FAA’s medical certification overview for the applicable instructions.

For certain FAA-requested psychiatric or psychological evaluations, the required review can include records, detailed clinical interviews, a mental-status examination, integrated findings, an explicit diagnostic statement, and recommendations. The FAA may require separate reports from a psychiatrist and clinical psychologist. It recommends evaluators familiar with aerospace medicine or psychology because limited familiarity may reduce a report’s usefulness. The requirements are set out in Specifications for Psychiatric and Psychological Evaluations.

For the initial antidepressant pathway, the FAA instructs applicants to provide the HIMS AME with treatment records from periods involving psychiatric medication or symptoms and to use the FAA certification aid to coordinate provider reports. The precise packet depends on the FAA’s case requirements; the FAA’s antidepressant-treated pilot FAQ addresses what to take to the AME exam.

Does FAA certification settle an airline’s requirements?

No. FAA medical certification and an airline employer’s own mental-health screening or employment requirements are separate matters. The FAA notes that commercial airlines may have their own screenings and requirements; a medical certificate alone does not establish what an employer will require. The FAA’s Pilot Mental Fitness page discusses this distinction.

What does the FAA’s denial statistic mean?

The FAA reports that about 0.1% of medical certificate applicants who disclose health issues are denied. This is a broad figure on the FAA’s Pilot Mental Fitness page, last updated August 22, 2024 and checked October 7, 2026; it is not broken down by mental-health condition, medication, or applicant group. It cannot be used as an individual prediction or as a mental-health-specific denial rate.
