“Bloodied” depends on the D&D edition. In the 2024 rules, a creature is Bloodied when it has half its Hit Points or fewer; the status has no effect on its own, though another rule can use it as a trigger. The 2014 fifth-edition Basic Rules do not define Bloodied as a condition. Fourth Edition used a bloodied-value threshold that some powers and effects referenced.
What is the Bloodied condition in D&D?
In the 2024 rules, Bloodied is a defined Hit Point threshold: a creature is Bloodied while it has half its Hit Points or fewer remaining. The D&D 2024 Rules Glossary defines it this way. Because the rule says “half or fewer,” a creature at exactly half its Hit Points qualifies.

The 2024 Basic Rules clarify the consequence: “If you have half your Hit Points or fewer, you’re Bloodied, which has no game effect on its own but which might trigger other game effects.” This is stated in Playing the Game, Damage and Healing. Bloodied is therefore a status threshold, not an automatic damage penalty or incapacitating condition. Apply an effect only when a separate feature, spell, item, or monster rule says to.

Does Bloodied do anything in 5e?
2014 fifth edition
The 2014 Basic Rules conditions appendix does not list Bloodied as a defined condition. A DM can use “bloodied” as descriptive shorthand—for example, to signal that a monster is badly hurt—but narration alone does not impose a mechanical penalty under those rules.

2024 fifth edition
Bloodied is explicitly defined in the 2024 rules, but the threshold itself still does nothing. It matters when another rule uses the Bloodied state as a trigger or condition. The 2024 rules overview and changelog distinguish the 2024 rules from the earlier 2014 rules.
When is a monster Bloodied?
Under the 2024 definition, a monster is Bloodied once its remaining Hit Points are at or below half its Hit Points. Use the Hit Point total and maximum applicable to that creature; exactly half counts. The threshold does not, by itself, change the monster’s actions, defenses, or damage.

In 2014 fifth edition, there is no default Bloodied threshold in the Basic Rules. A DM may choose to use the word narratively or adopt a house rule, but should make clear whether that choice has any mechanical effect.

How did Bloodied work in 4th Edition?
A 4th Edition Dungeon Master’s Kit excerpt describes a typical creature’s bloodied value as half its maximum Hit Points. It notes that some powers and effects work only against, or work better against, a bloodied target. This makes Bloodied a meaningful mechanical threshold for rules that refer to it, rather than a universal effect that applies identically to every creature. The excerpt is available in an archival copy of the Dungeon Master’s Kit.

For a specific 4e power or printing, consult the relevant book and errata. Wizards’ 4th Edition support page says customer service no longer answers rules questions for that edition and directs readers to collected developer updates and errata.
Can I use Bloodied as a homebrew condition?
Yes. In a 2014 campaign, a DM can borrow the 2024 threshold or use “bloodied” only as a narration cue. Tell players which approach the table uses. If Bloodied has a mechanical consequence, write that consequence into the relevant feature or monster rule instead of assuming the label alone creates one.

A clear house rule should specify:
- Threshold: Is Bloodied half of maximum Hit Points, a fixed number, or another value? If the number can be odd, specify how the threshold is rounded.
- Duration: Does the status end as soon as healing raises the creature above the threshold?
- Timing: Does an effect trigger once per encounter, each time a creature becomes Bloodied, or on every turn it remains Bloodied?
- Scope: Does the rule apply to characters, monsters, objects, or only specified creature types?
- Temporary Hit Points: State how they interact if the homebrew rule needs to address them; do not assume an unstated answer.
The 2024 rules give a threshold, but a custom trigger still needs its own timing and effect spelled out.
