What Data Should a Workers’ Comp Form Integration Capture and Protect?

A practical guide to mapping workers’ compensation form data by program, limiting sensitive information, and building claim-matching and privacy workflows.

Capture the worker and employer identifiers, insurance details, incident information, and program-specific claim identifiers needed to establish and route a claim. Collect medical information only for a defined purpose and handle it according to the sender’s role, applicable law, and any required authorization. There is no universal workers’ compensation form schema: the controlling fields and workflows depend on the jurisdiction, program, and current form or EDI guide.

Start with the program’s form and EDI rules

Before designing fields, identify every state or federal program the integration will serve and obtain its current forms, EDI implementation guide, reporting timelines, and disclosure rules. Treat each program as a separate mapping target, even if the integration presents a shared screen to users.

Pennsylvania, for example, says its First Report of Injury (FROI) establishes a claim in WCAIS and describes a state-specific EDI implementation guide. Its agency also warns that a form received before the initial FROI may not be attachable to a claim in that system. These are Pennsylvania workflow rules, not a national sequence standard. Keep a common data model, but map it to each program

A shared internal model can make an integration easier to maintain, but it should not flatten program differences. For each field, record the destination form or transaction, whether it is required or conditional, the accepted format, its purpose, and the rule for sending it. Preserve the program’s transaction sequence and identifier formats rather than assuming one state’s form can stand in for another.

Which data should the integration capture?

The table is a design checklist, not a universal list of legally required fields. The target program’s current specification determines what is mandatory.

Data category Why it may be needed Capture and handling approach
Worker identity and contact details Identify the claimant and support claim communications. Capture the identifiers required by the program and validate them before routing. Pennsylvania lists employee name, address, and Social Security number among the details to check. Because an SSN is sensitive, do not make it the integration’s universal matching key when the program permits an approved claim identifier. Pennsylvania claim forms guidance; OWCP billing requirement for claimant information
Employer and insurance details Associate the reported event with the correct employer, carrier, or administrator. Validate against the target program’s required fields and insurance arrangements. Pennsylvania specifically calls out employer name and address and insurance information. Pennsylvania claim forms guidance
Injury or illness date and description Establish what event or condition is being reported and support claim routing and administration. Use a structured date and an accurate description where the form requires them. Do not ask for a broad medical history by default; collect additional health details only when the workflow and purpose call for them. Pennsylvania identifies the injury date and description as accuracy checks. Pennsylvania claim forms guidance
Claim and program identifiers Connect later forms, bills, and reports to the correct case. Enforce each program’s sequence and format rules. Pennsylvania’s FROI/WCAIS workflow and federal OWCP billing identifiers illustrate that matching requirements differ by program. Pennsylvania claim forms guidance; OWCP billing requirement for claimant information
Medical reports and other health information May support care coordination, claim administration, or payment where allowed. Limit collection and disclosure to the relevant purpose and recipient. Track the applicable legal basis or authorization when needed, along with its scope. HHS guidance on workers’ compensation disclosures; HHS minimum-necessary FAQ
Notice, authorization, and submission record Explain collection or use where required and document what was submitted and to whom. Use the official form and workflow for the relevant program. Keep the applicable notice or authorization and a record of the submission; provide claimant-facing copies when the program requires them. Protect health information according to role and purpose

Do not assume that HIPAA applies to every organization in a workers’ compensation workflow simply because health information is present. HHS says the HIPAA Privacy Rule generally does not apply to workers’ compensation insurers, workers’ compensation administrative agencies, or employers, except to the extent they are otherwise covered entities. That does not remove a covered provider’s disclosure obligations or requirements imposed by other applicable laws. HHS: Disclosures for Workers’ Compensation Purposes

For covered entities, HHS explains that disclosures may be made without an individual authorization when they are authorized and necessary under workers’ compensation law, required by state or other law, or made for payment for health care provided to an injured or ill worker. A covered entity may also disclose information under a valid individual authorization. For certain workers’ compensation and payment disclosures, covered entities must reasonably limit protected health information to the minimum necessary. HHS notes that this standard is not intended to block information needed for claims processing or care coordination; disclosures required by law or made under authorization are outside that standard. Translate the rules into integration behavior

  • Assign a purpose to each sensitive field and define which recipient or role may receive it.
  • Separate claim-establishment data from medical reports when the workflow allows, rather than sending every available record in every transaction.
  • Where a disclosure depends on authorization, preserve the authorization and its scope with the submission record; do not reuse a form beyond the purpose it permits.
  • Use access restrictions, transmission safeguards, validation, and audit records appropriate to the organizations’ laws, contracts, and policies. The cited rules do not establish one complete technical-control baseline for every integration.

Program examples show why one workflow will not fit all

Pennsylvania: first report and claimant copy

Pennsylvania says the initial FROI establishes a claim in WCAIS and identifies employee, employer, insurance, and incident details to check. It also says a true and correct copy of submitted EDI or uploaded information must still be provided to the claimant in the circumstances described by the agency. Build the applicable sequence and copy workflow into the Pennsylvania implementation rather than assuming a successful upload completes every obligation. Federal OWCP: billing identifiers are program-specific

OWCP’s medical billing page gives claimant-information requirements for its programs and directs submitters to program-specific companion guides and billing instructions. It states that the listed claimant-information requirements took effect January 10, 2025. Apply those requirements to the relevant OWCP billing workflow, not to every workers’ compensation claim form. OWCP billing requirement for claimant information

New York: authorization scope matters

New York’s HIPAA-1 form describes a claimant’s authorization for a provider to file reports with parties selected on the form. The Board’s claimant forms page also describes a limited release for specified previous injury or similar-illness circumstances. These are examples of New York workflows, not reusable authorization language for other jurisdictions. New York HIPAA-1 authorization; New York injured-worker forms

Texas: claimant-file confidentiality

The Texas Division of Workers’ Compensation says information in or derived from a claimant’s file is confidential and may not be disclosed except as provided by the Texas Workers’ Compensation Act. Its page describes restricted channels for requesting confidential claimant records, so a Texas integration should route such requests through the permitted process. Federal employee compensation: program-specific privacy notice

The Department of Labor’s iFECS privacy assessment describes Privacy Act considerations for claimant forms and says the claim form explains information use. That is a federal employee compensation example, not a universal notice template. DOL iFECS Privacy Impact Assessment

Decisions to settle before implementation

  1. Define scope: list the jurisdictions and programs the integration will support, then identify the current form and EDI guide for each.
  2. Map roles: identify whether data is collected or sent by an employer, provider, insurer, administrator, board, or integration vendor, and determine which participants are covered entities.
  3. Document purpose and authority: specify why each field is collected and whether its disclosure is required by law, authorized, or otherwise permitted.
  4. Minimize and match: distinguish required data from convenient data, choose program-approved identifiers, and test transaction sequence and claim matching to prevent orphaned forms or misrouted bills.
  5. Design notice and claimant-copy steps: use the official notice or authorization where applicable and include any required claimant access or copy workflow.
  6. Set security and retention rules: derive controls and retention schedules from applicable law, contracts, and organizational policy. The cited sources do not establish a universal retention period or a complete security baseline.

The final field list, filing deadlines, authorization requirements, and retention schedule cannot be determined without the target jurisdiction, program, sender and recipient roles, and integration boundary. Verify those details against current official forms and specifications before enabling submissions.
