What Causes Hearing Loss, and Which Treatments Can Help?

Hearing loss may stem from blockage, infection, noise, aging, medicines, injury, or other conditions. The right response depends on its cause and type—and sudden loss needs urgent assessment.

Hearing loss can result from a blockage or infection in the outer or middle ear, damage to the inner ear, or problems along the auditory pathway. Some causes can be treated directly; permanent loss may be helped by hearing aids or, for selected people, cochlear implants. A sudden drop in hearing—especially in one ear—needs urgent medical assessment rather than watchful waiting.

What happens in the ear when hearing is lost?

Clinicians distinguish two broad patterns. Conductive hearing loss occurs when sound cannot travel normally through the outer or middle ear. Earwax, fluid, or infection can contribute. Sensorineural hearing loss involves the inner ear’s sensory structures or the auditory nerve and related pathways. The two types can also occur together.

The distinction matters because a blockage or infection may be treatable, while damage to inner-ear structures may call for hearing technology or other support. Symptoms alone cannot reliably identify the type or cause; an examination and hearing test are often needed.

What can cause hearing loss?

Age-related changes

Age-related hearing loss, or presbycusis, usually develops gradually. It can make speech harder to follow, particularly in noisy places. Changes in the inner ear may be involved, and age-related loss can coexist with effects from noise, medicines, or other health conditions. NIDCD guidance on age-related hearing loss

Loud sound

Noise-related loss can build gradually after repeated exposure or happen immediately after an extremely loud sound, such as a gunshot or explosion. NIDCD says sounds at or below 70 A-weighted decibels (dBA) are unlikely to cause hearing loss even after long exposure; that does not mean every sound above 70 dBA causes damage at every duration. Human inner-ear hair cells do not grow back once lost. NIDCD guidance on noise-induced hearing loss

Wax, fluid, and infection

Earwax or fluid can impede sound transmission, and an ear infection may affect hearing. These are examples of conductive causes that a clinician can assess and may be able to treat. Do not assume a new or marked hearing change is only wax or congestion, especially if it came on suddenly.

Medicines, injury, and other health conditions

Some medicines, head injuries, diseases, and other conditions can contribute to hearing loss. In sudden sensorineural hearing loss, possible causes include infection, trauma, autoimmune disease, certain drugs, circulation problems, neurological disorders, and Ménière’s disease. An identifiable cause is found in only about 10 percent of diagnosed sudden sensorineural cases, according to NIDCD. NIDCD guidance on sudden deafness

When is hearing loss urgent?

A sudden loss of hearing is a medical emergency. Seek medical attention immediately for a sudden change in one or both ears; do not wait to see whether it clears or treat it as presumed wax, allergies, or sinus trouble. NIDCD warns that delaying assessment and appropriate treatment can make treatment less effective. NHS guidance also advises urgent help for sudden hearing loss or hearing that worsens over days or weeks. NIDCD: Sudden Deafness · NHS: Hearing loss

Sudden sensorineural hearing loss can appear all at once or over several days, often in one ear, sometimes alongside ear fullness, dizziness, or tinnitus. About half of people with the condition recover some or all hearing spontaneously, usually within one to two weeks from onset, according to NIDCD. That possibility is not a reason to delay assessment.

How is hearing loss assessed?

Sudden or unexplained change

A clinician will consider whether a conductive problem, such as wax or fluid, explains the change. For sudden unexplained loss, NIDCD says pure-tone audiometry should be performed within a few days. One clinical sign used for sudden sensorineural hearing loss is a loss of at least 30 decibels across three connected frequencies within 72 hours. This is a diagnostic criterion, not a self-test. Further tests may be needed to investigate a cause.

Gradual or persistent difficulty

A primary care provider can check for wax, infection, or injury and refer to an ear, nose, and throat specialist (otolaryngologist or ENT) or an audiologist. An audiologist tests hearing, identifies its type and degree, and can advise on devices. NHS care pathways and referral arrangements apply in the UK; elsewhere, the route into care may differ. Which treatments may help?

Option When it may fit What to know
Treating an identified cause When examination finds a treatable problem, such as wax or infection A clinician may remove wax or treat infection; management depends on the specific cause.
Corticosteroids for sudden sensorineural loss For sudden sensorineural hearing loss, especially when its cause is unknown NIDCD identifies corticosteroids as the most common treatment. They should be used as soon as possible for the best effect; a clinician decides suitability and route, and injections into the middle ear are an option in some cases. Treatment delayed more than two to four weeks is less likely to reverse or reduce permanent loss.
Hearing aids For some people with permanent loss who retain some inner-ear hair-cell function They amplify sound and may improve hearing and speech comprehension, but have practical limits. A hearing professional programs prescription devices.
Cochlear implants For selected people with severe or profound hearing loss They require surgery and candidacy assessment, followed by adaptation and therapy. They do not restore normal hearing.
Communication supports Alongside clinical care, to make conversations and listening situations easier Reducing background noise, facing the speaker, captions, and assistive listening devices can support participation; they do not treat the underlying loss.

NIDCD notes that corticosteroids are the most common treatment for sudden deafness, especially when the cause is unknown. The appropriate option depends on examination and audiologic findings, not just how severe the loss feels.

Hearing aids: prescription and OTC options

Hearing aids make sounds louder; they do not repair damaged hair cells. Benefit varies with the type and degree of loss and the remaining inner-ear function. In the United States, over-the-counter hearing aids are intended for adults who perceive mild-to-moderate hearing loss. More significant, complicated, or sudden problems call for clinical evaluation rather than choosing a device as a substitute for assessment. OTC rules and availability are jurisdiction-specific. U.S. FDA: OTC Hearing Aids

Cochlear implants are not hearing aids

A cochlear implant bypasses damaged parts of the ear and electrically stimulates the auditory nerve. It is a surgical option for some people with severe or profound loss, not a louder version of a hearing aid. Whether someone is a candidate depends on specialist assessment, including the hearing profile and auditory nerve function; adapting to the implant takes time and therapy. NIDCD guidance on cochlear implants

How can you reduce risk and improve communication?

  • Reduce exposure to loud sound and use suitable hearing protection around it. This can help prevent additional noise-related injury but cannot reverse established loss.
  • In conversations, reduce background noise and face the speaker so visual cues are available.
  • Use captions or assistive listening devices where they make communication easier.
  • Arrange an assessment for new, worsening, or persistent hearing difficulty; communication aids do not replace finding out what is causing it.
