It does not reveal one definitive age hidden in the body. What a result can tell you depends on the test’s sample, method, intended purpose and validation.
What does a biological-age test actually measure?
“Biological age” is an umbrella term, not a standardized measurement. Tests may use DNA methylation patterns, routine clinical measures or other molecular inputs. The result is an estimate produced by applying a model to those measurements; it describes the target the model was built to estimate, not every dimension of a person’s health or aging. The 2025 review in Annual Review of Public Health discusses these different approaches and their uses.

Many consumer epigenetic tests analyze DNA methylation: chemical marks at selected sites in DNA. A clock uses a chosen pattern of those marks and a model trained against a target. Depending on the clock, that target might be chronological age, health outcomes or the pace of aging. The target matters: a clock trained to estimate chronological age should not be treated as a measure of healthspan or aging speed unless its validation supports that interpretation. The 2024 review on validation of aging biomarkers explains why the intended target and validation matter.

| Output type | What it represents | What not to assume |
|---|---|---|
| Age-like estimate | A model’s estimate expressed in years, often trained against chronological age or another specified target. | That it captures overall health, future lifespan or healthspan. |
| Age acceleration or age difference | A comparison of a result with a reference or comparison group, using the test’s stated method. | That the difference is a diagnosis or a universal threshold for being healthy or unhealthy. |
| Pace-of-aging measure | An estimate about the rate or pace of aging, rather than an age in years. DunedinPACE is one example discussed as a DNA-methylation pace-of-aging biomarker. | That its value can be read as a number of years of biological age. |
These categories are not interchangeable. The 2024 biomarker-validation review discusses the need to match an interpretation to what a measure was designed and validated to estimate.
How accurate are biological-age tests for one person?
Epigenetic clocks are useful research tools, and studies have reported associations between clock results and health outcomes. But a statistical relationship in a population does not automatically give a dependable forecast for an individual. A model may distinguish or rank groups without establishing a clear personal cutoff, reliable repeat measurements or evidence that acting on a result improves health.

There is no single accuracy percentage that applies to all biological-age tests. Accuracy depends on what the test claims to measure, the people and outcomes used to validate it, and how well the method performs for the person and use in question. The 2025 review on the limits of epigenetic clocks as personal biomarkers examines the gap between population-level findings and individual clinical use.

Why can two tests give different results?
Two services may use different clocks, algorithms, laboratory methods, sample types, reference populations or rules for interpreting a result. Even where both analyze DNA methylation, they may be estimating different targets. Their scores should not be treated as competing readings on a single universal scale.

The tissue used for a sample can matter as well. Research has compared results across tissues such as saliva, buccal cells and blood; similarity in how methods rank people in a study does not by itself establish close agreement for an individual. The 2025 cross-tissue comparison of epigenetic aging clocks addresses this issue. Population characteristics and context—including race, sex, lifestyle and environmental influences—are also relevant to how well a clock applies, as discussed in the 2025 public-health review.
Can a lower score show that you slowed or reversed aging?
No. A lower follow-up result, by itself, does not establish that aging slowed, was reversed or that health improved. Scores can only be compared meaningfully when the same model, assay, sample type, collection protocol and interpretation framework are used. Even a repeatable change in a biomarker is not automatically a clinical benefit: that requires evidence connecting the change to meaningful outcomes such as less disease, disability or mortality risk.

A 2026 review of epigenetic biomarkers’ responsiveness to longevity interventions in humans considers how these measures respond in intervention research. A score change should not be taken as proof that a particular intervention extended life or improved health.
What should you check before buying or interpreting a test?
Before relying on a consumer result, look for specific, understandable information about the method and its limits:
Best Value

- Sample and collection: What specimen is required, and what collection instructions must be followed?
- Method: Which clock, markers or algorithm produces the score? A general “biological age” label does not identify the method.
- Purpose: Is the output intended to estimate chronological age, predict a health outcome, or describe pace of aging?
- Validation: What population and outcome were used to evaluate the method, and is there independent evidence relevant to people like you?
- Reliability: Is there evidence that measurements can be repeated consistently under the same conditions?
- Interpretation: Does the provider explain limitations and make clear that the result is not a diagnosis?
These questions follow from the distinction between measuring a marker, validating what it predicts and showing that using the result helps someone make better health decisions. They also help reveal whether the test addresses the question you actually want answered.
What FDA oversight means for direct-to-consumer tests
The FDA describes direct-to-consumer tests as tests in which consumers collect a specimen—often saliva or urine—and send it to a company for analysis. Oversight depends on a test’s intended use and risk. The FDA says many general-wellness, non-medical or low-risk tests are generally not reviewed before being offered, while moderate- to high-risk tests with medical purposes are generally reviewed for their claims and validity. That does not establish the regulatory status of any particular biological-age product; check the product’s stated intended use and applicable status in the FDA’s direct-to-consumer test guidance.
The FDA distinguishes analytical validity—whether a test accurately and reliably measures what it claims to measure—from clinical validity—whether that measurement predicts a health state. These are separate from whether using the result improves a consumer’s decisions or health. The FDA advises against using test results as the sole basis for medical decisions or as a replacement for recommended screenings and healthcare visits. Discuss health concerns and medical decisions with a qualified healthcare professional.
