Cough, throat symptoms, tingling, shortness of breath, indigestion and nausea are also reported. Do not stop prescribed VNS therapy on your own for an ordinary side effect; contact your VNS team about persistent or troubling symptoms. Possible device malfunction is different: FDA physician labeling instructs patients who suspect malfunction to use the magnet to stop stimulation and contact their physician immediately.
This guidance concerns implanted VNS, not non-invasive stimulators worn or held against the skin. Those devices have different labeling and side-effect profiles.

Which side effects are common with implanted VNS?
Hoarseness or another change in voice is the most commonly reported effect associated with stimulation in the FDA patient manual for the VNS Therapy System. It usually occurs during the device’s ON period. Its intensity can range from barely noticeable to severe, and may vary with stimulation settings. Other reported effects include increased coughing, throat inflammation or discomfort, tingling, shortness of breath, indigestion, nausea and, less commonly, laryngismus (a spasm involving the larynx).

The figures below come from two distinct clinical-study reports in FDA patient manuals. They describe those study populations, not the expected rate for every patient, indication, device model or current treatment setting.

These percentages are reported in separate FDA patient manuals: the VNS Therapy System manual’s historical observed table and its D-02 depression study report. The D-02 figures are study-specific and should not be generalized to other VNS indications or models. A percentage from either report cannot predict whether a particular person will experience a symptom or how severe it will be.
When should you contact your VNS care team?
Tell your treating clinician about symptoms that are persistent, worsening, painful, or interfering with breathing, swallowing, sleep or daily activities. The timing and pattern matter: a brief voice change only during a stimulation cycle differs from hoarseness that continues between cycles.

- Hoarseness that persists between stimulation cycles: FDA physician labeling says persistent hoarseness not associated with stimulation may indicate nerve irritation and should be investigated immediately. A brief stimulation-linked change is not the same warning sign, but discuss it if it is bothersome.
- Breathing or sleep changes: Shortness of breath can occur during active stimulation. FDA labeling also warns that VNS may increase obstructive sleep apnea events or lead to new-onset sleep apnea; people with pulmonary disease may be at increased risk. Report new or worsening breathing symptoms promptly.
- Difficulty swallowing, choking or possible aspiration: Stimulation can cause difficulty swallowing, and worsening swallowing problems may raise aspiration concerns. The physician manual notes that pausing stimulation with the magnet while eating may mitigate aspiration risk. Follow the instructions for your specific device and the plan from your care team rather than improvising changes.
- Painful, irregular or unexpected stimulation: FDA physician labeling warns that device malfunction can cause painful or direct-current stimulation and potential nerve damage. If you suspect malfunction, the manual instructs you to use the magnet to stop stimulation and contact your physician immediately; prompt surgical intervention may be needed.
- Sudden loss of stimulation or worsening symptoms: Report a change in your clinical symptoms or no longer perceiving stimulation to your VNS team so they can assess the device. An FDA Class 2 recall, initiated December 12, 2024, and posted January 31, 2025, concerns specified SenTiva and SenTiva Duo generators with serial numbers below 500,000 that may stop delivering stimulation. The notice advises clinicians to run diagnostics; ask your team to check whether your model and serial number are included.
The cited labeling does not set one universal emergency-care rule for every symptom. For severe or rapidly worsening symptoms, follow local emergency guidance and any emergency plan your clinician has given you.

Should you stop stimulation for an ordinary side effect?
Do not treat routine hoarseness, cough or another expected stimulation-linked effect as an instruction to discontinue prescribed therapy. Contact your VNS team to discuss bothersome effects; device settings may be adjustable, but setting changes should be made with the treating clinician.

The magnet instruction is specifically for suspected malfunction in the FDA physician manual: use the magnet to stop stimulation and contact the physician immediately. It is not blanket advice to stop treatment whenever a side effect occurs. If swallowing difficulty is a concern while eating, the manual describes magnet use to pause stimulation as a possible way to mitigate aspiration risk; follow your own device instructions and clinician’s plan.

“Vagus nerve stimulation” can refer to implanted systems or external, non-invasive devices. Their indications, labeling, instructions and adverse effects differ, so do not apply implanted-device rates or magnet instructions to an external stimulator.

For example, FDA labeling for the prescription gammaCore Sapphire CV describes application-site discomfort or redness, local pain, facial or neck muscle twitching, headache, dizziness and tingling. Those are device-specific effects, not a proxy for the rates or risks of implanted VNS. Check the labeling for the exact device and consult its prescribing clinician about symptoms.
