Utah has authorized a limited pilot in which Nolla Health’s app assesses acne from patient photos and can recommend first-time prescriptions or refills for certain topical treatments. It is not permission for AI to diagnose and prescribe freely: the pilot is restricted to eligible Utah adults with mild-to-moderate acne, and two Utah-licensed physicians review every prescription before it is sent during the initial stage.
What the Nolla pilot allows
Nolla’s agreement covers assessment of acne using patient photographs and prescriptions for a short list of topical creams and gels. It applies to Utah adults with mild-to-moderate acne and permits both first-time prescriptions and refills. The agreement’s stated term is October 5, 2026, through October 5, 2027.

- Oral acne medication and isotretinoin are outside the pilot.
- Severe acne is excluded.
- The process stops for certain risk factors, including pregnancy or plans to become pregnant, breastfeeding, a weakened immune system, or a prior reaction to a listed medication.
That is a defined clinical task under one company’s agreement—not a general authorization for AI to examine patients or prescribe any medication in Utah.

How clinician oversight works
During Nolla’s first stage, two Utah-licensed physicians review every prescription before it is sent. The agreement allows later stages to use retrospective review and then sampling, but neither change is automatic: each requires written approval from the state office. The state listing does not specify a patient-count threshold for moving between those stages.

The agreement also calls for identity and age verification, prescriptions only to Utah addresses, disclosure to pharmacists, a physician contact for pharmacists, adverse-event reporting, and protections for patient data. These are controls required for this specific pilot; they do not establish how well the system performs in practice.
How Utah’s other healthcare AI pilots differ
Utah announced five healthcare agreements on October 5, 2026. Two are umbrella agreements with University of Utah Health and Intermountain Health for future pilots that will be reviewed individually. The targeted agreements cover different activities; Nolla’s acne prescribing should not be confused with medication-renewal pilots.

Separately, Utah’s current FAQ describes refill-pilot formularies as ranging from roughly 20 to roughly 200 approved medications, depending on the program, and says no listed refill formulary includes controlled substances. That is a broad description of refill programs, not Nolla’s acne-treatment list. The figures and review stages above describe agreement terms or published commentary, not safety or effectiveness results.

What participation and state oversight mean
Utah’s Department of Commerce says its Office of AI Policy operates a supervised sandbox to study AI use in the state. Each agreement must define its limits, safeguards, consumer disclosures, and reporting. Participation is a patient’s choice; AI involvement must be disclosed; and a patient may request a licensed clinician at any stage. Each agreement applies to a named company, product, and task for a limited period. The state also cautions that an agreement is not an endorsement of the product.

For prescription-refill programs generally, the state says participants must file monthly reports with agreement-specific metrics and give pharmacists a direct line to a licensed professional. Its FAQ describes escalation to a physician when a case falls outside agreed thresholds, information conflicts, or a possible medication-related problem arises. Those general refill-program statements should not be mistaken for Nolla-specific procedures beyond the controls described in its own agreement.
The October 5 announcement says participating organizations are subject to state oversight, must provide clear AI disclosures, carry specialty medical malpractice insurance, maintain 24-hour incident response, and preserve patients’ traditional legal remedies. Utah says pilot data will be shared with the Legislature and public to inform future regulation.

What is not yet known about safety or effectiveness
The cited state materials do not report completed safety or efficacy results for Nolla’s newly launched pilot. Oversight requirements and plans to report data are not evidence that the system makes accurate assessments or that its prescribing is safe. Utah Office of AI Policy Director Zach Boyd said in the October 5 announcement, “Our foundational approach is that technical innovation will never supersede patient safety.” The pilot’s results will matter more than the existence of safeguards alone.
There is also a broader debate about automated prescribing. In a May 13, 2026 letter, Public Citizen argued that even medication renewals can require individualized judgment about changing conditions, overdue lab work, side effects, interactions, and whether a treatment remains appropriate. The group urged substantive physician oversight, clear disclosure, and records sufficient for investigation. That is an advocacy organization’s critique of prescribing automation generally, not an official finding about Nolla or evidence that the Utah pilot has caused harm.
