Treating Hospital Wastewater: Sources, Risks and Treatment Decisions

Hospital wastewater varies by department. Effective management combines source control, collection, context-appropriate treatment and disposal under local requirements.

Hospital wastewater is managed through a system of source control, collection, treatment and disposal—not one treatment process that fits every facility. Its composition varies by department, so hospitals need to account for local wastewater conditions, sewer access, discharge requirements and the capacity to operate and maintain treatment.

What contaminants are in hospital wastewater?

There is no single hospital wastewater profile. General medical areas may produce wastewater similar to domestic wastewater, while particular departments can add substances that need separate attention. The World Health Organization’s healthcare waste handbook describes ward urine that may contain higher amounts of antibiotics, cytotoxic substances, drug metabolites and X-ray contrast media, as well as higher disinfectant concentrations. Kitchen wastewater can contain food residues, detergents, disinfectants, grease and oils. WHO’s healthcare waste handbook and its healthcare wastewater guidance discuss these source differences.

Potential concerns also include pathogens, drug-resistant microorganisms, pharmaceuticals and laboratory chemicals. Which substances are present, and at what concentrations, depends on the facility and the wastewater source; their presence should not be assumed to be identical at every hospital. WHO identifies infectious, chemical, pharmaceutical and cytotoxic, and radioactive categories within hazardous healthcare waste. These categories help describe the broader waste-management challenge, but healthcare waste includes solid and liquid streams and is not another name for wastewater. See the WHO healthcare waste fact sheet and WHO handbook.

How is hospital wastewater treated?

Treatment is one part of a management chain: identify and control hazardous inputs, collect wastewater appropriately, select treatment suited to the facility, and dispose of treated water and residuals under applicable requirements. WHO’s handbook describes treatment options for healthcare facilities without central sewerage, underscoring that decentralized facilities need context-appropriate planning rather than a universal recipe. The available WHO guidance does not establish one best treatment train or a universal design specification. WHO, Safe management of wastes from health-care activities, 2nd edition.

For a hospital connected to a centralized sewer, planning must account for the local sewer and discharge arrangements. A facility without that connection must consider an on-site or otherwise decentralized approach. In either case, the decision should be grounded in local requirements and the characteristics of the wastewater; a connection or treatment step alone does not establish that every chemical or pathogen has been removed.

What should hospitals consider before choosing an approach?

  • Where wastewater comes from: Identify department-specific inputs, including laboratory, imaging, oncology, infectious-disease and kitchen activities, rather than treating the whole facility as one uniform source.
  • Whether inputs can be managed at source: Unused or hazardous pharmaceutical products should follow appropriate pharmaceutical waste pathways, not be treated as ordinary wastewater. WHO’s 2025 pharmaceutical waste guidance addresses healthcare facilities; discharge rules remain subject to local requirements. WHO pharmaceutical waste guidance.
  • Sewer access and discharge requirements: Establish whether the facility uses centralized sewerage or needs decentralized treatment, and identify the applicable local public-health and discharge rules.
  • Operational capacity: Consider whether the hospital has the staff, resources and maintenance capacity to operate its chosen system reliably.
  • Residuals and environmental effects: Account for what treatment produces and how those residuals will be managed, alongside the impacts of disposal.

These are decision factors, not a ranking of treatment technologies. WHO’s broader guidance on alternatives such as autoclaving, microwaving and steam treatment concerns hazardous healthcare waste generally; it should not be read as a list of wastewater treatment processes. Why source management matters

Treatment planning cannot substitute for managing hazardous materials before they enter the wastewater stream. Pharmaceutical and other hazardous liquid waste require appropriate handling under healthcare waste procedures, while wastewater itself must be assessed and managed as a liquid stream. Local discharge rules determine what may enter a sewer or be released to the environment. WHO’s 2025 pharmaceutical waste guidance and healthcare waste handbook address these related but distinct responsibilities.

Poorly managed healthcare waste can release pathogens and toxic pollutants, creating risks that sound management is intended to reduce. That is a risk to manage, not evidence that every hospital discharge causes harm. WHO reported that about 85% of the total waste generated by healthcare activities is general, non-hazardous waste and the remaining 15% is hazardous. Those figures describe healthcare waste overall—not wastewater composition, treatment performance or the proportion of hospital effluent that is hazardous. WHO, 2024.
