TMS vs. ECT: How the Treatments Differ

TMS uses magnetic pulses without intentionally inducing a seizure; ECT induces a brief seizure under anesthesia. Their schedules, risks and clinical roles differ.

TMS and ECT are different clinician-delivered treatments, not two versions of the same procedure. Repetitive transcranial magnetic stimulation (rTMS, usually called TMS) uses magnetic pulses to stimulate a targeted brain area and generally does not require anesthesia or intentionally cause a seizure. Electroconvulsive therapy (ECT) uses electrical stimulation to deliberately induce a brief seizure under general anesthesia. Their roles, schedules, side effects and medical requirements differ, so the choice depends on clinical circumstances—not a universal ranking.

How are TMS and ECT different?

The clearest distinction is what each treatment does during a session. TMS stimulates neural circuits with repeated magnetic pulses. ECT delivers a controlled electrical current that intentionally produces a brief seizure. That difference shapes the procedure, the setting, the time commitment and the risks clinicians consider.

Decision point TMS (rTMS) ECT
What stimulates the brain A coil on the scalp delivers repeated magnetic pulses that induce weak electrical currents in targeted neurons and circuits associated with mood regulation. Electrodes deliver a controlled electrical current through the brain, intentionally inducing a brief seizure.
Anesthesia Generally performed without anesthesia. Performed under general anesthesia, with a muscle relaxant to reduce movement.
Seizure Does not intentionally induce one; seizure is an uncommon serious adverse effect. A brief seizure is part of the treatment.
Typical setting Usually an outpatient clinical or office setting. Outpatient or hospital care, depending on the person’s circumstances.
Common schedule A typical course may involve sessions five days a week for four to six weeks; protocol and session duration vary. Often given several times a week as a course; NIMH describes three sessions weekly, commonly six to twelve treatments.
Commonly discussed effects Scalp discomfort, headache, facial twitching or lightheadedness. Confusion, disorientation and memory problems, as well as headache, nausea or muscle aches.

These are general patterns, not a personal treatment plan. Device, protocol, health history and local practice affect the details.

What does a TMS or ECT session involve?

What happens during TMS

A clinician positions a magnetic coil against the scalp and delivers repeated pulses. The magnetic field induces weak electrical currents in a targeted area, stimulating neurons and neural circuits. Because ordinary rTMS does not involve general anesthesia or an intended seizure, a session is typically delivered in a clinic or office without the anesthesia component required for ECT.

Session length depends on the protocol: it can range from a few minutes to tens of minutes. Accelerated protocols may deliver more than one session in a day, but availability and schedule depend on the clinic and treatment approach. Ask which protocol is proposed and how much time it requires, rather than assuming every course follows the same timetable.

What happens during ECT

ECT is performed under general anesthesia. A muscle relaxant is given to reduce movement while a controlled electrical current induces a brief seizure. Electrode placement and pulse width are among the procedural choices worth discussing with the treating doctor; brief-pulse or ultrabrief-pulse approaches may be used. The clinician can explain the proposed technique and how it relates to the person’s priorities and medical situation.

ECT is usually a series of treatments, not a one-time procedure. It can be delivered as outpatient care or during a hospital stay. The care team determines the setting and schedule in light of the person’s condition and treatment plan.

When might a clinician consider one treatment over the other?

Situations in which ECT may be considered

ECT may be considered for severe or treatment-resistant depression, and when improvement is needed rapidly. Some life-threatening presentations can make speed especially important; examples include a high risk of suicide or stopping eating or drinking. ECT may also be considered in other clinical contexts, including severe mania or catatonia. These are reasons for a specialist assessment, not a checklist that establishes which treatment an individual should receive.

Situations in which TMS may be considered

TMS is used for moderate-to-severe depression when medication has not worked sufficiently or has not been tolerated. It may be a consideration after other care has not adequately reduced symptoms, but that does not mean every person with depression needs brain stimulation. Psychotherapy, medication and other care may remain appropriate depending on the situation.

FDA clearance and age criteria depend on the specific device and its current labeling. NIMH’s brain-stimulation overview describes rTMS as cleared for several indications and notes both a restriction for youth under 15 in its page text and a 2024 clearance for adjunctive depression treatment in adolescents aged 15 and older. Because those statements are indication- and device-specific, patients and families should ask the treating clinician what current labeling applies to the proposed device and use.

Why urgency changes the conversation

NIMH says brain-stimulation treatments are generally considered after psychotherapy and medication have not sufficiently reduced symptoms, while noting that they may be used earlier when severe depression becomes life-threatening. The decision depends on a person’s needs, preferences and medical circumstances. If symptoms pose immediate danger, or someone is at risk of harming themselves or cannot maintain basic safety, seek urgent professional or emergency help rather than waiting to compare routine appointment schedules.

What factors shape an individualized choice?

There is no single feature that determines the right option. A clinician considers how severe and urgent the symptoms are, what treatments have already been tried and how the person responded, and the person’s overall medical situation. Preferences matter too: one person may prioritize avoiding anesthesia, while another may be especially concerned about memory effects or the time required for frequent appointments. Those concerns do not decide the treatment on their own, but they belong in an informed discussion.

Practical details can also affect whether a course is feasible. A typical TMS schedule means attending a clinic five days a week for several weeks, although protocols vary and accelerated approaches may shorten the calendar course. ECT is commonly given several times weekly, and the treatment may be arranged as outpatient care or during a hospital stay. Ask the clinic what the proposed schedule and setting would be in your particular case; a typical pattern is not a guarantee of the plan you will receive.

How many TMS or ECT sessions are needed?

Neither treatment has one schedule that fits everyone. The following are typical patterns described by NIMH, not guarantees or prescriptions:

  • TMS: A typical course may involve daily sessions five days per week for four to six weeks. A session may take a few minutes to tens of minutes, depending on the protocol. Accelerated approaches can provide multiple sessions in one day and shorten the overall course.
  • ECT: NIMH describes treatment three times a week until symptoms improve, commonly within six to twelve treatments. ECT is generally delivered as a series, either outpatient or as part of hospital care.

The number of visits and the calendar time are not interchangeable: session duration, number of sessions, spacing and the person’s response all matter. Before starting, ask the care team for the expected schedule, how progress will be assessed and what could change the plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

It can help to ask separately about the length of an individual visit and the length of the overall course. A TMS session may be relatively brief, but the usual weekday cadence adds up to many clinic visits. ECT sessions occur less often than daily TMS in the typical schedules above, but they involve anesthesia and may be provided in a hospital or outpatient setting. The care team can explain how its local protocol compares with these general patterns.

What are the side effects and risks?

TMS: common effects and less common serious risks

Commonly described TMS effects include scalp discomfort or pain, headache, facial muscle twitching and lightheadedness. They are often mild to moderate, but the treatment is not risk-free. Uncommon serious risks include seizure and mania; hearing loss is a concern if ears are not appropriately protected. Ask what screening and protective measures the clinic uses.

Metal implants or implanted devices need to be disclosed before treatment. They may make TMS inappropriate or require specific screening, so provide the care team with details about any implants and devices rather than assuming that a treatment is suitable.

ECT: memory, anesthesia and physical effects

Confusion, disorientation and memory problems are important ECT effects to discuss, not minor details to gloss over. Some people have trouble recalling events around the treatment period. Memory symptoms often improve, but their degree and duration vary. The clinician can explain how the proposed electrode placement and pulse width relate to the treatment plan and how memory concerns will be monitored.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Headache, nausea, jaw pain and muscle aches can also occur. General anesthesia is part of ECT, and temporary increases in heart rate and blood pressure are relevant to the medical risk assessment. The care team evaluates the person’s health and anesthesia risks before treatment; a patient’s medical history can affect the balance of risks and benefits.

What to disclose and ask before treatment

  • Tell the clinician about medical conditions, medications, implanted metal or devices, prior treatment responses, and any concerns about memory or anesthesia.
  • Ask which side effects are most relevant to the proposed protocol, what monitoring and precautions are used, and whom to contact if a problem occurs.
  • Ask how benefits and adverse effects will be reviewed during the course, and what would prompt a change in treatment.

The screening questions differ in emphasis. For TMS, disclose implanted metal and devices so the clinic can assess whether the proposed equipment and protocol are appropriate. For ECT, ask about the medical and anesthesia evaluation, including how the team will account for your health history. In both cases, share previous responses to treatment and raise concerns before a course starts rather than assuming a general description covers your circumstances.

Is TMS the same as electroconvulsive therapy—or magnetic seizure therapy?

No. TMS (in the depression context, usually repetitive TMS or rTMS) uses magnetic pulses to stimulate a targeted area and does not intentionally induce a seizure. ECT uses electrical stimulation to induce a seizure under general anesthesia. The similar words “magnetic” and “stimulation” do not make rTMS a type of ECT.

Magnetic seizure therapy (MST) is also distinct from ordinary TMS. MST uses magnetic pulses to deliberately induce a seizure under anesthesia and remains under investigation. A 2023 NIMH research highlight discussed a trial comparing MST with a specific ECT protocol; that result is not evidence that ordinary TMS and ECT have equal efficacy.

Can you say whether TMS or ECT works better?

There is no universal winner established for every patient group. NIMH says rTMS does not match ECT’s therapeutic effects, while also describing the treatments in different clinical contexts. The sources do not provide a current, directly comparable response or remission percentage for TMS versus ECT across patient groups. Readers should not treat the options as interchangeable or turn a general comparison into a prediction about an individual’s outcome.

For a particular person, the relevant comparison is how likely each option is to help in that person’s clinical situation, how quickly improvement is needed, what risks matter most, and what treatment is feasible. Prior response, medical status, preferences and the care plan all belong in the clinician’s assessment. If you see a result about MST and ECT, check which treatment was actually studied: MST deliberately induces a seizure and is not ordinary rTMS.

What should you ask your clinician?

A useful appointment should leave you understanding why a treatment is being proposed and how it would fit into the broader plan. Consider asking:

  • What is the treatment goal in my case, and why are you considering TMS or ECT now?
  • How does the urgency of my symptoms affect the options and expected timing?
  • Which exact TMS device and protocol, or which ECT technique, are you proposing?
  • For ECT, what electrode placement and pulse width are planned, and how will memory effects be monitored?
  • What screening, anesthesia evaluation, seizure precautions or hearing protection apply to my situation?
  • How many sessions are anticipated, how long does each take, and how will you assess whether the treatment is helping?
  • What side effects should I report promptly, and who should I contact between visits?
  • What follow-up or continuation care would be considered if I improve?

Mayo Clinic psychiatrist Simon Kung has advised patients to discuss the risks and benefits with their health care provider and how TMS may fit into their treatment plan. That is a practical principle for either option: understand the proposed procedure, its trade-offs and the care that follows it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What happens after the treatment course?

Improvement does not eliminate the need for follow-up. After ECT, medication, maintenance ECT or both may be used. After TMS, depression care such as medication and psychotherapy may continue. Ask about continuation planning before the course begins, including how symptoms will be tracked and what the next step would be if they return or do not improve enough.

ECT and TMS are procedures delivered by healthcare professionals, not consumer devices or at-home substitutes. A qualified clinician can assess whether either belongs in an individual treatment plan and explain locally available protocols.

Frequently Asked Questions

Is TMS the same as electroconvulsive therapy?

No. Repetitive transcranial magnetic stimulation (rTMS, usually called TMS) delivers magnetic pulses to stimulate a targeted brain area and generally does not require anesthesia or intentionally induce a seizure. ECT uses electrical stimulation to deliberately induce a brief seizure under general anesthesia.

Does TMS cause seizures or memory loss?

TMS does not intentionally cause a seizure, although seizure is an uncommon serious adverse effect. Scalp discomfort, headache, facial twitching and lightheadedness are more commonly described. ECT intentionally induces a brief seizure and can cause confusion and memory problems; the degree and duration of memory effects vary.

When is ECT used instead of TMS?

A clinician may consider ECT for severe or treatment-resistant depression, or when rapid improvement is important, including some life-threatening presentations. TMS is used for moderate-to-severe depression when medication has not worked sufficiently or has not been tolerated. The right option depends on clinical assessment, urgency, medical circumstances and preferences.

How many TMS or ECT sessions are needed?

A typical TMS course may involve sessions five days a week for four to six weeks, though protocol and session length vary and accelerated approaches may offer multiple sessions in one day. NIMH describes ECT three times weekly until symptoms improve, commonly within six to twelve treatments. Individual schedules differ.

Does TMS require anesthesia?

TMS generally is performed without anesthesia and usually in an outpatient clinical or office setting. ECT is performed under general anesthesia with a muscle relaxant. Ask the clinician about the exact protocol and setting being proposed.

What should I tell the clinician before TMS or ECT?

Share your medical conditions, medications, previous treatment responses and concerns about memory or anesthesia. For TMS, disclose implanted metal and devices because they may require screening or affect suitability. For ECT, ask about the medical and anesthesia evaluation and how your health history affects the assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What happens after TMS or ECT helps?

Follow-up care still matters. After ECT, medication, maintenance ECT or both may be used. After TMS, standard depression care such as medication and psychotherapy may continue. Ask how symptoms will be tracked and what the next step would be if symptoms return or do not improve enough.

The Bottom Line

Bottom line: TMS uses targeted magnetic pulses and usually avoids anesthesia and an intentionally induced seizure; ECT deliberately induces a brief seizure under general anesthesia and may be considered when depression is severe or rapid improvement is important. Their schedules, risks and roles differ. A qualified clinician can help weigh urgency, prior treatment response, medical status, memory concerns, preferences and follow-up needs for an individual plan.
