“Therapeutic screening for Alzheimer’s disease” can mean evaluating potential treatments in laboratory research and clinical development, or screening people to see whether they qualify for a clinical trial. Those are different activities from testing an individual to diagnose Alzheimer’s. This guide explains the distinctions and how biomarkers and clinical outcomes are used.
What therapeutic screening means
There is no single standard definition of “therapeutic screening” in the cited guidance. In treatment research, it describes a sequence of evaluations: researchers study possible interventions, select candidates for further investigation, and test whether those candidates are safe and beneficial in people. In clinical trials, screening can also mean checking whether a potential participant meets a study’s eligibility requirements.

These uses should not be confused with diagnostic testing. A biomarker may help determine whether someone fits a research study or help researchers track a biological response, but that does not make it a stand-alone diagnosis or predict how that person will fare on a treatment.

Drug development is organized around the question researchers need to answer at each stage: what biological process is being targeted, which people are being studied, whether the intervention is safe, and whether it produces meaningful benefit. Guidance from the U.S. Food and Drug Administration (FDA) and European Medicines Agency (EMA) addresses clinical investigation, but with different scopes.

FDA: early Alzheimer’s drug development
The FDA’s March 2024 revised draft guidance, Early Alzheimer’s Disease: Developing Drugs for Treatment, focuses on developing drugs for sporadic Alzheimer’s before overt dementia. It discusses diagnostic criteria, clinical staging, outcome measures, and surrogate endpoints. The document is labeled a Draft Level 1 Guidance and says it is not for implementation; its recommendations are nonbinding.
The FDA describes its aim as helping developers select suitable diagnostic criteria, stages, and outcomes. A surrogate endpoint is a measure used in place of a direct measure of clinical benefit. Even when it may be reasonably likely to predict benefit, it is not itself proof that patients experience clinical benefit.

EMA: studies across disease stages
The European Medicines Agency’s guideline on clinical investigation of medicinal products for Alzheimer’s disease addresses studies across disease stages. It covers diagnostic criteria, biomarkers, trial outcomes, and the design of efficacy and safety studies. Its Revision 2 became legally effective on 1 September 2018. The page also records a Revision 3 concept paper, first published on 6 August 2025, with consultation closed on 28 February 2026.

Biomarkers are measurable indicators of biological processes. In Alzheimer’s research, they can help researchers identify participants who meet a study’s requirements and monitor how a drug or other intervention affects a biological target. A change in a biomarker, however, is not automatically evidence that memory, daily function, or another clinically important outcome improved.

The National Institute on Aging (NIA) explains that no single test diagnoses dementia. Biomarker results are considered alongside other information, and some tests are available only through specialty clinics or research facilities. Insurance coverage also varies by test and insurer. For an overview of how these tests are used, see the NIA’s explanation of Alzheimer’s diagnosis and biomarkers.
How to judge what a screening result means
The meaning of a result depends on the purpose of the test and the study or clinical setting. When evaluating a claim about a screening method, consider these points:

- Purpose: Is it identifying a biological target, assessing trial eligibility, measuring a response, or establishing clinical benefit?
- Population and stage: Which people and stage of disease were studied? Results from early Alzheimer’s research do not automatically apply to later stages.
- What is measured: Does the test measure a biological process, a symptom, or daily functioning? These are related questions, not interchangeable outcomes.
- Validation and limits: Has the measure been evaluated for the use being claimed? A biomarker can support research without diagnosing dementia or proving treatment benefit.
- Safety and relevance: Does the evidence address adverse effects and outcomes that matter to patients, as well as the biological target?
What Alzheimer’s research includes
Therapeutic research is broader than screening drug compounds. The National Institutes of Health’s inventory reported 466 active Alzheimer’s and related dementias trials in March 2025. That dated count includes clinical drug development, nondrug studies, dementia care and caregiving, studies of disease processes, diagnostic tools and imaging, and treatment of neuropsychiatric symptoms. It is a March 2025 snapshot, not a current 2026 total. The NIH inventory is available at Alzheimer’s Disease and Related Dementias Clinical Trials.

Approved medicines and experimental candidates are different
In the United States, the FDA has approved lecanemab and donanemab for early Alzheimer’s. These prescription medicines target amyloid plaques. Clinical trials showed slower cognitive decline in some people, but neither medicine cures Alzheimer’s, and both require careful medical monitoring because of potential side effects. The NIA summarizes their use and risks in its overview of Alzheimer’s treatment.
Research continues on safety, use at other disease stages, participation by more diverse populations, and combinations with other therapies. These questions do not change the distinction between medicines approved for a specified use and candidates still being studied. See the NIA’s overview of ongoing Alzheimer’s treatment research.
Screening for a trial is not a route to guaranteed treatment
A person considering research participation can ask a clinician or a trial site whether a study is appropriate and what its screening involves. Eligibility depends on that study’s criteria, which may include disease stage, diagnostic evidence, health history, and other requirements. Passing screening does not guarantee a place in a study, access to an experimental medicine, or a particular result.

Therapeutic screening can refer to distinct steps in research and clinical development, while participant screening determines whether a person may join a particular trial. Neither should be mistaken for a consumer test that predicts treatment response or outcome.
