To find the real killer in “Carnal Sins,” question Reverend Nathaniel at Crippled Kate’s before deciding what to do with him. Ask who left the letter on Patricia Vegelbud’s body; Nathaniel’s answer leads to coroner Hubert Rejk, the actual murderer. Killing Nathaniel before he gives that information loses the direct trail. Finish the optional quest before beginning The Isle of Mists, or it can be failed.
Quest overview and timing
“Carnal Sins” is an optional secondary quest that follows Cabaret. During the cabaret’s opening, Dandelion learns that Priscilla has been attacked. Go with him to Vilmerius Hospital, speak with Dandelion and surgeon Joachim von Gratz, and agree to investigate after von Gratz connects Priscilla’s attack to a similarly mutilated body at the morgue.

Complete the quest before starting The Isle of Mists; advancing to that main quest can make “Carnal Sins” unavailable. Prima Games suggests level 16 and lists 200 crowns and Axii Delusion rank 3 among preparations, but these are guide recommendations rather than hard quest-entry requirements. Investigate the mortuary and all three leads
Examine Fabian Meyer
Meet von Gratz and follow him through the sewers to the Novigrad mortuary. Use Witcher Senses to find dwarven woodcarver Fabian Meyer. His eyes have been removed and replaced with burning coals, his throat treated with formaldehyde, and his heart replaced with a scorched salamander egg. Coroner Hubert Rejk arrives during the examination. Von Gratz remarks that Hubert appears not to have aged in decades—a detail worth remembering.

Search Priscilla’s attack alley
Use Witcher Senses to find a knocked-over brazier and smell formaldehyde. The trail ends abruptly at an unusually high wall, suggesting the attacker could make an inhuman jump.
Inspect Fabian’s workshop and death scene
Examine Fabian’s workshop and the place where he was posed among sculptures made by his brother Gus. The body has much less blood than expected, another clue that the killer is not an ordinary human.
Get the note from Eustace
Speak to Eustace, the corpse collector, who robbed Fabian’s body and initially refuses to cooperate. The guide lists two ways to obtain his note: use Axii Delusion rank 2 or pay 200 crowns. The note is written on human skin and names Priscilla as the next intended victim.
Investigate all three leads to receive 200 XP, according to the Witcher.tools reward listing. Taken together, the scant blood, impossible jump, and Hubert’s unchanged appearance point toward a supernatural killer.
Follow the trail to Patricia Vegelbud
Hubert brings news of another victim, theology lecturer Joris Aquinus, and the investigation leads to Patricia Vegelbud. Go to the Vegelbud residence, follow Ingrid to Patricia’s room, then pursue the fleeing killer. Guards may stop Geralt during the chase; Ingrid clears up the misunderstanding. The letter found on Patricia points to Sweet Nettie at Crippled Kate’s.
Question Nathaniel before choosing what to do
Find Nettie upstairs. Reverend Nathaniel Pastodi is torturing her, making him appear to be the killer. Do not attack him before questioning him if you want the direct route to Hubert. Keep the conversation going and specifically ask who left the letter on Patricia’s body. Nathaniel says Hubert was the only person who knew where Nathaniel would be and points Geralt to warehouse twelve, where Hubert is buying formaldehyde. Some dialogue choices can end with Nathaniel’s death before he reveals this information. Prima Games’ branch walkthrough describes the conversation and confrontation.

Choose Nathaniel’s outcome
- Use Axii Delusion rank 3: Geralt can make Nathaniel leave Nettie alone; the walkthrough says Nathaniel leaves unharmed.
- Kill Nathaniel: The Witcher.tools reward listing associates his death with 100 XP, Tracker’s trousers, and the Ard Skellig aketon diagram.
- Leave him with Nettie: Nathaniel claims he will cause no permanent harm, but Nettie remains with him for the moment. This is the consequence described by the quest source, not a risk-free rescue.
If Nathaniel has already named Hubert, you can still follow that lead and confront the real killer. If you kill him before questioning him, the quest can still reach a failure-style resolution, but the direct investigation is lost; a later confession note can reveal the truth. Travel to warehouse twelve near Novigrad’s docks using Nathaniel’s lead. Hubert reveals that he is the murderer and a vampire. Defeat him to solve the case. The Witcher.tools guide lists 500 XP, the Assassin’s boots diagram, and a Lesser Svarog runestone for killing Hubert. Prima Games also describes Hubert’s transformation during the fight.
Return to the hospital
After the successful confrontation, return to Vilmerius Hospital and speak with Dandelion about Priscilla. The walkthroughs report that she is recovering and able to speak again, closing the successful route.

Best route if you want to solve the case
- Complete Cabaret and begin “Carnal Sins” at the hospital.
- Examine Fabian at the mortuary, then investigate the alley, workshop and Eustace’s note.
- Follow the trail to Patricia Vegelbud and take her letter to Crippled Kate’s.
- Question Nathaniel fully. Ask who left the letter before choosing his fate.
- Use Axii Delusion rank 3 if you want the walkthrough-supported option to make Nathaniel leave Nettie unharmed; otherwise choose whether to kill him or leave him with her.
- Go to warehouse twelve, defeat Hubert, then return to the hospital and speak with Dandelion.
Keep the two Axii checks distinct: rank 2 is listed as one way to get Eustace’s note, while rank 3 is the option for making Nathaniel leave Nettie.
