The More Ultra-Processed Food People Ate, the Higher Their Risk of Disease and Death

People who ate more ultra-processed food (UPF) tended to have higher rates of several diseases and death in large observational studies. A 2026 analysis pooling 51 prospective cohorts found higher all-cause mortality among people with higher intake, but its hazard ratios describe population-level associations—not proof that UPFs caused those outcomes or a prediction of any one person’s risk.

What the studies found

The largest analysis in the evidence reviewed, published in Family Medicine and Community Health in 2026, pooled 51 prospective cohorts covering 8,819,894 adults. It reported associations between UPF intake and cardiovascular events, cancer, overweight or obesity, metabolic syndrome or diabetes, depression or anxiety, digestive diseases, hypertension, and all-cause mortality. The cohorts included participants from the Americas, Europe, Asia, and Oceania, with follow-up periods ranging from 2 to 32 years. Read the 2026 analysis.

For all-cause mortality, the analysis estimated a hazard ratio (HR) of 1.18 (95% confidence interval [CI] 1.10–1.26) when comparing higher with lower intake. An HR compares the rate at which an outcome occurs over time between groups. This estimate is a relative comparison: it does not mean an 18-percentage-point increase in an individual’s chance of dying, or that 18% more people in the higher-intake group died.

The same analysis also estimated associations per additional 100 grams of UPF consumed per day. These are pooled observational dose-response estimates; they should not be read as the effect of deliberately adding that amount to an individual diet.

These estimates come from the 2026 pooled analysis and apply to its study populations and methods; each is a relative hazard, not an absolute change in risk.

How to compare the mortality estimates

Other reviews also found an association between greater UPF intake and mortality, but their pooled estimates use different groups of studies and exposure comparisons. They are related findings, not interchangeable measurements.

The 2025 meta-analysis also estimated HR 1.10 (95% CI 1.04–1.16) for each 10% increment in UPF consumption, with I²=91%. I² describes variation among study results beyond what would be expected from chance alone; the high values in that review indicate that estimates varied considerably between studies. Differences in populations, dietary measurement, exposure definitions, and outcomes can all complicate comparisons.

What counts as ultra-processed food?

The studies generally classified foods using NOVA, a system that groups foods by the extent and purpose of processing. UPFs are a broad category, not a single nutrient profile or one uniform food. Items grouped together can differ substantially in ingredients and nutritional composition, so the association for the category does not establish that every food within it has the same relationship to health.

In the 2026 analysis, intake was assessed using food-frequency questionnaires, 24-hour recalls, or dietary histories. The authors noted that applying NOVA across different dietary instruments can lead to variation in how foods are classified. That measurement difference is one reason the estimates should be understood as broad patterns rather than precise effects of a specific product.

How certain is the evidence?

Prospective cohort studies measure intake before later health outcomes, helping establish that exposure preceded the outcome. But they are observational, not randomized trials. People with different diets may also differ in other ways that affect health; dietary reports can be inaccurate, eating habits may change during follow-up, and results may differ across populations. These factors can influence the observed associations, so the studies do not establish that UPFs caused each condition.

The 2026 analysis rated evidence certainty as low for cardiovascular events, cancer, metabolic syndrome or diabetes, depression or anxiety, digestive diseases, and all-cause mortality; it rated certainty as moderate for overweight or obesity and very low for hypertension. These ratings reflect the limitations of the evidence for each outcome, not whether an association was reported.

Reviews can reach different evidence classifications because they include different studies, define outcomes differently, and use different grading approaches. A 2024 BMJ umbrella review classified evidence for cardiovascular disease-related mortality and type 2 diabetes as convincing under its own criteria, while rating GRADE certainty very low for the cardiovascular mortality association and moderate for the dose-response association with type 2 diabetes. A separate 2024 Clinical Nutrition umbrella review assessed 39 meta-analyses, updating them with 122 individual articles across 49 outcomes. Its strongest evidence classifications differed, identifying convincing evidence for renal function decline and wheezing in children and adolescents. That difference is a reminder that a review’s headline classification depends on its scope and method; it does not cancel out the findings of another review. What readers can take from the findings

  • Across large observational datasets, higher UPF intake is associated with higher rates of several health outcomes, including all-cause mortality.
  • The size of an HR is a relative comparison between groups, not a personal absolute-risk estimate.
  • The results describe a broad food category and do not show that every item in it has the same nutritional profile or health association.
  • Because the evidence is observational and certainty varies by outcome, these findings do not prove that UPFs cause disease or establish the effect of changing one person’s diet.
