Its strongest callbacks are to Resident Evil 4 healing items and supplies, while its outbreak-night setting and survival-horror structure draw heavily on Resident Evil 2. Some details are confirmed by director Zach Cregger; others are visual parallels identified by reporters or remain speculative.
First, what story is this movie telling?
The film follows Bryan, an original protagonist caught in a night of chaos. Sony’s official film page describes him as a medical courier and lists the theatrical release date as September 18, 2026. Cregger has said he did not want to retell a game or adapt Leon’s story, which he felt would be redundant beside the games.

Instead, he described the film as a journey that could happen on the sidelines of a game. He says the world of Resident Evil 2 is where the movie takes place, with “a couple of little shifts for dramatic license.” That makes the callbacks a mixture of direct prop references, familiar franchise imagery, and filmmaking choices that recreate the feel of playing a survival-horror game.

The clearest Resident Evil 4 callbacks
The green herb and its pot
The most specific visual reference is a green herb in a black vase with a gold stripe. GamesRadar identifies it as matching the herb presentation in Resident Evil 4, and reports that Cregger said he replicated the healing item and pot. Cregger also said he took healing items from Resident Evil 4 and mimicked them exactly. This is more than a generic nod to the series’ healing herbs: it points to a recognizable game item and presentation.
Healing items beyond the herb
Cregger explicitly said the film borrows healing items from Resident Evil 4. GamesRadar’s inventory also points to healing sprays and herbs among the game-like objects that appear. The props matter because they connect to the movie’s survival logic: health is something Bryan has to manage, not take for granted.
Heavy Field shotgun shells
GamesRadar identifies a box of Heavy Field shotgun shells in a drawer as the same brand shown in Resident Evil 4. This is a reporter-identified prop reference, rather than a separately quoted confirmation from Cregger.

The movie turns game mechanics into story beats
Counting bullets and choosing what to carry
Cregger recalled the resource conservation in Resident Evil 2: players track ammunition and healing items and decide what to carry. He wanted that deliberate scarcity in the movie. The Atlantic also reports a scene in which Bryan checks his ammunition, framing the moment as part of the series’ broader resource-management tradition. The point is not just that supplies appear on screen; Bryan must treat them as limited.
From pistol to shotgun to MP5
Cregger describes a progression from pistol to shotgun to MP5, echoing the way a game can gradually give a player more capable weapons. He pairs that progression with Bryan’s lack of combat training: the protagonist does not move through danger like an action hero who already knows how to fight. The changing weapons therefore signal both escalation and the continuing vulnerability of the person using them.

- In San Francisco, Jill Valentine is dealing with a zombie outbreak, Leon Kennedy is tracking a kidnapped DARPA scientist and Claire Redfield is investigating a monstrous fish. Joined by Chris Redfield and Rebecca Chambers, they discover the trail of clues from their separate cases all converge on Alcatraz Island, where a new evil awaits their arrival.
Searching rooms and opening routes
GamesRadar compares Bryan taking a shotgun from a wall and searching drawers for ammunition to situations in the first Resident Evil and Resident Evil 4. Treat that as a gameplay and visual parallel, not a separately confirmed statement of intent. The broader pattern is familiar: search the room, find what might keep you alive, and move on when the route opens.

He singled out Resident Evil 2’s police station as an effective model because players unlock new areas and move onward to places such as garages, with each location bringing another danger. GamesRadar reports that the film cycles through settings and includes typewriters, sprays, herbs, padlocks, and weapon progression—details that help give the trip a level-by-level rhythm.

Franchise imagery: lab, sewers, and a mutated dog
The Umbrella laboratory
GamesRadar describes the hospital laboratory as resembling the recurring labs seen across the games. It is recognizable Resident Evil imagery, but the reporting does not establish it as a reference to one particular room or installment.
The sewer passage
Bryan’s passage through a sewer recalls the sewer sections of Resident Evil 2, according to GamesRadar. This connection fits Cregger’s broader description of the film’s setting, but it is best understood as an environmental echo rather than a precise recreation.
The mutated dog
GamesRadar compares the creature’s tentacles and teeth to mutated dogs in Resident Evil 4 and Resident Evil 5. That is a design comparison; it does not establish that the movie’s creature is literally a named enemy from either game.

Possibly, but it is the least certain connection. GamesRadar notes that Resident Evil 4 includes a puzzle involving a cuckoo clock, while also cautioning that clocks are common horror-film devices. Without stronger confirmation, the clock is a possible nod—not a confirmed Easter egg.

Which references are confirmed—and which are interpretation?
Cregger summed up his approach as: “So I would rather just kind of celebrate everything I love about the games by telling the story that could exist on the sidelines of one of the games.” He also said, “Yeah, there’s a lot of easter eggs for the games, and that goes beyond things like the weapon progression and the resource management.” For GamesRadar, he described the aim as being “loyal to the games in spirit and in tone” while building a story that puts the audience in that scenario again with increasing stakes.
- Director-confirmed: the film’s relationship to the Resident Evil 2 world; its resource-management approach; and healing items borrowed from Resident Evil 4.
- Identified by reporting: the Heavy Field shells, shotgun-and-drawer parallels, sewer and lab imagery, and the mutated dog’s resemblance to game creatures.
- Speculative: the cuckoo clock as a nod to an Resident Evil 4 puzzle.
For anyone who wants to inspect the most specific item references in their original context, Cregger said Resident Evil 4 was the game he played most. The film’s approach, though, is to borrow recognizable objects and survival-horror rhythms for an original story rather than reproduce a game’s plot.
