Evidence does not become useful for a decision simply by passing a universal pass/fail gate. In GRADE, a threshold or range makes the question decision-relevant: assessors consider how certain they are that the true effect lies on a specified side of that threshold or within the range. The threshold clarifies what matters; it does not replace judgment about the evidence, outcomes, or context.
What “evidence standard” and “gate” mean here
“Evidence standard” can refer to different systems in clinical care, law, research, or statistics. This article uses GRADE, a framework for rating certainty in evidence used in health guidance and evidence assessment. In this context, a “gate” is best understood as a decision threshold or range—not a universal rule that makes evidence count or not count in every field.

The GRADE Working Group describes certainty as “the certainty that a true effect lies on one side of a specified threshold, or within a chosen range.” The threshold gives the assessment a decision-relevant reference point; it does not turn certainty into a simple binary verdict. GRADE rates certainty across a body of evidence for each critical or important outcome, rather than awarding a single study a stand-alone quality badge. An assessment for one outcome may differ from the assessment for another. For example, confidence in evidence about a benefit need not be identical to confidence in evidence about a harm. The four certainty categories
- High: The evidence provides strong confidence in the effect estimate.
- Moderate: There is reasonable confidence, but the true effect could differ meaningfully from the estimate.
- Low: Confidence in the effect estimate is limited.
- Very low: Confidence is very limited.
These categories describe certainty in the evidence for an outcome; they are not ratings of whether a technology, treatment, or recommendation is “good.” Cochrane’s handbook explains the categories and the considerations used to assess them. Cochrane Handbook, chapter 14.
How the threshold makes certainty relevant to a decision
An effect estimate alone does not answer whether an effect is large enough to matter. A threshold or range lets assessors ask whether the true effect is likely to fall on a decision-relevant side of a boundary, or within a range of magnitudes. The answer depends on both the evidence and the chosen boundary.

For a guideline, the threshold should be interpreted in context: which outcomes are critical, and how much do decision-makers value them relative to one another? The GRADE Working Group distinguishes this fully contextualized approach from less contextualized effect-magnitude ranges that may be useful in systematic reviews or health technology assessments. What can lower certainty
GRADE commonly considers five reasons confidence in an effect estimate may need to be lowered. Their importance depends on the evidence body and outcome being assessed; a concern does not automatically have the same impact in every case. Cochrane Handbook, chapter 14; WHO, Guidance on evidence, 2025.
- Risk of bias: Limitations in how studies were conducted or reported could distort their findings.
- Inconsistency: Results vary across studies in ways that reduce confidence in a common effect estimate.
- Indirectness: The evidence does not fully match the population, intervention, comparison, or outcome relevant to the question.
- Imprecision: The estimate is uncertain enough that plausible effects may fall on different sides of a decision-relevant threshold.
- Publication bias: The available evidence may not represent all relevant studies or results.
Why a study-design hierarchy is not a decision gate
GRADE uses study design as a starting convention, not an automatic verdict on whether evidence counts. In the ACIP GRADE Handbook, randomized controlled trials initially start at high certainty and nonrandomized studies traditionally start at low certainty. Assessors then consider the evidence and relevant domains; the initial position does not guarantee the final rating. CDC’s ACIP GRADE Handbook.

A simple pass/fail gate or rigid hierarchy can obscure the question GRADE is designed to make explicit: how confident should decision-makers be in the evidence for this particular outcome, given its limitations and the threshold that matters to the decision?
How certainty relates to recommendations
Certainty informs recommendations, but does not dictate them by itself. A recommendation also depends on the outcomes that matter and their relative value in the relevant context. A threshold helps connect an effect estimate to that decision; it cannot settle value judgments or make every decision identical across settings. GRADE Working Group, 2017; CDC’s ACIP GRADE Handbook.
