You cannot change your age, family history, or a past stroke, but many important risks—including high blood pressure, smoking, diabetes, cholesterol, and some heart conditions—can be managed with appropriate care and support. A risk factor raises the likelihood of stroke; it does not predict that you will have one, and having no known risk factors does not make the risk zero.
Which stroke risk factors can be managed?
“Manageable” does not mean every factor is fully within your control. Access to healthcare, medication, and other circumstances shape what steps are possible. A clinician can help decide what matters most for your health and what response is appropriate.

High blood pressure
High blood pressure is a leading cause of stroke, and the National Institutes of Health describes it as the most powerful risk factor. It often has no symptoms, so feeling well is not a reliable way to know whether your blood pressure is high. The CDC advises getting it checked often; lowering high blood pressure through lifestyle changes or medicine can reduce stroke risk. Discuss your readings and treatment options with a healthcare professional.
Smoking and other tobacco exposure
Smoking and exposure to secondhand smoke are stroke risks. Quitting smoking and reducing exposure can be part of risk management; a healthcare professional can help identify support. NIH also lists other drug use as a risk factor.

Diabetes, cholesterol, and heart conditions
Diabetes and high cholesterol can contribute to stroke risk. Heart conditions matter too, including atrial fibrillation, an irregular heart rhythm. These are reasons to discuss screening, treatment, and follow-up with a healthcare professional rather than trying to manage a diagnosis alone.
Activity, weight, and diet
Physical inactivity, obesity, and diet are among the factors associated with stroke risk. A diet high in sodium can raise blood pressure, which is itself an important risk factor. Advice should fit a person’s health and circumstances; the point is to identify areas where a realistic change or clinical support may help.

Age
Stroke risk increases with age, but stroke can happen at any age, including in children and younger adults. The CDC says the chance approximately doubles every 10 years after age 55. It also reports that about one in seven strokes occur among people ages 15 to 49; this figure appears in CDC guidance published in 2024, which cites an earlier study, and is not a current-year estimate or an individual prediction.

Family history and inherited predisposition
You cannot change your family history. It may reflect inherited predisposition, but family members can also share environments and behaviors, so it does not by itself establish why risk is higher. Tell your healthcare professional about relevant family history when discussing prevention.
A previous stroke or TIA
A past stroke or transient ischemic attack (TIA) cannot be undone, and a history of either raises concern about another stroke. However, subsequent risk can be addressed with medical care. A TIA is a medical emergency: the American Stroke Association says it requires immediate follow-up with a healthcare professional.

Sex, pregnancy history, and differences among populations
Observed stroke risks differ by sex and across racial and ethnic groups, but those differences should not be treated as simple or inevitable biological destiny. The CDC and American Stroke Association also point to family, environmental, and societal influences, including barriers that can affect related health conditions and access to care.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to turn risk factors into a practical plan
- Ask about blood pressure. Arrange regular checks, since high blood pressure may have no symptoms. A home blood pressure monitor can help you track readings, but buying or using one alone does not prevent stroke. Share readings with a healthcare professional and ask what they mean for you.
- Review relevant medical conditions and history. Bring up diabetes, cholesterol, heart rhythm conditions such as atrial fibrillation, pregnancy-related history, and any prior stroke or TIA. Ask what follow-up or treatment is appropriate.
- Discuss changes and support that fit your situation. Smoking, activity, diet, alcohol use, and other factors can be part of the conversation. A plan may involve behavior changes, medical care, or both.
- Follow prescribed treatment with professional guidance. Do not stop a prescribed medicine or change contraception or hormone therapy without first speaking with the clinician who helps manage your care.
What risk statistics can—and can’t—tell you
The National Heart, Lung, and Blood Institute says controllable factors account for 82% to 90% of all strokes. That is a broad population estimate, not a promise that a particular person can prevent that share of their own risk. Likewise, a risk factor raises likelihood but cannot say whether an individual will have a stroke. Risk management is worthwhile even though it cannot guarantee an outcome.
