To open the Transfusion Door in Silent Hill: Townfall, match Sarah Watson, Matthew Clark, and George McEwan’s blood bags to the patient notes, route H.M., S.W., and T.F. into the center of the dial, then activate the Ordell control. Simon is A+, and the cited guide lists O−, O+, A−, and A+ as compatible blood types.
Collect the three bags for the final door
The Medical Center sequence has five blood bags. Alan Duncan’s and Lesley Greer’s bags are used earlier to open the west and east wings. Keep track of the other three—Sarah Watson, Matthew Clark, and George McEwan—for the Transfusion Door.

Match each bag to the patient note
At the final bag-placement wall, use the names and symptom descriptions on the in-game notes rather than relying on slot numbers from a guide. Guides can number the positions differently.

- Sarah Watson: “Rhythm holds. Head doesn’t.” Match this to the head-wound note.
- Matthew Clark: “Words scatter. Body complies.” Match this to the note describing confusion and verbal difficulty while obeying commands.
- George McEwan: Match his burns description to the burns note.
For the relative positions, VGC places Sarah in the left-side machine beside the head-wound report, Matthew in the adjacent left-side machine, and George in the remaining machine on the right. The note text is the safer guide if the arrangement or numbering seems different.

Route the blood lines and activate Ordell
- At the rotating blood-routing dial, turn the rings until H.M., S.W., and T.F. connect to the center. Simon’s A+ blood type can receive O−, O+, A−, and A+.
- When the lines are aligned, activate the central Ordell control. The cited guides show the interaction as X/A; follow the prompt displayed by your game if it differs.
The puzzle therefore has two required parts: placing the three correct bags and routing the eligible donor lines. Solving only the initials does not complete the sequence if the bags have not yet been matched and placed.

Townfall has another wheel-based door puzzle earlier in Room 204. That door uses a date. The Transfusion Door is the blood-routing dial in the altered Medical Center, so a date solution will not open it.
