Should People With Asthma Switch to Lower-Carbon Inhalers?

Lower-carbon asthma inhalers may be suitable for some people, but a switch should preserve asthma control and be discussed with a healthcare professional.

Not automatically. UK guidance says clinicians should consider environmental impact when choosing among inhalers that are already suitable, while also weighing asthma control, correct technique, patient preference and whether the person can use the device. Do not stop or change a prescribed inhaler without speaking to a GP, respiratory nurse or pharmacist.

What is a lower-carbon inhaler?

Pressurised metered-dose inhalers (pMDIs) use propellants associated with greenhouse-gas emissions. Dry powder inhalers (DPIs) and soft mist inhalers (SMIs) do not use those propellants and are generally described as lower-carbon options. That is a broad comparison of device types, not a universal footprint figure for every medicine or inhaler.

The joint BTS/NICE/SIGN patient decision aid says switching to a DPI offers the greatest climate benefit among the options it compares. Keeping the same inhaler type while reducing its environmental impact can also help, but offers less benefit in that comparison. Climate impact is only one part of the decision.

How should an inhaler be chosen?

The 2024 BTS/NICE/SIGN guideline recommends basing asthma inhaler choice on correct technique, the person’s preference, the lowest environmental impact among suitable devices, and an integral dose counter. In practice, an inhaler with a lower footprint is not a good choice if the person cannot use it properly or it does not support their asthma treatment.

  • Technique: the person needs to be able to use the device correctly.

  • Dose counter: the guideline includes an integral counter as a selection consideration.
  • Environmental impact: consider the lowest-impact option among those already suitable.

If a device is changed, the guideline says to observe the person’s technique and find another alternative if they cannot use it correctly. Ask the clinician or pharmacist to demonstrate the new device and check your technique.

Could a dry powder inhaler suit you?

A DPI is breath-actuated: the user must inhale strongly enough to draw the medicine in. Some children, older people and people with more severe lung conditions may find that difficult, particularly when symptoms are bad. A healthcare professional can assess whether a DPI or another option is suitable and show you how to use it.

What should you do before changing inhalers?

  1. Keep taking your prescribed inhaler. The BTS/NICE/SIGN patient decision aid says: “Do not stop using the inhalers you already have without talking to your healthcare professional.”
  2. Discuss options with a GP, respiratory nurse or pharmacist. Ask whether a lower-carbon device is suitable for your medicine, asthma control and ability to use it.
  3. Get instruction if the device changes. Have a healthcare professional observe your technique with the new inhaler and help identify an alternative if needed.

Do spacers affect an inhaler’s carbon impact?

NICE says a spacer should usually be prescribed with a metered-dose inhaler, particularly for children. A spacer is an accessory for use with a prescribed inhaler; it does not change the inhaler’s carbon impact and is not a substitute for prescribed treatment. Ask a healthcare professional whether one is appropriate rather than buying one on the assumption that it is needed.

How should used or expired inhalers be disposed of?

NICE recommends pharmacy disposal, and NHS England encourages returning inhalers to pharmacies. Follow the arrangements at your local pharmacy.

What does the NHS carbon-footprint figure mean?

NHS England estimates that the NHS’s inhaler-related carbon footprint is approximately 3% of the NHS carbon footprint. The source page does not state the year for this estimate. It describes an organizational share, not an individual patient’s emissions.
