Spoilers for Zach Cregger’s 2026 theatrical film Resident Evil. Bryan delivers the substance the hospital needs to make an antidote, but he transforms before the scientists can save him. What happens at the end of Resident Evil?
Bryan, a medical courier, reaches Raccoon City General Hospital carrying a substance needed by the scientists trying to synthesize an antidote. He is already infected, but hides how far the illness has progressed while they work. Before they can treat him, his transformation accelerates: his body develops tentacles and a mouth, and he attacks the scientists.

During the final struggle, Bryan and the last scientist fall out of the hospital. The vial drops toward the ground, but the film does not show whether it breaks. Bryan ends up transformed on the hospital sky bridge, roaring as a helicopter approaches. The movie does not show what happens to the helicopter crew or whether anyone can still use the antidote.
Sony describes the film as an “all-new story” following Bryan, played by Austin Abrams. It is scheduled for theatrical release on September 18, 2026, according to Sony Pictures’ official film page.

Who dies in the movie?
The reported casualties include the infected woman Bryan runs over, the police officer who stops him, the woman’s infected husband and dog, and the infected girl Bryan shoots after she attacks him. The two Umbrella employees who find Bryan are also killed. During his transformation at the hospital, Bryan kills the scientists. The story does not provide a complete count of everyone killed in the city’s outbreak.
Bryan’s fate is a loss of his human self: he completes his transformation and is presented as a monster. The ending does not show him cured or restored to human form.
Are Resident Evil game characters in the film?
No game characters appear as characters in the film, according to GamesRadar’s spoiler explainer. Bryan is an original character. Director Zach Cregger places the story in the world of Resident Evil 2, but follows a different person during the night events go wrong in Raccoon City. In an interview with PlayStation.Blog on April 30, 2026, he said: “So the world of [Resident Evil] 2 is kind of where this takes place, even though I’d make a couple of little shifts for dramatic license.”

That makes the film a parallel story, not a scene-by-scene adaptation of the game. GamesRadar notes that Bryan’s final monster form may visually recall William Birkin or Nyx, but this is a resemblance, not a confirmed identity or cameo.

Does the ending set up a sequel?
The ending creates room for another story, but it does not confirm one. The antidote may have been lost, and the outbreak is still active in the closing image. The film does not reveal whether the vial survived, whether the cure can be made again, or who might continue the story.
GamesRadar reported that no sequel had been confirmed at the time of its spoiler explainer, while noting Cregger had ideas for other directions the story could take. Ideas and an unresolved ending are not a production announcement; sequel status can change.

How the released ending differs from an earlier version
Cregger told TheWrap on September 22, 2026 that an earlier version of the ending had the scientist drop and break the needle early in the attack. He explained: “Originally in the script, that scientist pulls the needle out, and as soon as Austin grabs the woman’s face, he drops it and it shatters. And then the rest of the movie is just Austin killing the scientists.” In the released version, the vial’s fate on impact remains unresolved.
