Rare study reveals a striking divide inside an Alzheimer’s patient’s brain

A donated Alzheimer’s brain showed neighboring regions with sharply different amyloid, tau and tissue damage after prolonged aducanumab treatment. Researchers say the pattern supports early, thorough amyloid removal as a hypothesis—not a proven guarantee.

A single donated brain has given researchers an unusually close comparison: cortical ridges that had lost most amyloid plaques contained little tau, while neighboring grooves still packed with amyloid showed more tau, inflammation and tissue degeneration. The finding supports—but does not prove—the idea that early, extensive amyloid removal can limit later Alzheimer’s damage.

What did the autopsy reveal?

The study examined brain tissue from one man who had Alzheimer’s disease and had received prolonged treatment with the anti-amyloid antibody aducanumab. The contrast was not between two different patients or even distant parts of the brain. It appeared in adjacent cortical structures within the same brain.

Gyri had little remaining pathology

Gyri are the raised ridges created by the brain’s folded outer layer, the cortex. In the donated tissue, these ridges had lost most of their amyloid plaques and contained minimal tau accumulation.

Sulci retained amyloid and showed more damage

Sulci are the grooves between gyri. Nearby sulci retained considerable amyloid and contained substantially more tau tangles, inflammation and neurodegeneration. The uneven pattern also matched scans obtained while the participant was alive, which had shown that amyloid clearance was not uniform across his brain.

Why is a within-brain comparison important?

Alzheimer’s studies often compare different people, but patients vary in genetics, disease duration, health and treatment exposure. This case places regions with different amyloid-clearance histories side by side in one person. That reduces some sources of variation and makes the relationship between residual amyloid and other forms of pathology easier to see.

Edward B. Lee, MD, PhD, a co-senior author, described it as a “Goldilocks” situation: some regions cleared amyloid extensively while others did not. The comparison does not remove every uncertainty, because the regions may have differed before treatment and the study cannot reconstruct all of the biological events that produced the final pattern.

Who was the patient and what treatment did he receive?

The tissue came from a man diagnosed in his fifties with mild cognitive impairment caused by Alzheimer’s disease. He enrolled in an aducanumab clinical trial and received 30 doses over four and a half years. He died about four years after his final dose, more than a decade after his first symptoms, and had requested that his brain be donated to research. The tissue was provided to the University of Pennsylvania’s Center for Neurodegenerative Disease Research.

  1. Diagnosis: Mild cognitive impairment attributed to Alzheimer’s disease in his fifties.
  2. Trial treatment: 30 aducanumab doses over 4.5 years.
  3. After treatment: Death occurred about four years after the last dose.
  4. Study material: Donated postmortem brain tissue examined alongside his earlier imaging.

Does clearing amyloid stop tau from building up?

This case suggests that extensive amyloid removal may constrain subsequent tau accumulation, but it does not establish that amyloid clearance always stops tau or that one process directly caused the other. The regions with little amyloid also had little tau, whereas amyloid-rich grooves had more tau and tissue injury. That association is consistent with the hypothesis that amyloid helps drive later tau-related degeneration, especially when amyloid is removed early enough.

Christopher A. Brown, MD, PhD, the first author, noted that patients may accumulate amyloid for years before symptoms appear. The authors say the findings therefore support studying whether removing amyloid early and thoroughly can limit the changes that ultimately damage brain cells.

Can an anti-amyloid drug protect the brain after treatment ends?

The patient’s brain still showed a regional difference about four years after his final aducanumab dose, but the autopsy cannot determine how much protection persisted because of treatment, how long it lasted or whether it affected his cognition. The tissue records an end point, not a controlled experiment with repeated measurements after treatment stopped.

Most clinical trials follow participants for roughly 18 months. A possible effect lasting years beyond dosing would therefore require larger studies with substantially longer follow-up. No percentage of amyloid clearance, tau reduction or clinical benefit was reported for this case, so the findings cannot be converted into a treatment effect size.

How does this case fit current Alzheimer’s medicines?

Aducanumab was the first drug in its class to receive accelerated approval from the U.S. Food and Drug Administration, but it is no longer marketed. Penn Medicine’s account identifies donanemab (Kisunla) and lecanemab (Leqembi) as newer anti-amyloid medicines with full FDA approval. All are prescription therapies; this autopsy is not a recommendation to start, stop or seek any medicine without a clinician.

Medicine Regulatory context described by Penn Medicine What this autopsy can and cannot show
Aducanumab First anti-amyloid drug in the class to receive FDA accelerated approval; no longer marketed. The patient received 30 doses, but one autopsy cannot establish its causal effect or define a durable benefit.
Donanemab (Kisunla) Newer anti-amyloid medicine with full FDA approval. The case is not a head-to-head comparison and provides no outcome data for donanemab.
Lecanemab (Leqembi) Newer anti-amyloid medicine with full FDA approval. The case is not a head-to-head comparison and provides no outcome data for lecanemab.

What does the study not prove?

  • It does not prove that aducanumab caused the healthier appearance of the gyri.
  • It does not identify a threshold of amyloid removal needed to limit tau.
  • It does not show that every patient will clear amyloid unevenly or experience the same regional protection.
  • It does not establish how the imaging and tissue findings relate to the patient’s day-to-day cognitive course.
  • It does not replace evidence from randomized trials or justify changing treatment recommendations on its own.

The authors’ interpretation is narrower: the case provides rare human evidence that extensive amyloid removal and lower tau burden can occur together in neighboring regions, and it motivates studies of early treatment, clearance completeness and regional biology.

What should patients and families take from the finding?

The most useful takeaway is that amyloid removal may not be an all-or-nothing event across the brain. In this patient, areas that cleared more amyloid had less tau and tissue damage than adjacent areas that did not. That pattern makes the timing and completeness of clearance important research questions, but it does not predict an individual patient’s response.

People considering an anti-amyloid medicine should rely on an individualized discussion with a qualified clinician about eligibility, expected benefits, monitoring and risks. The autopsy adds biological context; it does not provide a new prescription, a guarantee of protection after treatment, or proof that one drug is superior to another.
