Plague vs. Pneumonic Plague: Symptoms, Spread, and Treatment

Plague has several clinical forms; pneumonic plague affects the lungs and is the only form that can spread between people. Know the warning signs and why suspected plague needs urgent care.

Yersinia pestis causes plague, which can take bubonic, septicemic, or pneumonic forms. Pneumonic plague is the form that affects the lungs; it can arise when infection reaches the lungs from another form or after someone inhales infectious droplets. It is also the only form that can spread from person to person. Suspected plague is a medical emergency: seek care immediately, because treatment should begin as soon as clinicians suspect it, not after final test results.

What is the difference between plague and pneumonic plague?

They are not separate diseases or bacteria. Plague is the infection; bubonic, septicemic, and pneumonic describe its clinical forms. The forms differ in the parts of the body affected, common exposure routes, symptoms, and whether person-to-person spread is possible.

Form Main features Common exposure or development Person-to-person spread
Bubonic Fever, headache, chills, weakness, and one or more swollen, painful lymph nodes called buboes. CDC gives an incubation period of 2–8 days. CDC clinical signs Usually follows a bite from an infected flea. CDC clinical signs Not through ordinary person-to-person contact; pneumonic plague is the form that can spread between people. CDC transmission
Septicemic Fever, chills, extreme weakness, abdominal pain, and shock; bleeding into the skin or organs may occur. CDC clinical signs May be the first form or develop from untreated bubonic plague. Exposure can follow an infected flea bite or contact with an infected animal. CDC clinical signs CDC transmission Not through ordinary person-to-person contact; pneumonic plague is the form that can spread between people. CDC transmission
Pneumonic Rapidly developing pneumonia with fever, headache, weakness, shortness of breath, chest pain, and cough; mucus may be bloody or watery. CDC clinical signs Can follow inhalation of infectious droplets or occur when plague from another form reaches the lungs. CDC clinical signs Yes. Infectious respiratory droplets can spread it during close contact. CDC transmission

What are the symptoms of pneumonic plague?

Pneumonic plague can cause fever, headache, and weakness along with pneumonia that develops rapidly. Respiratory symptoms include cough, chest pain, and shortness of breath; mucus can be watery or bloody. These symptoms require urgent assessment, particularly when there has been a plausible exposure to plague.

Primary and secondary pneumonic plague

  • Primary pneumonic plague follows inhalation of infectious droplets.
  • Secondary pneumonic plague develops when infection from bubonic or septicemic plague reaches the lungs.

The distinction describes how the lungs became involved; both are pneumonic plague and need prompt medical attention.

How does plague spread, and can pneumonic plague spread from person to person?

People most often acquire plague from bites by infected rodent fleas. Handling infected animal tissue or body fluids is another exposure route. Pneumonic plague can also spread when an infected person or animal coughs infectious droplets and another person inhales them, usually during direct, close contact. CDC describes respiratory-droplet transmission as the only way plague spreads between people. CDC transmission

CDC’s 2021 review estimated a basic reproductive number (R₀) of 1.18 for pneumonic plague in the United States, using surveillance data from 1900–2009. This is a historical estimate based on those data, not a current universal forecast of an individual’s risk. CDC Emerging Infectious Diseases, 2021 review

Plague occurs naturally in parts of the western United States and in certain regions of Africa and Asia, according to CDC. CDC reports that human-to-human spread has not been documented in the United States since 1924; it also notes rare pneumonic plague cases after exposure to sick cats. These U.S.-specific observations do not mean plague is absent in other places. CDC transmission CDC plague maps

How is plague diagnosed?

Clinicians assess symptoms and exposure history and use laboratory testing on patient specimens, especially blood or material from a swollen lymph node. If plague is strongly suspected, CDC advises collecting specimens when possible and starting antibiotics without waiting for final test results. CDC diagnosis and testing

Is pneumonic plague treatable?

Yes. Plague can be treated with antibiotics, but prompt treatment is essential. CDC advises clinicians to begin appropriate therapy as soon as plague is suspected. In U.S. clinical guidance, gentamicin and fluoroquinolones are first-line options; treatment generally lasts 10–14 days, while the specific drug, route, and regimen depend on severity and patient factors. These are clinician-directed treatments, not instructions for self-medication. CDC clinical care

If you think you may have been exposed or have symptoms consistent with plague, seek urgent professional care and tell clinicians about the possible exposure. Do not wait for symptoms to worsen or attempt to treat it yourself. Public-health authorities may also need to be notified.

What precautions are used for pneumonic plague?

In U.S. emergency guidance, patients with suspected or confirmed pneumonic plague should be managed with standard and respiratory droplet precautions during the first 48 hours of antimicrobial treatment and until they improve clinically or plague is ruled out. CDC’s 2021 review describes spread through large respiratory droplets and reports no evidence for airborne transmission. This is not a reason to treat ordinary proximity without close exposure as generally contagious. CDC emergency guidance CDC Emerging Infectious Diseases, 2021 review
