Either can make an odor seem unpleasant. The key distinction is whether a real smell is being perceived incorrectly or no odor stimulus is present.
How are phantosmia and parosmia different?

These are descriptions of smell symptoms, not diagnoses of their cause, and they are not necessarily mutually exclusive. An example can help explain the distinction, but it cannot establish which symptom a person has.

Could a supposed phantom smell have a real source?
Yes. A smell that seems to come from nowhere may have a source in the nasal cavity or sinuses that is not obvious to the person experiencing it. A clinician may examine the nose and sinuses before describing the symptom as phantosmia. The fact that other people do not notice the odor does not, by itself, prove that there is no source.
Phantosmia is a clinical smell symptom; the word does not mean that the smell is imaginary or that it has a psychological cause.

What can cause these smell changes?
Causes vary, and many are associated with smell disorders generally rather than exclusively with either parosmia or phantosmia. The National Institute on Deafness and Other Communication Disorders (NIDCD) lists aging, sinus and other upper respiratory infections, smoking, nasal growths, head injury, hormonal disturbances, dental problems, chemical exposures, medications, head and neck radiation, and nervous system conditions among possible causes of smell disorders. That broad list does not mean each cause affects both symptoms equally.
Mayo Clinic lists head injury, upper respiratory infection, aging, temporal lobe seizures, inflamed sinuses, brain tumors, certain medicines, Parkinson’s disease, and COVID-19 as possible causes associated with phantosmia. These are possibilities, not a way to identify the cause in an individual case. Post-traumatic smell dysfunction is one context in which either parosmia or phantosmia may occur; recovery estimates for that context should not be applied to other causes or used to predict an individual’s outcome.

How are they evaluated?
Evaluation starts with the person’s history and a clinical examination. A health professional may ask when the change began, whether an identifiable odor triggers it, and about relevant events such as an injury or chemical exposure. Assessment can include an ear, nose, and throat examination and a supervised smell test that measures odor detection or identification.

CT or MRI may be considered in selected cases; imaging is not automatically necessary for everyone. The NIDCD notes that diagnosis matters because it can help identify and treat an underlying cause.

What treatment is available?
Treatment depends on the cause; there is no single remedy established for everyone with parosmia or phantosmia. If a medication may be involved, discuss it with the clinician who prescribed it rather than stopping or changing it on your own. If a structural nasal problem such as polyps is responsible, care may target that obstruction. Some people improve as an underlying illness resolves, while others improve spontaneously.
The sources cited here do not establish that a particular consumer product, nasal device, essential oil, or course of smell training cures either symptom. Ask a clinician which steps fit the cause identified in your case.

When should you seek medical advice?
Talk with a healthcare professional about a new or concerning change in smell. NIDCD recommends discussing smell changes with a doctor; an otolaryngologist may help identify a cause and treatment options. Mayo Clinic likewise advises professional evaluation for phantosmia so possible serious causes can be considered and ruled out when appropriate. A phantom smell has multiple possible causes and does not, on its own, establish a dangerous diagnosis.
If the change begins suddenly alongside other acute neurological or serious symptoms, seek prompt medical care. The smell symptom alone is not enough to determine whether an emergency is occurring.
