Patient-Specific Orthopedics: Can Custom Devices Deliver Speed and Precision?

Patient-specific orthopedic devices may help in complex cases, but custom design does not guarantee a better fit, outcome, or faster operation. Understand the device types, evidence, and workflow checks.

Patient-specific orthopedic devices can help address unusual anatomy, bone loss, tumors, or complex fractures by tailoring a model, surgical guide, or implant to an individual patient. But customization is not a general guarantee of greater precision, better outcomes, or a shorter operation. “Speed” may mean getting a design and device delivered to the operating team—not making surgery itself faster.

What “patient-specific orthopedics” includes

The phrase covers different products that start with patient imaging and translate a person’s anatomy into a plan or device. A model, a temporary guide, and a permanent implant have different purposes and risks; they should not be treated as interchangeable.

Approach What it does What it must get right
Anatomical model Represents anatomy for planning or reference; it is not implanted. The image-derived anatomy must be accurate enough for the model’s intended use.
Patient-specific instrumentation (PSI) A temporary guide can help reproduce a planned cut, drill path, or placement. It must seat stably on the intended bone surface and be used in accordance with the plan.
Custom implant A device tailored to the patient that remains implanted. It must satisfy the material, mechanical, and biological demands of its intended use, as well as fit the case.

All three may depend on imaging and planning, but only the implant remains in the body. A guide can help carry a plan into the operation, but it does not itself establish that the plan is clinically appropriate or that the implant will perform well.

When might a custom solution make sense?

The clearest rationale is a case in which standard devices or instruments may not suit the anatomy or defect. Examples discussed in orthopedic literature include irregular anatomy, bone loss after tumor resection, and complex fractures. These are potential reasons to consider customization—not proof that a custom device is necessary or superior.

  • Anatomical model: may support planning when the relevant anatomy is difficult to understand from images alone.
  • PSI: may help translate a surgeon-approved plan into a cut, drill trajectory, or placement in a specific operation.
  • Custom implant: may be considered when a defect or anatomy calls for a device that standard options do not adequately address.

The choice depends on the procedure and the problem being solved. Evidence about PSI in total knee arthroplasty, for example, cannot establish the value of a custom implant for tumor-related bone loss.

Does patient-specific design make surgery faster?

Not necessarily. There are at least two different clocks: the time needed to plan, manufacture, check, and deliver a device before surgery, and the time the patient spends in the operating room. A faster design-to-delivery process would address the first; it does not by itself demonstrate a shorter operation.

In a September 30, 2026 Digital Journal article about Ortho Solutions Group and its OrthoMatrix platform, U.S. President Kevin Brothen described delivery speed as an industry priority: “The disruption will be speed to delivery.” The article names patient-specific fusion cages and total talus implants for significant foot-and-ankle bone loss. That is company-reported context, not independent evidence that these products improve outcomes, shorten surgery, or are available for every patient.

For operating time, a 2025 Cureus systematic review and meta-analysis examined 14 comparative studies covering 2,704 total knee arthroplasty (TKA) procedures. Its pooled results favored PSI on alignment measures, but found no statistically significant difference in operative time; the authors also reported high heterogeneity among studies. Those findings concern PSI in TKA—not delivery time, all orthopedic operations, or custom implants generally.

What does the evidence establish—and what remains uncertain?

A 2025 review by McAnena, McClennen, and Zheng selected 58 papers on 3D-printed orthopedic implants and surgical devices. The literature describes potential uses and promising case outcomes, but the review cautions against treating that record as proof of broad comparative benefit: most included studies were small, many lacked a conventional-device control group, and only five had more than 20 participants.

Within that review’s sample, 48% of the 58 papers involved orthopedic oncology. That is a share of reviewed articles, not the proportion of patients with cancer or a measure of how often custom devices are used. Of the 47 reviewed implant articles, 81% used titanium alloy—a proportion of the literature sample, not a market share or a recommendation for an individual patient.

  • A STANDING OVATION: Ovation Medical is a family-run, US-based business, passionate about providing exceptional quality, service, and selection to its customers. Ovation guarantees all products are defect free.

The authors conclude: “Large controlled studies are necessary to compare patient-specific implants with the standard of care and evaluate their safety profiles over time.” In other words, case reports and small series can show that a technique has been used; they cannot on their own establish that it is safer, more effective, or more cost-effective than standard care.

Findings about PSI in knee replacement have also changed with the evidence base and differ by endpoint. A 2018 review discussed limitations including guide registration and fit, the possibility of misplacement, and the lack of navigation-style intraoperative image feedback. It also reported that earlier meta-analyses did not support routine PSI in TKA. The 2025 meta-analysis later reported improved pooled alignment measures but no statistically significant operation-time difference, with high heterogeneity. These results address different publication dates and endpoints; they do not support a blanket claim that PSI should—or should not—be used in every TKA or other operation.

Where precision can break down

A device derived from imaging is only as dependable as the steps connecting the image, the plan, and the operation. Inaccurate image segmentation can distort the anatomy used for design. A PSI guide also needs a stable footprint against the intended bone surface; if it does not seat as planned, it can carry the surgeon away from the intended cut or trajectory.

That is why patient-specific does not mean automatically precise. The workflow needs surgeon input, review of the plan, checks that the manufactured device matches its intended case, and a way to recognize when fit or operative conditions do not match expectations. PSI is a planning aid, not a substitute for surgical judgment or verification during the procedure.

How to assess the workflow before choosing it

The practical comparison is not simply “custom” versus “standard.” A patient and care team need to consider the device’s purpose, the evidence for that particular procedure, and the complete path from imaging to delivery and use.

  1. Define the problem. Identify the anatomical feature, defect, or planned reconstruction that a standard approach may not address adequately.
  2. Name the device type. Clarify whether the proposal is an anatomical model, temporary guide, or permanent implant, and what role it plays in the operation.
  3. Review evidence for the same indication. Ask whether the available studies concern the same procedure and device type, what they compare against, and how many patients and how much follow-up they include.
  4. Review the image and plan. Understand how the patient’s imaging is translated into anatomy and design, who reviews the plan, and how the surgeon approves it.
  5. Ask how fit and use are checked. For a guide, ask how stable contact with the bone is assessed and how the team responds if it does not seat as expected. For an implant, ask how the device is checked for the intended defect and use.
  6. Confirm timing and contingencies. Ask for the expected planning, manufacturing, checking, and delivery schedule for the specific case, plus the plan if the device is delayed or unusable. Comparable supplier turnaround times are not established by the cited literature.
  7. Discuss safety, oversight, and total cost. Ask what quality checks, complication information, and jurisdiction-specific regulatory status apply to the particular device. Costs and regulatory requirements vary; the cited reviews do not establish a current universal pathway or comparable vendor prices.

What adoption figures can—and cannot—tell you

Historical figures should not be mistaken for current uptake. In a 2013 survey, Thienpont reported that manufacturers contacted estimated 82,556 PSI procedures worldwide in 2012. That figure is limited to the manufacturers surveyed and that historical year; it does not describe present-day volume, the number of custom implants, or clinical effectiveness.

How should patients discuss a patient-specific device?

Ask the treating orthopedic surgeon what specific limitation of a standard option the proposed device addresses, and whether the proposal is a model, guide, or implant. Then discuss the evidence for that operation, the plan-review and fit checks, the delivery schedule and backup plan, and the device’s risks and total cost. The case for customization is strongest when it solves a clearly identified problem; the label alone is not evidence of a better result.
