Nausea and other digestive symptoms are common across these medicines, but their approved uses, formulations and prescribing information are not interchangeable. In the United States, the right comparison depends on the specific product and the reason it is prescribed—not just the active ingredient. This overview reflects FDA information available October 4, 2026, and is not individual medical advice.
How the three medicines differ
Ozempic and Wegovy use the same active ingredient, semaglutide, but are distinct prescription brands with different labeled uses and dosing contexts. Zepbound uses tirzepatide, which acts on both GIP and GLP-1 receptors. Check the current prescribing information for the exact product and formulation; a shared ingredient does not make two brands interchangeable.

| Brand | Active ingredient | FDA-labeled context described in the sources | What to keep in mind |
|---|---|---|---|
| Ozempic | Semaglutide | For adults with type 2 diabetes, glycemic control and specified cardiovascular and kidney risk-reduction indications. | Its label and dosing context differ from Wegovy’s weight-management use. Confirm the specific indication and dose with the prescriber. FDA Ozempic prescribing information |
| Wegovy | Semaglutide | Weight management; FDA review material also describes a cardiovascular-risk indication for eligible adults and a newer oral formulation. | Do not assume the tablet and injection have the same labeling, titration or adverse-reaction information. Use the current prescribing information for the formulation being considered. FDA semaglutide review |
| Zepbound | Tirzepatide | Chronic weight management; recent FDA review material also lists adult obstructive sleep apnea in people with obesity. | It is a dual GIP/GLP-1 agonist, not semaglutide. Consult current product information for formulation-specific details. FDA Zepbound prescribing information |
For weight management, FDA describes Zepbound as an adjunct to reduced-calorie eating and increased physical activity for eligible adults with obesity, or overweight plus a weight-related condition. FDA approval announcement
Which side effects are common?
Nausea, vomiting, diarrhea, constipation, and abdominal discomfort or pain appear in product-label materials for these medicines. The exact lists and reported frequencies vary by product, indication, formulation and clinical-trial population. A percentage in one product’s trial should not be read as a direct comparison with a percentage in another: FDA labeling cautions that adverse-reaction rates from different trials cannot be compared directly because trial conditions differ. See the Ozempic and Zepbound labels for their product-specific information. Exact Wegovy adverse-reaction percentages are not included here; consult the current label for the formulation in question.

- CONSTIPATION RELIEF FOR ADULTS ON A GLP-1 - Magnesium citrate (250 mg) pulls water into the bowel for gentle, non-cramping regularity. A daily laxative alternative built for slowed digestion: it works with your medication, not against it.
- DEBLOAT, GAS & SULFUR BURP RELIEF - Ginger root (200 mg) is a natural prokinetic that keeps food moving and cuts fermentation gas at the source, while a 19-strain probiotic blend rebuilds the gut bacteria slowed transit disrupts.
- COMPLETE GLP 1 SUPPORT IN TWO CAPSULES - 14 actives: magnesium, ginger, peppermint, 19-strain probiotics, methyl B12, B6, iron, zinc and digestive enzymes. No psyllium, no added fiber. Fiber only adds bulk to a stalled gut.

Serious warnings and when to contact the care team
Thyroid C-cell tumor warning
All three products carry a boxed warning about thyroid C-cell tumors observed in rodents. Whether this risk applies to humans is unknown. The semaglutide and tirzepatide labels contraindicate use in people with a personal or family history of medullary thyroid carcinoma, or with multiple endocrine neoplasia syndrome type 2 (MEN 2). Review relevant personal and family history with a prescriber before treatment. Ozempic label; Zepbound label
Other serious risks in the reviewed labeling
Product information also identifies serious gastrointestinal reactions, acute kidney injury related to volume depletion, acute gallbladder disease, pancreatitis, serious hypersensitivity reactions, and pulmonary aspiration concerns during general anesthesia or deep sedation. This list is not a diagnosis or a prediction of individual risk; ask a clinician or pharmacist how the current label applies to your health history and medicines.

Before anesthesia or deep sedation
Tell the healthcare professionals arranging a procedure that you use one of these medicines. The labels address aspiration concerns, but a general comparison cannot determine whether or when an individual should pause treatment. Get instructions from your care team rather than using a generic medication-holding schedule.
Suicidal-thought warning language was updated in 2026
On January 13, 2026, FDA said its review found no increased risk of suicidal ideation or behavior associated with GLP-1 receptor agonists and requested removal of that warning from affected labels, including Wegovy and Zepbound. Older materials may still show the prior language, so check FDA’s January 13, 2026 safety communication for the update.

What direct comparison evidence can—and cannot—tell you
SURMOUNT-5 was a phase 3b, open-label trial of tirzepatide versus semaglutide in adults with obesity or overweight plus a weight-related condition and without diabetes. In its 2025 results release, sponsor Eli Lilly reported greater weight reduction with tirzepatide and that 6.1% of participants assigned tirzepatide and 8.0% assigned semaglutide discontinued treatment because of adverse events. Those are trial-specific figures from the sponsor’s release, not expected rates for every patient or proof that one medicine causes fewer side effects in every population. The comparison also does not make Ozempic and Wegovy interchangeable or establish how every formulation or indication compares. Lilly’s SURMOUNT-5 results release
For a side-effect comparison, do not rank medicines by percentages drawn from separate trials. Ask which evidence applies to the exact product, formulation, indication and patient group being discussed.

Approved prescriptions are not the same as compounded products
FDA warns of dosing errors and other risks involving compounded semaglutide and tirzepatide. Semaglutide salt forms, including sodium and acetate, are different active ingredients from the semaglutide in approved medicines; they should not be assumed equivalent. FDA advises obtaining medicines through a prescription and a state-licensed pharmacy rather than unverified online sources or products marketed as equivalent.

As of May 31, 2026, FDA reported 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide. These are reports, not incidence rates or proof that a product caused an event; FDA notes that adverse events may be underreported and causality may be uncertain. How to compare options with a clinician
- Start with the treatment goal and the product’s FDA-labeled indication, rather than assuming the three brands serve the same purpose.
- Identify the exact brand and formulation under consideration; injection and tablet information may differ.
- Review digestive symptoms, serious warnings, contraindications, medical history and other medicines against the current prescribing information.
- If a procedure with anesthesia or deep sedation is planned, tell the care team and ask for individualized instructions.
- When weighing comparative trial findings, check the population and study design, and distinguish sponsor-reported trial results from a universal prediction.
