Those are separate events. A provider status— even one labeled delivered—does not prove the intended person saw the message. To make an SMS login flow auditable, record each stage distinctly, avoid logging the code itself, and treat any price estimate as dependent on region, retries, and message segments.
How an SMS OTP login flow works
A login verification has three stages: start a challenge, send a code through a channel such as SMS, then check the code the user submits. A managed verification service can operate much of that lifecycle. A custom Node.js endpoint using a general SMS API must also implement and maintain code generation, expiry, retry rules, verification checks, and abuse controls.

Twilio’s Verify documentation describes a managed flow: create a Verify Service, request a verification for a destination using the SMS channel, then submit the user’s code to the Verification Check API. Its REST API is served over HTTPS; Twilio says unencrypted HTTP is not supported. The documentation includes Node.js client-library examples and environment-based credentials. Keep credentials outside source control rather than embedding secrets in application code. Twilio Verify API documentation

What each status can establish
Do not collapse an API response, a delivery event, and a successful login into one “delivered” result. Each says something different about the flow.
| Event | What it supports | What it does not establish |
|---|---|---|
| Verification request accepted | The provider accepted an API operation to initiate a challenge. | That an SMS reached a carrier, handset, or person. |
| Provider attempt record or status | The provider recorded a channel action or attempt; the available details depend on the API and implementation. | That the intended recipient personally saw the text. |
| Delivery status | A provider-reported delivery outcome. Twilio cautions that SMS is a best-effort protocol and that confirmation may only show delivery to the sending carrier. | That the handset displayed the message, that the recipient read it, or that the account owner controlled the number. |
| Verification check succeeds | The submitted token matched the verification flow. | That SMS is resistant to interception or phishing, or that the person is otherwise trustworthy. |
Twilio’s troubleshooting guidance explicitly distinguishes best-effort SMS from proof of receipt by the intended person. Twilio SMS troubleshooting guidance

What to log for an auditable OTP endpoint
Build the audit trail around events, not a single success flag. The fields below are an application-side proposal, not a complete logging schema prescribed by Twilio. Provider attempt records can add channel-specific details, conversion status, and price where the chosen API exposes them; the exact fields vary by implementation. - Application: run or correlation ID, request timestamp, destination region, retry count, and elapsed time. Avoid placing the full phone number in broadly accessible logs.
- Provider: verification and attempt identifiers, channel, response or status, message-segment count if available, and recorded price.
- Application verification result: whether the submitted-code check succeeded, failed, or the challenge expired, plus the check timestamp.
Never log raw OTP values. Limit personal data to what operations and security teams need, restrict log access, and define retention appropriate to your use case. Keep failed, expired, and completed flows distinguishable. A successful test message demonstrates only that one test flow completed; it cannot establish a general delivery rate or reliability level.

Custom Node.js endpoint or managed verification API?
| Decision area | Custom endpoint plus SMS API | Managed verification service |
|---|---|---|
| Code lifecycle | Your application owns code generation, expiry, storage, retries, and checking. | The service can manage much of the verification lifecycle; confirm exact behavior in its API documentation. |
| Abuse controls | You must design and operate rate limits and fraud controls. | Provider controls may be available, but settings and limits still need review. |
| Audit visibility | Depends on what your application records and what the SMS API reports. | Verification and attempt records may expose channel activity and conversion-related data. |
| Cost model | SMS charges depend on geography, segments, retries, and the selected provider; engineering and operating effort also matter. | May combine a per-successful-verification charge with channel and message fees. |
| Operational ownership | Your team maintains the security-sensitive workflow as well as delivery integration. | The vendor handles more of the OTP lifecycle, while your team remains responsible for application integration, policy, and safe use. |
A general SMS API sends messages; a verification product is designed around initiating and checking verification challenges. Neither option is automatically cheaper: compare actual usage, geography, message length, retries, and engineering effort.
How much can SMS verification cost?
Twilio’s pricing page, marked current as of August 2026, lists $0.05 per successful verification plus standard channel fees. Its US SMS row lists an additional $0.0083 per SMS. These are vendor-published figures for that pricing page, not a market average or a guaranteed total for other regions. Twilio Verify pricing

For a US SMS flow under those listed rates, the base arithmetic is $0.05 for a successful verification plus $0.0083 for each SMS sent. More than one SMS attempt adds message charges; message segmentation can also change SMS costs. Twilio’s best-practices documentation explains that segment pricing and character set affect message length and billing. Twilio developer best practices
Use your expected geography, channel, number of retries, and segment count to estimate spend. Recheck the provider’s current rate card and your account’s applicable fees before budgeting, since the cited prices are dated and can change.

Is SMS 2FA secure enough for login?
SMS codes add a verification step, but they have important weaknesses. OWASP identifies risks including SS7 interception, SIM swapping, and number porting, and notes that SMS/PSTN codes are a restricted authenticator under NIST SP 800-63B-4. SMS is not phishing-resistant and may not be appropriate for accounts or actions with higher security requirements. OWASP Multifactor Authentication Cheat Sheet
Choose the authentication method based on the impact of account takeover, not just the simplicity of sending a text. Where the risk warrants it, offer stronger alternatives and avoid treating successful code entry as proof against number takeover or social engineering.
Consent and service limits to check
Twilio’s Verify SMS overview says to obtain and document recipient consent under its Messaging Policy before sending OTPs. That vendor policy is not a universal legal determination: check the rules that apply to your geography and use case. Twilio Verify SMS overview
