Nation-State Cyber Risk Is an Identity and Resilience Issue for CISOs

Nation-state cyber risk can mean persistent access, credential theft, intelligence collection, or operational disruption. A CISO response must cover identity, exposed infrastructure, suppliers, OT, and recovery.

Nation-state cyber risk belongs on the CISO’s core risk agenda because it is not limited to a disruptive attack. State-linked operators may seek intelligence, intellectual property, trusted access, or a foothold that could support future disruption. Separately, pro-state hacktivists and other opportunistic actors can cause real operational harm with basic methods. A sound response therefore covers identity, exposed infrastructure, suppliers, and operational technology (OT)—not just the network perimeter.

What makes nation-state activity a business risk?

The FBI says actors associated with China, Russia, Iran, and North Korea continue to conduct intrusions targeting U.S. victims. It describes objectives that include intelligence and intellectual-property collection, as well as establishing destructive capability. That is the FBI’s U.S.-focused framing; it does not mean every named state targets every organization or that every intrusion is intended to cause disruption.

Microsoft’s Digital Defense Report 2026 describes a broader global pattern: activity associated with China, Iran, North Korea, and Russia increasingly emphasizes persistent, scalable access and long-term positioning in high-value environments. For a CISO, the implication is that an incident may be consequential even when it does not immediately interrupt service. An adversary with access to accounts, systems, or a supplier relationship may be collecting intelligence or preserving options for later.

These operations overlap with, but should not be conflated with, financially motivated cybercrime. The FBI notes that criminal ransomware ecosystems—including developers, affiliates, and service providers—change tactics. Security teams need to handle both kinds of risk without assuming that every intrusion has the same sponsor, motive, or end state.

Which access paths should CISOs account for?

Identities and trusted access

Perimeter defenses alone cannot address an intruder using valid credentials or an established trusted relationship. Microsoft’s 2026 report says 52.2% of valid-account intrusions in its observation set involved follow-on credential theft. This is a Microsoft report figure, not a universal rate for organizations or all intrusions. It nevertheless reinforces the need to treat identity compromise as a possible route to further access, not as an isolated login event.

Internet-facing systems and edge devices

The FBI’s 2026 alert index includes examples involving phishing against commercial messaging accounts, Iranian malware activity, North Korean targeting, and exploitation of vulnerable edge devices. These alerts describe specific campaigns and vulnerabilities; they do not establish that every organization is exposed in the same way. They do show why asset discovery, remote-access review, and timely remediation belong in the same risk conversation as user authentication.

Suppliers and connected ecosystems

Microsoft describes persistent access models that can extend across critical systems, identities, and digital ecosystems. That makes supplier access and dependencies relevant to the organization’s threat picture: identify which external parties can reach sensitive environments, what access they retain, and how their compromise could affect your services. The cited sources do not establish a single supplier-control model that fits every organization, so map access and recovery dependencies to your own contracts and operations.

Why OT changes the consequences

OT systems can connect cyber intrusion to physical processes, essential services, and safety. The FBI’s 2026 alert index includes a joint advisory on Iranian-affiliated actors exploiting internet-connected programmable logic controllers (PLCs) across U.S. critical infrastructure. A later index note describes expansion to additional PLC manufacturers and emphasizes restricting direct internet access. The index is not the full technical advisory, so those are the claims it supports; consult the advisory itself for technical details before changing a control system.

In a December 9, 2025 release, the U.S. National Security Agency (NSA) reported that agencies observed pro-Russia hacktivist groups using inadequately secured VNC connections to access OT control devices. The release identified water and wastewater, food and agriculture, and energy among affected sectors. It warned that indiscriminate, opportunistic attacks can reach unintended critical-infrastructure victims and cause unanticipated damage. The groups were described as pro-Russia hacktivists; that characterization should not be collapsed into a claim that each incident was a centrally directed state military operation.

The practical lesson is that simple, inexpensive techniques can still have serious consequences when they reach exposed control devices. Assess external connectivity and remote administration in the context of the process being controlled, the services that depend on it, and the safe ways to isolate or recover it.

How should a CISO turn the threat into a work plan?

Use the threat picture to assign ownership across security, infrastructure, identity, OT, procurement, and service owners. The priorities below reflect recommendations and examples in the cited FBI, Microsoft, and NSA materials; they are not a guarantee that any one control will prevent compromise.

  1. Find exposed assets. Inventory internet-facing systems, edge devices, remote-access services, and unsupported equipment. Prioritize remediation or replacement where exposure, known weaknesses, or end-of-support status creates unacceptable risk. Include ownership and service dependencies so an emergency fix does not inadvertently disrupt an essential process.
  2. Reduce what a stolen identity can reach. Strengthen identity verification, remove unnecessary standing privilege, and limit privileged access. Require phishing-resistant authentication where accounts and systems support it. A compatible FIDO2 security key is one possible implementation; compatibility with legacy systems and recovery procedures must be checked before deployment.
  3. Review OT and remote administration paths. Identify externally reachable control devices, VNC access, and other remote-management routes. Map which networks and essential services depend on them, then decide how to restrict access and how to respond safely if a route is compromised.
  4. Make activity visible across systems. Improve cross-system detection so suspicious use of valid credentials or trusted access can be investigated across identity, cloud, endpoint, edge, and OT environments. Visibility gaps between platforms can hide the relationship between otherwise isolated events.
  5. Exercise response and recovery with the right owners. Involve business, legal, communications, infrastructure, supplier, and operational stakeholders. Test decisions about containment, service continuity, notification, and recovery. NSA’s June 30, 2025 Iran-related release urged at-risk critical-infrastructure organizations to assess weaknesses and update incident-response plans; it was tied to then-current events, not a standing forecast of present likelihood.
  6. Use current government guidance. Monitor relevant government advisories and share incidents with appropriate authorities and sector partners. The FBI says it issues advisories with hunting guidance and mitigations as indicators and techniques evolve. Reassess actions against the specific alert and your organization’s exposure rather than treating an index entry as evidence of universal impact.

What the available figures do—and do not—show

Microsoft’s Digital Defense Report 2026 also reports more than 46 million business contact impersonation attacks detected over the preceding 12 months. It says 25.5% of observed cyber threat activity impacted Microsoft customers in the United States from January 2025 through June 2026. Its reported 2026 global sector distribution among the top ten impacted sectors was government agencies and services at 27%, information technology at 17%, and research and academia at 14%.

These are Microsoft telemetry and report figures, tied to its observation set and stated periods. They are not estimates of global incident prevalence, the probability that a particular company will be attacked, or a measure of nation-state activity alone. No government-wide frequency statistic for nation-state attacks against all CISOs is established by the cited material.

Set priorities around exposure and operational impact

A practical CISO decision is not simply whether to buy another control. Evaluate existing controls against the systems and dependencies that matter to your organization:

  • Coverage of human and non-human identities, including supplier accounts.
  • How much standing privilege remains and how quickly access can be restricted.
  • Resistance to phishing and credential theft, including compatibility with legacy applications.
  • Visibility across identity, cloud, endpoint, internet-facing infrastructure, and OT.
  • Time to patch, isolate, or replace an exposed asset—and the effect of that action on essential services.
  • Recovery dependencies, including suppliers and systems needed to restore safe operations.

The order will vary by sector, geography, exposure, contractual obligations, and recovery needs. The common requirement is to make those trade-offs explicit: a control that reduces access risk but cannot be safely deployed or recovered may create a different operational risk that also needs an owner.
