Making Replacement Organs: What Science Can and Cannot Do Today

Organoids, organ-on-a-chip systems, and bioprinted tissue are real laboratory tools, but no complete replacement organ is ready for transplant. Blood supply is the central barrier.

No lab-grown organ that can be transplanted into a person as a complete, working replacement for a failed kidney, liver, heart, lung, or pancreas exists today. What laboratories can make now are smaller tools: organoids, organ-on-a-chip systems, engineered tissue constructs, and bioprinted tissue. These are used mainly to study organ biology and disease and to test scientific questions. Building a full, functioning solid organ remains a research goal, and the hardest unsolved part is the blood supply.

What “making replacement organs” covers

“Making replacement organs” is an umbrella phrase, not a single technique. The U.S. Food and Drug Administration (FDA) defines the broader field this way: “Regenerative medicine refers to a general approach to restore, replace, or recreate cells, tissues, or organs to treat or mitigate disease.” Under that definition, work can involve cell therapies, tissue-engineered products, scaffolds, gene-related methods, or combinations of cells and materials. Most of the progress so far sits in the smaller categories below, not in whole organs.

Organoids

Organoids are miniature, organ-like structures grown from cells in the laboratory. The National Institutes of Health (NIH) describes them as miniature organ-like research structures. They are useful for watching how tissue develops, how it responds to a drug, or how a disease alters it. An organoid is a model of an organ, not a substitute for one. In 2025 NIH reported that researchers had made miniature lung and intestinal organoids with specialized blood vessels, a sign of progress in models rather than a clinical source of organs.

Organ-on-a-chip systems

Organ-on-a-chip systems place living cells in small devices that mimic the environment of an organ. NIH cites a lung-on-a-chip model as an example. These systems help researchers study how cells and tissues behave under controlled conditions. They are laboratory tools and are not implanted in patients.

Scaffolds and engineered tissue

Tissue engineering combines cells with a scaffold, a supporting structure that guides how cells attach, grow, and organize. Engineered tissue products may be designed to restore or replace specific tissue. Some of these products fall under FDA oversight, which is discussed below. A scaffold can give a tissue a shape, but it does not by itself supply the mature cells, blood vessels, and function a working organ needs.

Bioprinting

Bioprinting arranges living cells and biomaterials in designed patterns, layer by layer. It can produce constructs with a defined geometry, which is valuable for research. A 2025 review describes major gaps in scaling up, vascularization, immune compatibility, maturation, and translation to patients. Structural resemblance to an organ does not show that a printed construct can perform all of that organ’s functions or be safely transplanted.

How the approaches compare

The table compares the main approaches on the points that matter most for an organ intended for a patient. Where the sources cited here do not address a point, the cell says so.

Approach Typical scale Blood supply Current role
Organoids Miniature, organ-like structures NIH reported in 2025 that miniature lung and intestinal organoids had specialized blood vessels; organ-scale vascular networks are not stated Research models
Organ-on-a-chip Small, chip-based systems (NIH cites a lung-on-a-chip) Not stated in the sources cited here Research models
Engineered tissue constructs Defined shapes; thicker tissue needs a vascular network Identified as a major unsolved challenge for thick tissue Research and product development; some products regulated by FDA
Bioprinted constructs Designed geometry built from living cells and biomaterials Vascularization identified as a major gap in a 2025 review Research constructs; structural resemblance does not establish full organ function
Whole solid organ Complete, functioning organ Must be built and connected to the recipient’s circulation Future research goal; no transplant-ready example in the sources cited here

Where the science stands

The most defensible framing is that complete lab-grown solid organs are a future goal, while organoids, organ-on-a-chip systems, and engineered tissue constructs are already useful as research models. The gap between the two is large. A model can reproduce some features of an organ in a dish, while a transplantable organ must carry out every job the original did, over years, inside a living body.

Readers often see headlines about printed hearts, kidneys, or livers. Those headlines usually describe a model, a piece of tissue, or a laboratory milestone. The sources cited here do not establish a field-wide rate of success for transplanting lab-grown whole organs, and secondhand figures quoted in reviews should not be read as one without checking the original study and its scope.

Why blood vessels decide whether an organ can be built

A thick, metabolically active tissue needs oxygen and nutrients delivered to every cell and waste products removed. In a small piece of tissue, these can reach cells by simple diffusion. In a larger organ, they cannot. The engineered construct must contain a connected vascular network that reaches deep into the tissue, and that network must link up with the recipient’s blood system once the organ is implanted.

Liver bioprinting reviews describe the same problem in more detail. They report difficulty sustaining liver cells and difficulty reproducing both the vascular system and the biliary system, the network that carries bile. Vascularization is therefore not one hurdle among many. It is the constraint that determines how large and how complex a construct can become.

  • Cell sourcing and expansion. Researchers must choose a suitable cell type and grow enough of it without losing its properties.
  • Functional maturity. Lab-grown cells must reach adult-like function, and a construct must keep working over time.
  • Immune compatibility. The engineered tissue must avoid rejection by the recipient’s immune system.
  • Cell behavior and migration. Cells must stay where they are placed and organize into the right structure.
  • Tumor formation. Cell populations must not develop into tumors after implantation.
  • Sterility. Products must be kept free of contamination throughout production.
  • Consistent manufacturing. Each batch must perform the same way, which is hard to guarantee for living, biological products.
  • Long-term safety and effectiveness. Benefits must be shown over time, not just at the moment of implantation.

The FDA lists many of these issues as questions it weighs when evaluating engineered scaffolds and regenerative products.

Who oversees this in the United States

The FDA regulates many human cell and tissue products and regenerative medicine products. Its tissue FAQ draws a line between those products and vascularized human organ transplantation. The Health Resources and Services Administration (HRSA) oversees the donation and transplantation of vascularized organs such as kidneys, livers, hearts, lungs, and pancreases. The FAQ also does not identify any licensed stem-cell treatment on that page. That is a statement about the page, not a verdict on every regulated cell product or every transplant.

How to read a claim about lab-grown organs

Several questions help separate a laboratory milestone from a usable therapy:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Is it a model or a treatment? Organoids and chips are used to study biology. A treatment is given to patients and is subject to regulatory review.
  • What is the scale? A miniature structure is not comparable to a full-size organ, even if it looks similar.
  • Is blood supply addressed? If a paper does not describe how the tissue receives oxygen and nutrients at useful size, the work is not a path to a whole organ.
  • Was it tested in animals or in people? Laboratory and animal results do not establish human clinical benefit.
  • What regulatory stage is it in? Ask whether the product is an approved therapy, part of a clinical study, or only a research tool.

A general-purpose consumer 3D printer or commercial bioink would not provide the capabilities needed to make a human replacement organ. Work at the frontier requires specialized laboratories, controlled manufacturing, and regulatory oversight.
