Digital projects fail when unclear goals, weak decision-making, misaligned stakeholders, underestimated complexity, inadequate capacity, expanding scope, and late technical problems compound. There is no single failure rate that applies to every software delivery, major IT program, or company-wide transformation—and failure is not synonymous with being late or over budget.
What counts as a failed digital project?
A project can miss its schedule or budget and still deliver useful outcomes. Conversely, it can meet both targets but fail to produce lasting business value. The distinction matters: delivery measures describe how work was completed, while outcome measures describe whether it improved the organization in a sustained way.

That is why failure statistics vary. The Project Management Institute (PMI) reports a 13% failure rate for projects overall and says about one-third of complex projects fail in its 2026 Pulse of the Profession. McKinsey’s 2018 survey asked about a different outcome: whether a digital transformation both improved performance and equipped the organization to sustain those improvements. Sixteen percent of respondents said theirs had done so. These figures have different populations and definitions; they are not competing estimates of one universal rate.

McKinsey and the University of Oxford’s BT Centre for Major Programme Management analyzed more than 5,400 projects in a database as of June 2012. For projects initially priced above $15 million, the analysis found averages of 45% over budget, 7% over schedule, and 56% below predicted value. Those historical findings describe that dataset, not the expected result for every digital project today.

Why do digital projects fail?
Success was never defined in a stable, shared way
When the business case is weak or objectives are vague, a team can deliver software without resolving the problem that justified it. Leaders may track cost and schedule but lose sight of strategic value, user needs, or the conditions needed to sustain improvement. McKinsey’s 2020 analysis of large technology programs lists unclear objectives among the causes leaders associate with failure and stresses continued involvement from business and functional leaders.
Governance and stakeholder alignment come too late
Projects need clear ownership, timely decisions, and participation from the functions affected by the change. If stakeholders are overlooked or decision rights are ambiguous, issues can sit unresolved until they force redesign or delay. McKinsey describes a case in which late involvement by a finance department required system changes and pushed back launch. Australia’s Digital Transformation Agency likewise identifies weak project-management practices and maturing governance among the most common root causes in its 2026 report on struggling major digital projects.

Leaders underestimate complexity and dependencies
A major program is not simply a large software build. Architecture, sourcing, functionality, data migration, rollout, and organizational change can depend on one another. A decision in one area may reshape the work elsewhere. PMI’s 2026 report describes complexity in terms of interconnected stakeholders, uncertainty, decision friction, rework, and pressure on teams—not just technical difficulty.

McKinsey cautions that exhaustive upfront plans cannot anticipate every change in a complex program. An integrated schedule still helps teams understand dependencies, but planning needs to be paired with iterative work across connected domains rather than treated as a promise that every detail is settled at the outset.

Team capability and capacity do not fit the work
A plan can be sensible on paper and still be impossible to deliver with the people available. Teams may lack technical mastery, business or domain knowledge, access to decision-makers, or enough time to do the work. McKinsey identifies capability gaps, weak change management, vendor deficiencies, and poor team capabilities among commonly cited causes of large-program failure. A project also suffers when incentives reward local milestones instead of the overall outcome.
Scope, integration, and quality problems arrive late
Changing or poorly understood requirements make it difficult to control scope and judge whether the result will meet users’ needs. Tight timelines can leave little room to resolve defects and integration problems. Excessive customization or a poor platform decision may add complexity that becomes visible only after connected systems and processes are brought together.

Australia’s 2026 report names scope complexity, technical obstacles, resource constraints, and compressed timelines among recurring challenges in its major-project portfolio. McKinsey recommends short delivery cycles and rigorous quality checks as ways to find problems sooner. Neither prevents every issue, but both make it less likely that defects and mismatched expectations remain hidden until late in delivery.

| 13% of projects overall; about one-third of complex projects fail | Project failure, with separate figures for overall and complex projects | PMI, 2026 Pulse of the Profession. PMI also reports that 97% of project professionals managed at least one complex project in the prior year and more than half classified those projects as significantly complex. |
| 16% said their transformation improved performance and equipped the organization to sustain improvements | Respondent-reported digital-transformation success under that combined definition | McKinsey online survey of 1,793 respondents, conducted January 16–26, 2018; McKinsey’s transformation analysis. |
| 45% over budget, 7% over schedule, and 56% below predicted value, on average | Budget, schedule, and value outcomes for large IT projects | McKinsey and the BT Centre for Major Programme Management at the University of Oxford, 2012; analysis of more than 5,400 projects in the database as of June 2012, defined as initially priced above $15 million. These are historical averages, not a current forecast for all projects. |
| 103 projects across 43 agencies; over $9.7 billion of investment, including $5.9 billion in digital technologies | Scale of the Australian government major-project portfolio covered | Digital Transformation Agency, Australian Government, 2026 Major Digital Projects Report 2026. These are portfolio figures, not a private-sector failure rate. |
PMI also reports that professionals who manage complexity effectively increase the likelihood of project success by five times. Treat that as an association reported by PMI, not a guaranteed causal effect or a promise for an individual project. Taken together, the figures point to recurring risks, but they do not establish one percentage that can be applied to every digital initiative.

How to keep a digital project on track
- Define the outcome, not just the deliverable. State what needs to improve, for whom, and how the organization will recognize sustained value. Keep the business case and user needs visible as the team makes delivery decisions.
- Make ownership and decisions explicit. Identify who can resolve scope, funding, design, and operational questions, and involve affected functions early enough to shape the solution. Do not wait until launch preparation to discover a key stakeholder or approval requirement.
- Plan around dependencies, then adapt. Maintain an integrated view of connected workstreams, risks, and decision points. For complex programs, work iteratively across dependent areas instead of assuming every requirement and sequence can be finalized upfront.
- Match people and incentives to the work. Secure experienced technical and business expertise, sufficient capacity, and access to decision-makers. Align team incentives with the project outcome rather than isolated milestones.
- Deliver in short cycles and check quality rigorously. Test integrations, validate requirements with stakeholders, and surface defects early enough to address them. Revisit scope and plans when dependencies or assumptions change.
- Track value alongside delivery. Report schedule and budget, but also whether the work is producing its intended benefit and whether the organization can sustain the improvement. A project that is technically complete is not automatically a successful transformation.
These controls address organizational and delivery causes together. A coordination tool may help teams share work, but buying software by itself does not settle strategy, clarify governance, supply missing skills, or resolve resistance to change.
