Injecting powdered drugs can lead to overdose, tissue damage, serious infection, and blood-borne diseases. The risks depend on the substance, dose, possible contaminants, combinations, and circumstances; a powder’s appearance cannot tell you what it contains, and no injection method makes illicit drug use safe.
Why a powder’s appearance cannot tell you its risks
Powders that look alike can contain different substances or mixtures. The CDC says fentanyl commonly appears as a powder and may be mixed with heroin, cocaine, or methamphetamine without the person’s knowledge. Appearance alone cannot confirm what a substance contains or how strong it is. CDC: Fentanyl

That uncertainty matters because the effects and overdose risks differ across substances. A mixture can also expose someone to an opioid when they do not expect one. In the United States, approximately 69% of all drug overdose deaths in 2023 involved synthetic opioids other than methadone, primarily reflecting illegally made fentanyl, according to the CDC. This figure describes U.S. overdose deaths overall; it is not a statistic about powdered drugs or injection specifically.

Overdose and drug combinations
An overdose can happen when the amount taken is larger than usual, tolerance has fallen, or drugs are combined. Unexpected opioid exposure is a particular concern when a powder may contain fentanyl. The risk profile is not the same for every substance, batch, or person. WHO: Rapid Assessment and Response Guide: Adverse Health Consequences

Contamination and preparation-related exposure
Harm can also come from contaminants in a substance or from contamination during preparation. The WHO identifies contaminants that are introduced or not removed during preparation as a source of adverse health consequences. This is one reason not to treat an unknown powder or improvised preparation as predictable.
Injection-site injury and bacterial infection
Injection can damage tissue and lead to bacterial infection at the injection site. These local harms are distinct from overdose and from infections transmitted through shared equipment; a person can face more than one category of risk.

Blood-borne infections from shared equipment
Sharing syringes or other injection equipment can transmit HIV, hepatitis B, hepatitis C, and other infections. Contaminated syringes and preparation equipment can also spread infection. The CDC describes sterile equipment access and related services as ways to reduce these risks, not as a guarantee that infection or other harm will be prevented. CDC: Infectious Diseases in Persons Who Inject Drugs
What to do if an opioid overdose is suspected
If opioid overdose is suspected, the CDC advises using naloxone. It says naloxone will not harm someone who is overdosing on drugs other than opioids. This guidance applies to a possible opioid overdose; not every powdered drug is an opioid. CDC: Heroin

Where to find prevention, testing, and treatment support
Syringe services programs
In the United States, syringe services programs (SSPs) can provide sterile syringes and other equipment, safe disposal, vaccination, testing, and links to infectious-disease care and substance-use treatment. Services vary by location. The CDC states that nearly 30 years of research shows comprehensive SSPs are safe, effective, and cost-saving, do not increase illegal drug use or crime, and help reduce transmission of viral hepatitis, HIV, and other infections. CDC: Infectious Diseases in Persons Who Inject Drugs
Hepatitis C testing
The CDC recommends hepatitis C testing for anyone who has ever injected drugs, regardless of age, and periodic testing if risk continues. It also recommends one-time screening for all adults. Ask a clinician about hepatitis screening, vaccination, and any other testing appropriate to your situation. CDC: Viral Hepatitis Among People Who Use or Inject Drugs
