Design oversight around the task and its risks, then monitor whether it works; adding a confirmation button by itself is not a safety measure.
When does human oversight make sense?
It depends on the use case, not on a blanket rule that every automated action needs a person to approve it. NIST’s AI Risk Management Framework (AI RMF) recognizes configurations ranging from fully autonomous to fully manual, and says some systems may need human oversight while others may not. Its example: a model used to improve video compression may not need oversight, while other systems may specifically require it. NIST AI RMF 1.0, Appendix C.

Start by mapping the task, the people affected, the operating context, system limitations and potential impacts. Use that picture to decide which actions require human judgment, what the reviewer must be able to do, and what should happen when the system is uncertain or a reviewer objects. Revisit the assessment as system capabilities, risks, benefits and impacts change. NIST treats this as ongoing risk management across the system lifecycle, not a one-time checkpoint. NIST AI RMF Core.
Give the reviewer a real job to do
A person’s presence in the workflow does not automatically make the decision safer. NIST notes that AI opacity and human cognitive biases can influence judgments; depending on the conditions and how the team is organized, human-AI interaction can amplify bias or produce complementarity. A reviewer who cannot understand the relevant information, challenge the system or affect the outcome is not exercising meaningful oversight.

Define responsibilities and communication lines for the AI system, operator, reviewer, decision owner and escalation path. Specify who may approve, reject, request more information, override or pause an action, and who handles appeals or unresolved cases. Train people for the duties assigned to them. NIST’s AI RMF Core includes documented roles, training and differentiated human-AI oversight responsibilities. NIST AI RMF Core.
Present the information needed to judge the particular action in a form the reviewer can interpret: what the system proposes, relevant context, and any material limitation or uncertainty. Give the reviewer a workable way to ask questions or escalate rather than making approval the only usable route. NIST identifies transparency, feedback and appeal mechanisms as relevant parts of risk management; it does not prescribe one universal interface or disclosure format.

Bind approval to the action that will run
An approval is meaningful only if execution remains connected to what the reviewer saw and authorized. The reviewer should see the material operation and its relevant parameters, and the workflow should preserve that operation’s identity through execution. If the operation changes after review, require a new authorization rather than carrying the earlier approval forward.

They characterize their tested cases as a purposive sample, not an estimate of how common such failures are across products. Read the preprint.

The practical design test is whether you can establish that the operation presented for review is the same operation later authorized or released. A confirmation screen alone cannot establish that connection if the underlying operation can change between review and execution.
Make oversight part of the operating cycle
Before launch, decide how the team will evaluate the workflow and detect changes in performance or impact. After launch, monitor performance and review outcomes periodically; maintain routes for feedback, challenge and appeal. Record enough to reconstruct what happened: the operation presented, the relevant context, who reviewed it, what they decided, any changes before execution, and the resulting action. NIST says collecting how often and why people overrule AI output may be useful. Override counts and approval logs can inform review, but a click record alone does not show that a reviewer considered the decision. NIST does not prescribe a universal override rate or threshold; interpret patterns in light of the task and impacts. NIST AI RMF 1.0, Appendix C.
Use a practical design checklist
- Risk rationale: Which actions are gated, and what about their context or potential impact makes human judgment useful?
- Review context: Does the reviewer see the actual proposed operation, relevant parameters, and information needed to assess it?
- Authority and competence: Is the reviewer trained, empowered to reject or challenge, and clear about when and how to escalate?
- Approval integrity: Does the action executed remain bound to the specific operation approved?
- Reconstruction: Can the organization determine what was reviewed, decided and executed?
- Ongoing learning: Are performance and outcomes monitored, with usable feedback, appeal and periodic review processes?
These are evaluation questions drawn from NIST’s guidance and the approval-binding concerns described in the preprint; they are not a NIST product checklist.
