How Weak Network Segmentation Lets Attackers Reach More of the Enterprise

Network segmentation can limit attackers’ routes from a compromised device, but only when boundaries are correctly designed, enforced, monitored and tested.

Weak network segmentation can turn one compromised device into a route toward systems it should never reach. Effective segmentation limits which connections can cross between groups of resources, reducing opportunities for lateral movement. It is not a guarantee against attacker movement: boundaries must be correctly designed, enforced, monitored and maintained, and network location alone cannot establish trust.

What is network segmentation?

Network segmentation divides systems into zones or groups and controls the traffic allowed between them. The goal is to permit necessary communication while preventing unnecessary access from one group to another. Zones might be organized around users, applications, critical services, production systems or operational technology (OT).

A VLAN, firewall rule or network diagram can help implement or describe a boundary, but none proves by itself that access is constrained. The meaningful test is whether controls block unauthorized flows between resources in practice. MITRE ATT&CK lists physical separation, VLANs, firewalls, routers, cloud configurations and software-defined workload segmentation as possible mechanisms.

How does segmentation limit lateral movement?

After gaining an initial foothold, an attacker may try to reach other hosts, accounts, applications or sensitive environments. If those resources can communicate broadly, the compromised system may offer a path farther into the organization. Segmentation narrows the connections available along that path: a user workstation, for example, should not automatically be able to initiate connections to every production or critical system.

CISA’s #StopRansomware Guide says, “Network segmentation can help contain the impact of any intrusion affecting your organization and prevent or limit lateral movement on the part of malicious actors.” The qualification matters: segmentation can reduce opportunities and contain an intrusion’s impact, but it cannot ensure an attacker will be stopped. Other routes, credentials, misconfigurations or unsegmented connections may still provide access.

Segmentation is also not a substitute for identity and access controls. NIST’s Zero Trust Architecture (SP 800-207, 2020) says that zero trust does not grant implicit trust based solely on network location. A system should not be considered trustworthy simply because it is inside a particular segment; access should be evaluated for the resource being requested.

Why does network segmentation fail?

There are no meaningful boundaries

CISA and NSA identify lack of network segmentation as a common misconfiguration. Without effective boundaries between user, production and critical-system networks, a compromised host may have a wider range of systems within reach. Their 2023 cybersecurity misconfigurations advisory highlights this as a risk that can increase ransomware exposure.

Boundaries exist but are enforced inconsistently

A boundary on a diagram may not match actual routes, firewall rules, host configurations or cloud networking. In a 2023 report describing a red-team assessment conducted in 2022, CISA said its team moved laterally across geographically separated sites despite the organization having logical and geographic boundaries. The assessment found misconfigured systems and insufficient monitoring. It is a case study, not an estimate of how common this failure is.

Policies are too broad, impractical or out of date

Rules that allow more traffic than the work requires can leave paths open between zones. At the other extreme, fine-grained policies can be challenging to develop and maintain. CISA’s 2025 microsegmentation guidance notes that coarser segments may be easier to manage but can call for additional protection and visibility. Policies that do not reflect real application dependencies can also disrupt business functions or prompt exceptions that weaken the intended controls.

Monitoring and testing are missing

Rules can change, systems can be added and network paths can be misconfigured. Without flow monitoring and periodic checks, an organization may not know that a formerly blocked connection is now allowed, or that an exception has become an unreviewed route between zones. MITRE ATT&CK’s enterprise Network Segmentation mitigation (M1030, version 1.2, last modified May 12, 2026) recommends reviewing firewall rules and access control lists (ACLs), monitoring network flows and periodically testing whether unauthorized access between segments is blocked.

IT and OT are not adequately separated

Connections between enterprise IT and OT environments can expose systems that control or support physical processes. CISA and NSA identify inadequate IT/OT segmentation as a risk to OT environments. MITRE ATT&CK’s ICS Network Segmentation mitigation (M0930, version 1.1, last modified May 12, 2026) recommends isolating critical systems, restricting access to required systems and services, and using controlled conduits between zones.

How should an organization plan segmentation?

There is no universal zone layout or deployment sequence that suits every organization. The following checklist reflects CISA guidance and MITRE recommendations; teams should adapt it to their architecture, operational needs and tolerance for disruption.

  1. Inventory resources and dependencies. Identify candidate systems and determine what each one needs to connect to—and which other systems depend on it. CISA’s 2025 microsegmentation guidance recommends validating dependency lists before designing policy.
  2. Set the security objective. Decide which movement or exposure the boundary is meant to reduce, such as access from user devices to critical services or direct connectivity between enterprise IT and process-control networks.
  3. Choose a boundary model. Compare finer-grained policies with broader zones against the organization’s architecture and workflows. Consider expected reduction in lateral movement, operational effort, monitoring and visibility needs, and the ability to maintain accurate rules.
  4. Define allowed communications. Specify required sources, destinations, services and directions of traffic. Enforce those rules with controls appropriate to the environment, such as firewalls, routers, VLANs, cloud configurations or software-defined workload controls. A device or technology choice alone does not establish an effective policy.
  5. Deploy in stages. Monitor and test policy changes as they are introduced. Plan how to reverse a change if it disrupts a business function; operational recovery should be part of the rollout, not an afterthought.
  6. Review and validate continuously. Review rules and logs, monitor flows for unexpected activity and periodically test whether unauthorized connections between segments are blocked. Update policies when dependencies or systems change.

How should IT and OT networks be segmented?

Do not assume that placing OT systems on a separate logical network is sufficient. Identify zones according to system criticality, potential consequences and operational need. Restrict connections between enterprise networks and process-control environments to the systems and services that must communicate, using controlled conduits rather than broad access. MITRE’s ICS guidance supports isolating critical systems and restricting access to required services.

Operational dependencies matter: a policy that blocks a required process may cause disruption, while an exception added to restore service can create an unexamined path. Map and validate dependencies before enforcement, then stage changes with monitoring and a recovery plan.

How can teams tell whether segmentation is working?

Validate the controls themselves rather than relying on intended design. MITRE recommends reviewing firewall rules and ACLs, monitoring network flows and periodically testing whether unauthorized access between segments is blocked. In practice, teams should test representative prohibited paths as well as confirm that required workflows continue to function.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Compare actual rules and routes with the approved policy; investigate broad permissions, unexpected exceptions and stale entries.
  • Review network-flow data for communication that crosses boundaries without a documented business need.
  • Periodically test whether prohibited paths are blocked, including paths introduced by system, network or cloud changes.
  • Monitor staged policy updates for blocked legitimate workflows and use the planned rollback process if a change disrupts business operations.

Testing demonstrates how controls behave under the conditions checked; it does not prove that every path is blocked or that a boundary will remain effective after future changes. Ongoing review is needed because systems, dependencies and policies evolve.

Why segmentation belongs in a layered security approach

CISA’s 2025 release on microsegmentation describes it as “a critical component of ZTA that reduces the attack surface, limits lateral movement, and enhances visibility for monitoring smaller, isolated groups of resources.” Microsegmentation can make policies more precise, but CISA also notes the challenge of developing and maintaining fine-grained controls.

Segmentation should therefore complement, not replace, identity-based access decisions, monitoring and other security controls. NIST cautions that attackers can adapt and target processes where zero-trust principles have not been applied. Treating a segment as an automatic trust boundary risks recreating the broad access that segmentation is meant to constrain.

The available CISA and NSA material identifies concrete failure modes, and CISA’s red-team report documents a specific assessment. These sources do not establish a prevalence rate or prove that segmentation failures are increasing over time.
