The pharmacy locker code in Silent Hill: Townfall is 6539. First use the Scalpel from the clinic to open the pharmacy entrance; then enter the code on the rusty locker in the back room. The locker contains the Prescription Dosage Error note, which you need for the clinic scales.
How to reach the pharmacy locker
- Use Zoe’s Lanyard to enter the clinic.
- Find the Scalpel in the green box in the Z Ellis room.
- Return to the pharmacy entrance and use the Scalpel to get inside.
- In the pharmacy, inspect the posters and use the CRTV broadcast to learn the order for reading their clues.
- Go to the rusty locker in the back room, enter 6539, and take the Prescription Dosage Error note.
How the pharmacy locker code works
The CRTV broadcast establishes the poster order. Read the number on each poster in that sequence to form the combination:

- 6: The handwashing poster shows six fingers. It is in the bathroom.
- 5: The “Prescription drugs and you” poster gives five steps. Find it in the hallway to the right of the phone.
- 3: The blood-pressure poster shows a reading of three. It is near the front window.
- 9: The “Taking Antibiotics” poster shows nine pills remaining. It is on the noticeboard by the lockers.
In that order, the clues make 6539. Walkthroughs from Silent Townfall and Townfall Guides describe the same code and clue sequence.

The combination is for the rusty locker inside the pharmacy, not its entrance. If you are still outside, return to the clinic route and get the Scalpel from the green box in the Z Ellis room. Use it on the pharmacy entrance, then enter 6539 on the back-room locker.

What the locker note is for
The Prescription Dosage Error note is one of three items used with the clinic scales. The other two are the Broken Stethoscope and Zoe’s Lanyard, as listed in the Silent Hill Wiki puzzle guide.
How well is the solution verified?
Three walkthrough and puzzle-guide sources agree on 6539 and the locker’s purpose. The available coverage is not an official Konami code sheet, and one guide says it has not replayed the puzzle on every difficulty. The code is corroborated by those guides, but difficulty-by-difficulty verification is not established.
