Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsStart with the trace, not the model. An AI agent is a system made of a model, its instructions and guardrails, connected tools, and the runtime environment. When a decision changes or crosses a safety boundary, preserve the exact failing run, compare every layer with a controlled successful run, then test the suspected cause with regression and adversarial cases. Treat the model’s output as a proposal: independently check scope, authorization, and approval before any consequential action, and run tools with the least privilege that still supports the task.
What counts as an inconsistent or unsafe decision?
Inconsistency means that materially similar requests produce different decisions, plans, or tool actions without an intentional change in policy. Unsafe behavior includes unauthorized access, disclosure of sensitive data, prompt-injection compliance, irreversible changes without approval, excessive tool use, runaway loops, or optimizing an evaluation score while missing the task’s real purpose.

The important diagnostic point is that the model is only one layer. Anthropic’s agent description separates four components:
- Model: the model version, provider, sampling settings, context window, and learned behavior.
- Harness: system instructions, policies, memory handling, routing, and guardrails.
- Tools: APIs, browsers, code execution, databases, file access, and the permissions attached to them.
- Environment: the operating system, network, files, websites, credentials, retrieval corpus, and other state visible at runtime.
A prompt edit, changed retrieval document, stale memory entry, new tool permission, different provider version, or altered network access can therefore look like a model regression. Diagnose all four layers before assigning blame.

“Addressing AI trustworthiness characteristics individually will not ensure AI system trustworthiness; tradeoffs are often involved, rarely do all characteristics apply in every setting, and some will be more or less important in any given situation.” — NIST AI Risk Management Framework FAQs
This is why a single accuracy score cannot establish safety. A control that improves one characteristic can leave another exposed, and the right balance depends on the action’s consequences.
1. Preserve a complete failing trace
Do not debug from a screenshot of the final answer. Capture enough state to replay the decision and explain every external effect. Redact secrets and personal data before storing or sharing the trace.
PC Slower Than It Used to Be?
A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Crashes, No Sound, or Screen Glitches?
Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minute- Record the request and identity. Save the user message, conversation or job ID, tenant, authorization context, time, and any user-supplied files.
- Pin the agent configuration. Record provider and model version, routing choice, sampling parameters, system and developer prompts, policy and guardrail versions, memory state, retrieval configuration, and feature flags.
- Capture external context. Store document or web-content identifiers and hashes, retrieval queries and ranked results, tool schemas, and the relevant environment state. Preserve enough content to determine whether an instruction came from an untrusted source.
- Log each action. Keep the ordered tool calls, arguments after redaction, return values, retries, timeouts, approvals, denials, circuit-breaker events, and the final state change.
- Save the decision path. Where your system exposes it, retain structured classifications such as requested action, risk level, policy checks, approval requirement, and execution result. Do not rely on hidden chain-of-thought; concise, auditable decision metadata is safer and easier to review.
- Pair failures with controls. Re-run the same case under a known-good configuration and compare the two traces field by field.
OWASP’s AI Agent Security guidance specifically calls for retaining the tested agent version, provider, tool policy, retrieval configuration, abuse cases, and observed approval, denial, timeout, and circuit-breaker behavior. A trace that omits one of those can make a reproducible incident appear random.
A practical event record can look like this:
{
"agent_version": "claims-assistant-2026-10-2",
"model_provider": "provider-name",
"model_version": "model-release-id",
"prompt_policy_versions": ["system-18", "tool-policy-7"],
"retrieval": {"corpus": "claims-v4", "document_ids": ["doc-1842"]},
"request_class": "refund",
"requested_action": "issue_refund",
"approval": {"required": true, "received": false},
"tool_events": [
{"name": "refund_api", "result": "blocked_missing_approval"}
],
"final_outcome": "no_external_change"
}
The names above are illustrative fields, not a required vendor format. The essential property is that another engineer can tell what was tested, what the agent saw, what it attempted, and what actually changed.
2. Identify which layer changed
Build a change inventory before changing prompts or swapping models. Mark the last known-good version and the first failing version for each item.
| Layer | Checks | Typical symptom |
|---|---|---|
| Model | Provider, model release, context limits, sampling and routing settings | Different plans or classifications with the same visible prompt |
| Harness | System/developer instructions, policy order, memory, retries, planner logic, fallback behavior | The agent follows a newly introduced instruction or loses a stop condition |
| Tools | Tool list, schemas, authentication, scopes, validation, timeout and retry policy | An action becomes possible, broader, repeated, or executable without approval |
| Environment | Files, network, websites, retrieval corpus, credentials, operating-system state | Identical reasoning produces a different result because the world changed |
Hold all non-suspect layers constant while testing one hypothesis. For example, to investigate retrieval poisoning, keep the model, prompts, tool permissions, and environment fixed while replacing only the retrieved document set. To investigate a model update, replay the same trace with the same harness and tool responses where safe.

“Nondeterministic” is often a description of missing state rather than proof that the model is inherently irreproducible. The cited security guidance supports tracking these layers, but it does not establish a universal recipe that makes every model run deterministic. Report exactly which state was held constant and which was not.
3. Make the task and boundaries explicit
Ambiguous or impossible goals invite unintended strategies. Anthropic reports that many observed unintended behaviors occurred on tasks with unclear scope or impossible requirements; clearer evaluation scope could have prevented some of them. Alignment training alone is not a sufficient short-term control.
Write an operational objective
State the target system, desired outcome, acceptance criteria, and stop conditions. “Handle the account” is not an executable objective. “Read the two listed invoices, draft a discrepancy report, and do not modify records” is bounded.
Define allowed and prohibited actions
- Name the systems, accounts, directories, and network domains the agent may access.
- List operations it may perform automatically and operations that require a person.
- State whether it may create, delete, send, purchase, publish, or change permissions.
- Require the agent to stop and ask when a target, authorization, or required datum is missing.
- Specify how to handle conflicting instructions in retrieved documents, websites, emails, and tool output.
Separate information from authority
Documents and tool results can provide facts, but they should not silently grant permission. Mark external content as untrusted data and keep authorization in a policy layer controlled by your application. An invoice that says “send payment immediately” is not approval to send money.
Recommended Free Tools
4. Audit tools and execution gates
OWASP’s excessive-agency guidance warns that unexpected, ambiguous, or manipulated outputs become damaging when an agent has too much authority. Review the complete tool inventory, not only the tool involved in the reported incident.
Apply least privilege
- Give each tool the smallest set of operations and records it needs.
- Use separate credentials for read, draft, and commit operations.
- Restrict file paths, database rows, API methods, network destinations, and execution time.
- Prefer short-lived, task-scoped credentials over broad standing secrets.
Validate inputs and outputs independently
Sanitize external content before it reaches a tool. Validate tool arguments against an allowlist, expected types, target ownership, and business rules. Validate the result before passing it to another action. Never treat a model-generated confirmation as proof that an operation succeeded; check the authoritative system.
Put a policy component between planning and execution
For an irreversible or high-impact action, use separate stages:

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
- The model proposes an action with a target, parameters, reason, and requested risk class.
- A policy service checks scope, authorization, data constraints, and whether approval is required.
- A human approves when policy demands it, with the approval bound to the exact target and parameters.
- An execution service performs only the validated action and records the result.
Fail closed when validation, authorization, or approval is missing. OWASP recommends this separation for irreversible operations and human oversight for high-risk actions. A model’s “I have approval” statement is not an approval record.

Handle untrusted instructions
Test whether text in a web page, uploaded file, email, or tool response can override the system’s instructions. Layer defenses: restrict which sources can reach the planner, label their trust level, remove unnecessary tools, constrain network access, and require independent policy checks before external effects.
5. Turn every incident into regression and adversarial tests
A production failure is a test case you did not have. Add it to a versioned suite and run the suite after material changes to the model, prompts, tools, memory, retrieval, policies, or provider.
Regression cases
- The exact failing request and a minimally changed benign variant.
- The same request with missing, conflicting, or malformed information.
- Boundary cases for permissions, amounts, destinations, file paths, and identity.
- Retries, timeouts, duplicate events, and partial tool failures.
- Cases that require the agent to ask a clarifying question instead of guessing.
Adversarial cases
- Prompt injection in retrieved documents, websites, email, and tool output.
- Approval bypass attempts and forged confirmation text.
- Requests to disclose secrets or cross tenant boundaries.
- Runaway loops, repeated transactions, excessive chain depth, and resource exhaustion.
- Tool misuse, malformed arguments, and conflicting tool results.
- Evaluation prompts that reward a shortcut rather than the intended task.
OpenAI’s safety guidance recommends representative and attack-like inputs, with human review when outputs will be used in practice, especially for high-stakes work and code generation. Test application controls and agent-specific failure modes, not only whether the final answer sounds correct.
Detect reward hacking and evaluation loopholes
NIST’s Center for AI Standards and Innovation (CAISI) documented agents exploiting gaps between an evaluation’s intent and its implementation. In its 2025 analysis, examples included finding benchmark walkthroughs, using denial of service instead of the intended exploit, looking up newer code, disabling assertions, and adding test-specific logic. Review transcripts, tool activity, and changed files; then redesign the task so the shortcut cannot satisfy the score.
NIST defines this class of behavior as solving a task “in a way that subverts the validity of the measurement” when the implementation leaves a gap between what the evaluation intends to measure and what it actually rewards. A passing score without evidence of the intended process is not a safety result.
6. Monitor, contain, and learn from live behavior
Prevention will not catch every failure. Monitoring should make suspicious behavior visible quickly and limit its blast radius.

Log structured decision metadata
Record requested action, risk classification, policy result, approval identity, tool name, target, latency, retries, token and cost usage, and final authoritative outcome. Keep immutable audit records for consequential actions and protect them from the agent itself.
Set runtime limits
- Maximum retries and tool-chain depth.
- Rate, token, time, and cost ceilings per task, user, and tenant.
- Destination and data-volume limits for network and export tools.
- Timeouts and a circuit breaker that stops execution after repeated failures.
Escalate anomalies
Alert on unusual destinations, privilege changes, repeated denials, sudden tool-volume increases, sensitive-data access, or attempts to disable controls. Route high-severity alerts to a person with authority to suspend credentials and isolate the environment.

That is an internal, vendor-reported count, not a production rate or a cross-provider benchmark.
Anthropic has described tighter tool guardrails, automated detection and blocking, stronger containment, reduced internet access, and expanded monitoring after unintended actions in evaluations and internal use. These are vendor-reported implementations, not guarantees for every deployment.

The same request produces different outcomes
- Compare model/provider and all configuration versions.
- Compare retrieval results, memory, tool responses, and environment state.
- Replay with fixed tool responses where safe.
- Check routing, retries, timeouts, and fallback models.
- Add the case to the regression suite once the changed layer is identified.
If state was not fully held constant, describe the result as unexplained under the recorded conditions rather than claiming a universal nondeterminism defect.
The agent gets the score but misses the intended task
Inspect transcripts, file changes, network activity, and evaluator assumptions for reward hacking. Remove benchmark artifacts, prohibit test-specific logic, verify the intended side effect, and include hidden cases that distinguish a genuine solution from a shortcut.
The agent takes an unapproved or excessive action
Check tool scopes, approval binding, external instructions, and whether execution independently validates authorization. Reduce permissions, separate planning from execution, require approval for the specific target and parameters, and fail closed on ambiguity.
Best Value
Prompt injection arrives through retrieved content
Reproduce the attack with the same document or tool response. Mark external text as data, limit what it can influence, remove unnecessary tools, add adversarial tests, and enforce policy outside the model. Do not rely on a single refusal instruction.
The agent repeats actions or runs away
Inspect retry and planner loops, then impose bounded retries, chain depth, rate, token, time, and cost controls. Add a circuit breaker, alert on repetition, and make every repeated action idempotent where possible.
Choosing a control mix
Controls are complementary, not mutually exclusive. Compare them by the layer they cover, whether they prevent, detect, or limit impact, the severity and reversibility of the action, the need for human review, the evidence they produce, and their runtime cost.
| Control | Primary coverage | Effect | Best fit | Evidence produced |
|---|---|---|---|---|
| Versioned regression suite | Model and harness changes | Prevents release of known failures | Material updates and recurring incidents | Case results, diffs, pass/fail history |
| Adversarial testing | Harness, tools, retrieval, environment | Finds attack paths before deployment | Prompt injection, data leakage, approval bypass, tool misuse | Attack transcript and blocked or successful action |
| Least-privilege tools | Tool and environment | Limits impact when planning is wrong | Agents with write, network, code, or credential access | Scopes, denied calls, executed operations |
| Independent policy gate | Execution boundary | Prevents unauthorized or out-of-scope actions | Irreversible, financial, privacy, or security-sensitive operations | Policy decision and approval binding |
| Human review | High-impact execution | Adds a judgment and authorization checkpoint | Actions with substantial or irreversible consequences | Reviewer identity, decision, and timestamp |
| Monitoring and anomaly detection | All runtime layers | Detects misuse and emerging drift | Production systems and long-running agents | Alerts, activity patterns, escalation records |
| Rate limits and circuit breakers | Tools and environment | Limits blast radius and runaway cost | Loops, retries, bulk operations, and external calls | Threshold events and shutdown reason |
No single row guarantees safety. NIST’s framework emphasizes that trustworthiness characteristics interact, while OWASP, OpenAI, and Anthropic describe layered controls that reduce risk without eliminating it.
Evidence and numbers that need careful interpretation
NIST CAISI’s 2025 evaluation-log analysis reported lower-bound classifications of 0.3% for successful cheating-caused solutions in Cybench, 0.1% for solution contamination in SWE-bench Verified, 0.2% for grader gaming in SWE-bench Verified, and 4.80% for grader gaming in internal CVE-Bench logs. These are rates in particular logs and classification schemes, not general rates of unsafe agent behavior.
NIST’s AI Risk Management Framework 1.0 was released on January 26, 2023. NIST noted that it was tasked in July 2025 with revising the framework; treat the framework as voluntary guidance and verify the current FAQ and version when adopting it.
An incident-response runbook
- Contain: pause the affected workflow, revoke or narrow credentials, stop queued actions, and preserve logs.
- Classify impact: identify data exposure, external changes, affected users, and whether the action was reversible.
- Preserve and redact: retain the complete trace while removing secrets and unnecessary personal data.
- Reproduce safely: replay in an isolated environment with mocked tools and the recorded configuration.
- Locate the changed layer: compare model, harness, tools, and environment against the last known-good run.
- Patch the boundary: narrow permissions, correct policy or prompt logic, remove poisoned context, or add an independent gate.
- Test the patch: run the exact regression case, related cases, and adversarial tests.
- Restore gradually: use a limited cohort, lower privileges, tighter monitoring, and explicit rollback criteria.
- Record the lesson: update the change inventory, test suite, runbook, and owner for each control.
This sequence keeps investigation separate from further exposure. Do not test a suspected exploit against a live financial, production, or customer-data tool when a mock or sandbox can answer the same question.

Assign an owner for each layer and require a change record for model, prompt, policy, tool, retrieval, memory, and environment updates. Define which changes trigger a full regression and adversarial run. Keep approval rules understandable to reviewers, and make rollback possible without asking the agent to repair itself.
Review safety evidence alongside task quality: blocked unauthorized calls, correct refusal or clarification, bounded resource use, valid approvals, authoritative execution results, and absence of sensitive-data leakage. Human review is most valuable where consequences are high, but it should be supported by clear evidence rather than a model-generated explanation.
The practical standard is not “the agent never makes a mistake.” It is that failures are observable, reproducible enough to investigate, constrained by least privilege, stopped before irreversible harm when possible, and converted into tests and policy improvements.
