How to Track Prior Authorization Changes: A Practical Guide for Providers and Patients

A practical method for logging authorization submissions, status changes, information requests, decisions, and payer policy updates—without assuming every CMS deadline applies to every plan.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Track each prior authorization as a dated case record: who the request went to, what was requested, when and how it was submitted, every status change, any follow-up information requested, and the final decision and approved scope. Keep the record current against payer notices and portal updates. Separately track payer-rule and system changes, because CMS-0057-F dates and requirements depend on the payer and program involved.

Build one dated record for each authorization request

Use one case record per request, with a timestamped history rather than a single overwritten status field. This makes it possible to tell what changed, when it changed, and who needs to act next. The fields below are an operational workflow recommendation, not a CMS-mandated log template.

As an Amazon Associate I earn from qualifying purchases.

  • Case and privacy: patient name or internal case identifier, stored and shared according to your organization’s privacy controls.
  • Payer and benefit: payer, plan, and whether the request concerns a medical or pharmacy benefit.
  • Request details: service, item, procedure, or medication; ordering clinician; and destination provider where useful.
  • Requirement check: whether authorization is required, when that was checked, and the source used to verify it.
  • Submission trail: submission date and time, channel (portal, API, fax, phone, or other), and confirmation or reference number.
  • Status history: each status and timestamp, retaining earlier entries rather than replacing them.
  • Information requests: what additional material the payer requested, when the request arrived, and when and how the response was submitted.
  • Decision: decision date; approval, denial, or still pending; any specific denial reason; approved scope; and authorization end date or ending circumstance.
  • Next action: responsible owner, due date, escalation or appeal status, and the next planned follow-up.

CMS describes the intended Prior Authorization API capabilities as including checking whether authorization is required, viewing covered items and services, identifying documentation requirements, and exchanging requests and responses. Its API guidance distinguishes approval, denial, and requests for more information. See the CMS fact sheet and CMS general FAQ.

Reconcile the record with payer updates

Check the payer’s portal, notices, or API response on a defined schedule that fits the request’s urgency and your applicable program rules. Record the time and source of each update. If a portal and notice appear inconsistent, preserve both, contact the payer through an appropriate channel, and record the clarification rather than silently choosing one version.

A response can approve the request and state the date or circumstance when authorization ends, deny it with a specific reason, or ask for more information. Make the next action explicit for pending cases: who will obtain the requested material, when it is due, and how submission will be confirmed. Do not treat a portal status such as “received” as a decision.

Track payer and policy changes separately

Keep a second change log for rules, payer policies, and technology updates that may affect many requests. For each entry, capture the guidance or rule title, its publication or update date, affected payer or program, applicable effective or compliance date, and the local workflow or system change needed. Assign someone to recheck official implementation materials before changing procedures.

CMS released the Interoperability and Prior Authorization Final Rule, CMS-0057-F, on January 17, 2024. CMS says operational provisions generally begin January 1, 2026, while API development and enhancement requirements generally begin January 1, 2027; exact dates vary by payer type. These are not universal deadlines for every insurer or authorization workflow. Consult the CMS implementation page and fact sheet for the relevant payer category.

CMS encourages implementers to consult HL7 FHIR Da Vinci guides, including Coverage Requirements Discovery (CRD), Documentation Templates and Rules (DTR), and Prior Authorization Support (PAS). These are technical implementation resources, not consumer tracking apps; CMS discusses them in its fact sheet.

321Done Order Form Pad with Carbon Copies, 2-Part, 5.5x8.5 Inches, 50 Sets
  • Keep a copy of every order: this 2-part carbonless order form pad copies what you write onto the sheet below as you write it, so your customer takes the white top form and you keep the yellow duplicate
  • Half letter size, 5.5 x 8.5 inches: small enough for a register drawer, tote, or booth box, printed on thick paper stock, 40% thicker than standard carbon copy forms, so it stands up to ballpoint pen without bleeding through
  • Fifty sets on a chipboard back: 50 carbonless sets edge glued onto a stiff chipboard backing, so each set tears away cleanly and the pad stays flat while you fill it in
  • Blank space for your own brand: an open area at the bottom left takes your stamp, label, or handwritten note, and the undated generic fields suit any product, service, or price you sell
  • Made in USA, simple to use: assembled and printed in the United States, 100 pages across 50 duplicate sets, and it works as a receipt book, invoice pad, sales order book, or carbonless purchase order form for small business

Understand what CMS-0057-F does—and does not—cover

The rule applies to specified impacted payer types and regulated lines of business, including Medicare Advantage organizations, state Medicaid and CHIP programs, Medicaid managed-care plans and CHIP managed-care entities, and certain Qualified Health Plan issuers on Federally-facilitated Exchanges. It does not automatically cover every insurer or every authorization process.

Drug prior authorizations are generally excluded from the rule’s API and process requirements. CMS says payers are not prohibited from including certain drugs covered under a medical benefit in Prior Authorization APIs. Check the benefit and payer context before applying a process or deadline. CMS also says applicable response timeframes are measured in calendar time and apply regardless of submission channel; program-specific applicability and exceptions still matter. See the CMS general FAQ and process FAQ.

CMS says required prior-authorization data must remain accessible for at least one year after the last status change. That is an API data-access requirement, not a replacement for an organization’s own record-retention policy. CMS also requires impacted payers to post annual prior-authorization metrics, with initial reporting beginning in 2026 for the prior year. A payer statistic is meaningful only alongside its metric definition, reporting period, and source document; the reporting requirement itself is not a performance benchmark. See the rule page and process FAQ.

Choose a tracking method that preserves the audit trail

A spreadsheet can work for a small volume if staff consistently timestamp changes, assign owners, and restrict access appropriately. For higher volume, evaluate an EHR or practice-management system, clearinghouse, or API-connected workflow software against the same practical needs:

If your workflow needs screenshots of payer portal pages for internal records, ScreenshotNeo can capture a URL with one request. Cookie banners are accepted and removed before capture, along with known consent platforms, newsletter popups, and chat widgets; each step can be turned off. Bot checks, blank pages, and failed loads are never billed, and response headers identify page verdict and billing status. Its MCP server gives AI agents tools to take screenshots, get page information, and capture PDFs. The free plan includes 1,000 screenshots a month with no card; paid plans start at $5 for 3,000. Do not use a screenshot as a substitute for the payer’s official decision record or your organization’s privacy and retention rules.

Common tracking failures and fixes

  • Status was overwritten: preserve an event history with timestamp and source for every change; recover details from notices or portal history where available.
  • Submission cannot be verified: save the confirmation or reference number and the channel used. If none was issued, contact the payer and log the follow-up and response.
  • Information request was missed: record the request date, specific materials needed, owner, and response due date; set a reminder and retain proof of resubmission.
  • Approval scope or end point is unclear: obtain clarification from the payer and record the covered service and stated end date or circumstance rather than assuming approval covers the full requested scope.
  • A deadline was applied to the wrong payer or benefit: verify the program, line of business, request type, and current CMS or payer guidance before using a timeframe.
  • Portal and notice disagree: retain both versions and timestamps, seek payer clarification, and document the resolution.
