In the U.S., the documented Apple Watch route is Dexcom G7: the watch can mirror readings from the iPhone app or, with Direct to Watch enabled, receive readings and alerts from the G7 transmitter. The G7 is a prescription CGM; check current device compatibility, app versions, and safety notices before relying on it.
What a smartwatch does—and does not do
A CGM sensor inserted under the skin measures glucose in interstitial fluid. Its transmitter calculates estimated glucose values and sends them to a supported display, such as a phone, receiver, or compatible smartwatch. The watch is a screen for CGM data, not a non-invasive glucose sensor.

The FDA says it has not authorized, cleared, or approved any smartwatch or smart ring intended to measure or estimate glucose on its own. It warns that inaccurate readings can lead to harmful diabetes-treatment decisions, including taking the wrong amount of insulin or another glucose-lowering medicine. See the agency’s February 21, 2024 safety communication.

The FDA-reviewed G7 system includes a sensor, transmitter, and at least one display device. For compatible Apple users, that can include the G7 iPhone app and Apple Watch app. The reviewed system is prescription-only and intended for diabetes management. Its two watch modes differ mainly in whether the iPhone must stay nearby.

- HEALTHY GLUCOSE SUPPORTS HEART HEALTH. What you eat matters to your glucose and your heart. Keeping your glucose in a healthy range (70–140 mg/dL) more often can help protect your heart from heart disease²⁻⁴.
| Mode | How readings reach the watch | Phone needed nearby? |
|---|---|---|
| Standard smartwatch mode | The G7 Watch App mirrors data from the iPhone G7 app. | Yes. The phone must be close to the watch. |
| Direct to Watch | After setup, the watch app connects directly to the G7 transmitter for estimated glucose values and alerts. | No, not for receiving readings and alerts once enabled; the iPhone app is still used for setup and settings. |
These modes and their setup requirements are described in the FDA’s G7 review and Dexcom’s Direct to Watch guidance. Direct to Watch requires a compatible iPhone and Apple Watch, an active sensor session, and enabling/configuring the feature in the iOS app. The FDA review does not provide a complete current model-by-model compatibility list, so confirm supported models, operating systems, and app versions in Dexcom’s live compatibility information before purchase or use.
Set up tracking and keep a backup display
- Confirm compatibility first. Check the manufacturer’s current phone, watch, operating-system, and app requirements. Do not assume every Apple Watch or iPhone works.
- Use the iPhone app to start and configure. Start the G7 sensor session in the app. To use Direct to Watch, enable and configure that feature there, then follow Dexcom’s current in-app instructions.
- Check connectivity and alert settings. Bluetooth and recommended device settings are part of the workflow. Confirm that readings and alerts appear on the intended display rather than assuming pairing alone is sufficient.
- Retain access to another display. The phone app can remain an alternative, and the separate G7 receiver is another display option that can communicate with the transmitter and show readings, graphs, and alerts. A watch is optional, not the CGM itself.
- Review current safety notices and updates. Check FDA and manufacturer notices for the specific app version you use and install required corrections before relying on readings or alerts.
What the G7 specifications mean in practice
The FDA-reviewed submission describes Bluetooth Low Energy communication, a reportable range of 40–400 mg/dL, with values outside that range shown as LOW or HIGH, and wear of up to 10 days plus a 12-hour grace period. These are details for the reviewed G7 system submission; they should not be treated as universal CGM specifications or assumed to apply to every market or product generation. Follow the labeling for your device.

Why app versions and corrections matter
Watch alerts and readings depend on functioning software as well as compatible hardware. FDA’s recall record says a correction initiated April 14, 2026, and posted May 11, 2026, covered G7 watchOS app versions 2.9.0 through 2.11.2. The issue could delay glucose-value and alert processing, creating a risk of missed detection or decisions based on outdated information. The FDA record showed the recall as open/classified when accessed for this article; check the live FDA record for current status and follow Dexcom’s correction instructions.
A separate FDA notice describes a failure-to-alert issue after transmitter failure in specified older G7 app versions. Those affected versions were to be updated; the notice is not evidence that every current app version is affected. Check the FDA correction notice and manufacturer guidance for the versions and instructions that apply to your setup.

Choosing a display route
- For convenience: A compatible watch can put CGM data and alerts on your wrist. Standard mirroring depends on a nearby iPhone; Direct to Watch avoids that proximity requirement after setup.
- For redundancy: Consider whether you can also access the phone app or a separate receiver, rather than making the watch your only display.
- For compatibility: Verify exact supported devices and software with the manufacturer; general Apple Watch ownership alone does not establish support.
- For clinical fit: Because the G7 is a prescription system, discuss whether it is appropriate for you with your healthcare professional.
The FDA announced U.S. authorization of Abbott’s Libre Duo 10 Day Continuous Dual Glucose Ketone Monitoring System on August 25, 2026, for people aged 2 and older living with diabetes. The announcement describes readings sent wirelessly to a compatible smartphone, but does not establish smartwatch-display support. Do not assume Libre Duo works with a watch unless its current official compatibility documentation confirms that feature.
