How to Tell Flu, COVID-19, and a Common Cold Apart

Symptoms can offer clues, but they cannot reliably distinguish flu from COVID-19. Learn when testing matters, how to read a negative rapid result, and when to seek care.

You usually cannot tell flu and COVID-19 apart by symptoms alone. They share many symptoms, and a cold can overlap with both. A cold often starts gradually and stays milder; flu often comes on abruptly and feels more intense. Those patterns are clues, not a diagnosis. Testing is needed to confirm flu or COVID-19.

If you are at higher risk of severe illness, contact a healthcare provider promptly when respiratory symptoms begin—even if they seem mild. Treatment can be time-sensitive.

What symptoms can—and cannot—tell you

Fever or chills, cough, sore throat, congestion, fatigue, aches, and headache can occur with more than one of these illnesses. Some people with flu or COVID-19 also have gastrointestinal symptoms. COVID-19 can cause changes in taste or smell more often than flu, but no single symptom proves which virus you have.

The CDC puts the central limitation plainly: “You cannot tell the difference between flu and COVID-19 by the symptoms alone because many of the signs and symptoms are the same.” That statement appears on its Similarities and Differences between Flu and COVID-19 page, dated September 17, 2024.

A cold is commonly milder and more focused on the nose and throat, but symptoms called a “cold” can also come from other respiratory viruses. Symptom patterns can help you decide whether to test or seek advice; they cannot reliably establish the cause.

That distinction matters because the same symptom can have different causes. A cough, for example, may accompany a cold, flu, or COVID-19. Congestion and a sore throat may feel like a familiar cold, but they can also occur with flu or COVID-19. Conversely, a person with a confirmed respiratory infection may not have every symptom commonly associated with it. Symptoms are useful clues about how ill you feel and whether you need care, not a reliable way to name the virus.

Even the apparent absence of a classic symptom does not settle the question. Not everyone with flu has a fever, and COVID-19 can range from no symptoms to severe illness. A mild illness or a lack of fever therefore should not be treated as proof that flu or COVID-19 is absent, particularly if you are at higher risk or symptoms are worsening.

How the usual patterns compare

These are tendencies, not a checklist. People can have unusual symptom combinations, and illness severity varies. In particular, not having a fever does not rule out flu.

Feature Common cold Flu COVID-19
Typical onset Usually gradual. Often abrupt. Timing alone cannot reliably distinguish it from flu.
Typical severity Often milder. Often more intense, with aches, chills, and fatigue common. Ranges from no symptoms to severe illness.
Symptoms that may occur Runny or stuffy nose, cough, sore throat. Fever or chills, cough, aches, fatigue, sore throat, congestion, or headache; some people have gastrointestinal symptoms. Fever or chills, cough, breathing difficulty, fatigue, sore throat, congestion, aches, headache, or gastrointestinal symptoms; taste or smell changes occur more often than with flu.
Can symptoms confirm it? No. Other respiratory viruses can also cause cold-like symptoms. No. Testing is needed to confirm. No. Testing is needed to confirm.

The table compares common patterns, not diagnostic rules. A person with flu may have congestion or a sore throat; a person with a cold may feel tired or cough. COVID-19 can look like either. There is no symptom combination in this comparison that reliably separates flu from COVID-19.

What an abrupt start may suggest

Flu often begins suddenly, with symptoms such as pronounced aches, chills, fever, and fatigue. That contrast with a gradual, milder start can make flu seem more likely, but it does not distinguish flu from COVID-19 with confidence. Also, not everyone with flu develops a fever.

When people describe flu as coming on “all at once,” they are describing a tendency rather than a required pattern. Some people’s symptoms may not match the familiar picture, and symptoms overlap with COVID-19. An abrupt onset can be a reason to consider testing or to contact a clinician, especially for someone at higher risk, but it cannot confirm flu.

What a gradual, nose-heavy illness may suggest

A slow onset with milder symptoms, sneezing, or nasal congestion is more typical of a cold. But congestion and sore throat also occur with flu and COVID-19, and other viruses can produce cold-like symptoms. Treat this pattern as a clue, not proof.

Likewise, a runny nose does not establish that an illness is an ordinary cold. The practical difference is that a likely cold often improves with time and comfort measures, while identifying flu or COVID-19 may matter for time-sensitive treatment, particularly for people at increased risk. If that distinction could change what you do next, rely on testing and timely medical advice rather than the symptom pattern alone.

When to test and how to interpret the result

At-home multiplex tests that check for COVID-19 and flu are available, according to the CDC. A multiplex test can detect more than one virus in the same test. Testing can help inform treatment and prevention decisions, but the result needs to be considered alongside symptoms and personal risk.

If you have symptoms and want to know whether flu or COVID-19 is the cause, a combination test is one available way to check for both. A test result can help guide decisions, but it is not a substitute for medical care when symptoms are severe or when you may need prompt treatment. No specific test brand or price is necessary to understand the key point: available home tests differ in what they detect and how reliably they detect it.

Antigen and molecular tests are not equally sensitive

Rapid antigen tests are generally less sensitive than molecular tests, including nucleic acid amplification tests (NAATs). An antigen test can return a negative result even when a person is infected, particularly when the virus is circulating widely. So a negative rapid antigen result is not proof that you do not have flu or COVID-19.

A negative result answers only what that particular test detected in that particular sample. It does not erase symptoms, prove that another respiratory virus is responsible, or guarantee that infection is absent. This is especially important when the result conflicts with how you feel, when you have a reason to suspect infection, or when knowing the cause could affect treatment decisions.

If a rapid result is negative but you still have symptoms or a reason to suspect infection, ask a healthcare provider whether molecular testing or repeat testing is appropriate. The right next step depends on your situation; do not use one negative rapid result to dismiss serious or worsening symptoms.

In practical terms, do not assume that every negative home result calls for the same response. A clinician can advise whether a more sensitive molecular test or repeat testing makes sense for your circumstances. If you are in a higher-risk group, make that contact promptly rather than waiting for symptoms to become severe, because treatment windows can be short.

A positive test does not rule out a second infection

Flu and COVID-19 can occur at the same time, although the CDC characterizes simultaneous infection as uncommon. A positive test for one virus does not necessarily exclude another, especially when multiple viruses are circulating. If symptoms or risk factors make the distinction important for treatment, discuss the result and next steps with a clinician.

  • Note: Before using the health care thermometer, carefully read the product manual. Start measuring when “Lo℉” shows up. To ensure the accuracy of the measurement, avoid touching the probe with your hands or other hot objects prior to taking the temperature.

This does not mean that a second infection is likely whenever one test is positive. It means that one positive result is not an absolute guarantee that no other infection is present. Symptoms that are severe, worsening, or concerning still deserve attention, whether or not a test has already identified one virus.

Who should contact a healthcare provider quickly

People at higher risk of severe illness should contact a healthcare provider promptly if respiratory symptoms develop, even when symptoms are mild. CDC materials identify older adults, young children, pregnant people, and people with certain chronic conditions or weakened immune systems among groups at increased risk. A provider can assess individual circumstances and whether treatment is appropriate; not everyone in these groups will qualify for every treatment.

These categories are reasons to seek timely individualized advice, not a promise that a particular person will receive a prescription or that treatment will be appropriate. The exact decision depends on the person’s circumstances and the illness being considered. If you are unsure whether a condition or circumstance makes you higher risk, ask a healthcare provider rather than ruling yourself out.

Timing matters. The CDC’s Common Cold: Treatment guidance, dated March 11, 2026, says COVID-19 treatment should begin within 5–7 days after symptoms start and flu treatment within 2 days. These are treatment-start windows, not guarantees of eligibility or benefit. Contact a clinician as soon as possible rather than waiting to see whether symptoms become severe.

The 5–7 day window applies to starting COVID-19 treatment; the 2 day window applies to starting flu treatment. They are not instructions to wait until a particular day, and they do not mean that everyone with symptoms should take an antiviral. A prompt conversation gives a clinician a chance to assess whether testing or treatment is appropriate while timing may still matter. If you are at higher risk, do not postpone that conversation simply because your symptoms seem manageable.

When symptoms need urgent medical attention

Seek emergency care for difficulty breathing, persistent chest pain or pressure, confusion, inability to wake or stay awake, or seizures. Severe or concerning symptoms warrant medical attention even if they are not on this list; CDC warning signs are not exhaustive.

These warning signs are different from the question of which virus caused the illness. Do not wait to identify the virus with a home test before seeking emergency help for a serious symptom. A test result cannot make difficulty breathing, persistent chest pressure, confusion, or inability to wake safe to ignore.

Warning signs in children

Children can show warning signs that differ from adults. Seek urgent help for fast or difficult breathing, bluish lips or face, ribs pulling in with breaths, dehydration, or lack of alertness. A fever in an infant younger than 12 weeks also warrants prompt medical attention.

For a child, watch how breathing looks as well as what the child says. Fast or difficult breathing, ribs pulling in with breaths, or bluish lips or face are specific warning signs. Dehydration and lack of alertness also call for urgent attention. The list is not exhaustive: if a child’s condition seems severe or concerning, seek care even if the exact sign is not named here.

What to do if it seems like a cold

Most colds improve on their own with time. Rest, fluids, saline, and a clean humidifier or cool-mist vaporizer can help with comfort; they do not identify the virus or cure the infection. If symptoms worsen, persist in a concerning way, or you are at higher risk, seek medical advice rather than assuming it is only a cold.

These measures are for symptom comfort while the illness runs its course. They do not tell you whether symptoms are caused by a cold virus, flu, COVID-19, or another respiratory virus. Use a clean humidifier or cool-mist vaporizer if choosing that option, and do not mistake temporary comfort for confirmation that an illness is harmless.

Antibiotics do not treat viruses, so they are not a treatment for an ordinary cold. A clinician may prescribe them if they identify a bacterial infection, but cold symptoms alone are not a reason to take antibiotics.

In other words, the fact that symptoms feel “cold-like” does not mean antibiotics will help. The common cold and the other illnesses discussed here are viral illnesses, and antibiotics do not work against viruses. If a clinician identifies a bacterial infection, that is a different situation; do not use leftover antibiotics or seek them as a way to treat an ordinary viral cold.

How to reduce the chance of spreading respiratory illness

These steps can lower transmission risk, but they do not reveal which illness caused your symptoms. CDC respiratory illness guidance, updated September 10, 2026, includes cleaner indoor air, hygiene, staying up to date with recommended immunizations, and precautions when sick. Masking can add another layer of protection. Choose precautions suited to your circumstances and current public-health guidance.

Cleaner indoor air can include purifying indoor air. Hygiene and precautions when sick are also part of the CDC guidance, and keeping recommended immunizations up to date is another prevention strategy. A mask can provide an additional layer of protection. These measures are about reducing the chance of spread; they do not replace testing when identifying flu or COVID-19 matters, and they do not change the need for urgent care when warning signs appear.

A simple way to decide what to do next

  1. Notice the pattern, but do not diagnose yourself from it. A gradual, milder, nose-heavy illness is more typical of a cold; abrupt, intense aches, chills, and fatigue are more typical of flu. Neither pattern proves the cause, and flu and COVID-19 cannot reliably be distinguished by symptoms alone.
  2. Consider testing if knowing the cause matters. At-home multiplex tests for COVID-19 and flu are available. Understand whether the test is antigen or molecular: antigen tests are generally less sensitive, so a negative rapid antigen result does not necessarily rule out infection.
  3. Contact a provider promptly if you may be at higher risk. Older adults, young children, pregnant people, and people with certain chronic conditions or weakened immune systems are among groups identified by CDC materials. Treatment decisions are individual, and timing can matter.
  4. Get emergency help for warning signs. Difficulty breathing, persistent chest pain or pressure, confusion, inability to wake, or seizures need urgent attention. Children may also show fast or difficult breathing, bluish lips or face, ribs pulling in, dehydration, or lack of alertness.
  5. Use supportive care for a likely cold and avoid antibiotics for viral illness. Rest, fluids, saline, and a clean humidifier can help with comfort. Seek advice if symptoms worsen or are concerning.

Frequently Asked Questions

Can you tell the difference between flu and COVID-19 by symptoms?

No. The CDC says their signs and symptoms overlap too much for symptoms alone to reliably distinguish them. Testing is needed to confirm flu or COVID-19.

Do I need a test if I have cold symptoms?

Not every cold-like illness requires the same next step, but cold symptoms can also come from flu, COVID-19, or other respiratory viruses. If identifying flu or COVID-19 could affect treatment or prevention decisions, testing can help. Contact a healthcare provider promptly if you are at higher risk.

Does a negative rapid antigen test mean I do not have flu or COVID-19?

No. Rapid antigen tests are generally less sensitive than molecular tests and can be negative when someone is infected. Ask a healthcare provider whether repeat or molecular testing is appropriate, especially if you still have symptoms or are at higher risk.

Can I have flu and COVID-19 at the same time?

Yes, simultaneous infection is possible, although the CDC describes it as uncommon. A positive result for one virus does not necessarily rule out another infection.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When should I contact a healthcare provider about flu or COVID-19 symptoms?

Contact a provider promptly if you are at higher risk of severe illness, even if symptoms are mild. CDC guidance gives a 5–7 day window from symptom onset to start COVID-19 treatment and a 2 day window for flu treatment; a clinician can assess whether treatment is appropriate for you.

When are respiratory symptoms an emergency?

Seek emergency care for difficulty breathing, persistent chest pain or pressure, confusion, inability to wake or stay awake, or seizures. Children may also show fast or difficult breathing, bluish lips or face, ribs pulling in, dehydration, or lack of alertness. A fever in an infant younger than 12 weeks warrants prompt medical attention.

Do antibiotics help a common cold?

No. Antibiotics do not work against viruses and are not a treatment for an ordinary cold. A clinician may prescribe them if a bacterial infection is identified.

The Bottom Line

Cold, flu, and COVID-19 symptoms overlap, so onset and severity can offer clues but cannot provide a reliable diagnosis. Testing can help identify flu or COVID-19; a negative rapid antigen result does not always rule them out. If you are at higher risk, contact a healthcare provider promptly because treatment timing can matter. Seek emergency care for serious warning signs, and use supportive care—not antibiotics—for an ordinary viral cold.
