How to Switch to a Vertical Gaming Mouse Without Straining Your Wrist

A vertical mouse can change forearm posture, but comfort and gaming control depend on fit and setup. Here’s how to make the switch and evaluate it safely.

Switch gradually and judge the mouse by both wrist posture and control in the games you play. A vertical mouse can reduce forearm pronation, but that posture change does not guarantee greater comfort or better pointing performance. Keep the mouse close, your wrist straight and neutral, and choose a shape that fits your hand and your tasks.

What a vertical mouse can—and can’t—do

A vertical mouse places the hand in a more upright position than a conventional flat mouse, changing how much the forearm rotates inward (pronates). That may help you use a less-pronated posture, but it does not establish that the device will prevent injury, treat pain, or improve gaming.

In a 2015 study of 12 participants performing standardized pointing tasks, the tested fully vertical mouse reduced forearm pronation but produced the poorest pointing performance of the tested designs. The authors concluded, “Increasing mouse height and angling the mouse topcase can improve wrist posture without negatively affecting performance.” Those findings apply to the devices and tasks tested—not every vertical mouse or gaming use. Set up your desk to keep your wrist neutral

Changing the mouse alone cannot correct a setup that makes you reach or bend your wrist. OSHA advises: “Position the pointer/mouse to allow you to maintain a straight, neutral wrist posture.” OSHA’s pointer and mouse guidance and NIOSH workstation guidance support keeping the wrist straight and positioning the mouse within easy reach.

Choose a mouse for your hand and the games you play

“Vertical” describes a shape, not a guarantee of fit or gaming suitability. OSHA recommends choosing an input device according to the user’s ability, task requirements, physical limitations, and preference; vertical mice are one available type, not a universal recommendation. See OSHA’s input-device guidance.

  • Placement: Can it sit close enough to use without reaching?

Posture and pointing performance may not improve together: the 2015 study found less pronation with its fully vertical design, but weaker pointing performance on its standardized tasks. It did not test games, so it cannot determine which design works best for gaming. Study details.

Make the change as a personal trial

  1. Start with your setup. Put the mouse close to you and adjust your position so your wrist can stay straight and neutral.
  2. Try it in everyday pointing. Notice whether you can move and click with control while maintaining that posture.
  3. Test your own games. Pay attention to the control demands of the games you actually play; standardized desktop pointing results do not establish gaming performance.
  4. Compare fit and comfort. If the grip feels awkward or control is poor, compare a different shape rather than assuming every vertical mouse will feel the same.

Moving from a standard mouse can be difficult, according to the Ontario-based Occupational Health Clinics for Ontario Workers. However, the available sources do not establish a universal number of days or hours needed to adapt. Treat the adjustment period as individual, not as a fixed schedule. OHCOW’s guidance.

When a vertical mouse may not be the right choice

If a device makes it harder to control the pointer, does not fit your hand, or prevents a neutral wrist posture in your setup, it may not suit your needs—even if its shape is marketed as ergonomic. The evidence summarized here addresses posture and pointing performance, not clinical treatment outcomes. Persistent pain or other symptoms warrant advice from a qualified health professional rather than relying on a mouse change as treatment.
