Manage ordinary app alerts in the iPhone’s Apple Watch app, and configure health features separately in Heart, Vitals, Sleep, and Health. The exact options depend on your watch model, software version, age, and country or region; health notifications can flag patterns but do not diagnose a condition.
Choose where app notifications appear
Use the Apple Watch app on your paired iPhone to decide which app alerts reach the watch and how they are delivered.

- Open the Apple Watch app on your iPhone.
- Tap My Watch > Notifications.
- Select an app. If available, choose Mirror my iPhone to use the iPhone’s notification settings, or select Custom to configure alerts for that app.
- With custom settings, choose Allow Notifications, Send to Notification Center, or Notifications Off. Some apps also offer grouping or other alert-specific options.
For lock-screen presentation, use the watch itself: open Settings > Notifications. Depending on the options available, you can control whether a summary appears while the watch is locked, whether a tap is required to show the full notification, and whether alerts appear when your wrist is down. See Apple’s notifications guidance.

Apple’s rule is: “Notifications will appear on your Apple Watch or iPhone, but not both.” If your iPhone is unlocked, a notification generally appears there. If the iPhone is locked or asleep, it generally appears on the watch—unless the watch is locked. Apple says alerts go to the iPhone when the devices are disconnected, or when the watch is locked or Do Not Disturb is on. Focus settings and connection state can therefore change where an alert appears. Third-party app notifications also require the paired iPhone to be connected to a network. These routing rules are described in Apple’s Apple Watch notification guide.

Set high- and low-heart-rate alerts
On the paired iPhone, open the Apple Watch app > My Watch > Heart. Tap High Heart Rate or Low Heart Rate and choose a threshold. Apple lists these notifications for Apple Watch Series 1 or later and people age 13 or older. They alert you when the measured heart rate meets the selected threshold; they are not a diagnosis. Check Apple’s heart-health notification guidance for current details.
Set up irregular-rhythm notifications
This feature has a separate setup path from high- and low-heart-rate alerts. On your iPhone, open Health > Browse > Heart > Irregular Rhythm Notifications > Set Up, then follow the eligibility prompts. Availability depends on country or region and compatible, up-to-date iOS and watchOS versions. Apple says the feature is not intended for people under 22 or people already diagnosed with atrial fibrillation (AFib); AF History must also be off. It can notify you of an irregular rhythm that may be suggestive of AF, but it does not diagnose AFib. Consult Apple’s current eligibility and setup information because availability and requirements can vary.

Turn on overnight Vitals notifications
On Apple Watch, open Settings > Vitals and turn on Notifications. The Vitals app can show overnight heart rate, respiratory rate, wrist temperature, blood oxygen, and sleep duration. It establishes typical ranges during sleep and can notify you when multiple measurements fall outside those ranges. Apple says notifications are on by default when at least two metrics are outside their typical ranges.

Add Medical ID information
On your paired iPhone, set up Medical ID in the Health app with the health details and emergency contacts you want available. The information can also be viewed on Apple Watch: hold the side button until the sliders appear, then slide Medical ID. Apple says sharing Medical ID information with emergency services when calling or texting 911 or using Emergency SOS is available in the United States and Canada; do not assume the same sharing behavior in other regions. See Apple’s Medical ID guidance.

- Turn on Fitness Tracking.
To review what third-party apps can read or write, open Health > profile > Apps and allow the categories you want to share. An app may also require permission in its own settings. Health can combine measurements from more than one source; within a category, open Data Sources & Access to view and reorder sources. Apple documents these controls in its Health data management guidance.
