Reviewers should assess the complete change against secure-coding standards and the fix’s intended security behavior, then require regression tests plus independent negative or adversarial tests that the generating AI did not create. Preserve the approver’s identity, tool and model details, review notes, test results, and issue-triage records.
This approach follows NIST DevSecOps guidance, NIST’s Secure Software Development Framework (SSDF), the July 2024 SP 800-218A AI-focused profile, and OWASP’s Secure Coding with AI guidance. They do not define one universal test suite or risk threshold for every patch, so your policy must scale gates to the patch’s threat surface and impact.

Start with a non-negotiable merge rule
An AI system may draft code, tests, explanations, or remediation ideas, but it must never approve its own output. Every security patch needs a person who accepts responsibility for correctness, security, and future maintenance.
Minimum approval requirements
- Assign one named developer as the change owner.
- Require an identified human reviewer with suitable security and codebase knowledge.
- Require explicit approval after the reviewer has examined the complete diff, related configuration, and test changes.
- Block merging when approval is missing, withdrawn, or supplied only by an automated agent.
- Keep an audit trail showing who approved the change, when, and which AI tool and model version produced or modified it.
NIST’s DevSecOps reference material calls for human monitoring and validation of AI output within established peer-review, security-validation, automated-testing, and approval workflows. OWASP states: “Every AI-assisted change should be reviewed, approved, and attributable to a developer who is responsible for its security and maintainability.”

Separate ownership from mechanical checks
| Role | Responsibility | What cannot be delegated |
|---|---|---|
| Change owner | Explains the vulnerability, intended fix, affected assets, and maintenance implications. | Accepting responsibility for the patch’s behavior and risk. |
| Security-capable reviewer | Challenges the threat model, implementation, dependencies, and test evidence. | Determining whether the fix actually closes the security condition. |
| CI system | Runs repeatable builds, regression tests, static checks, and policy gates. | Providing human judgment or approval. |
| AI tool | Suggests implementation and possibly test cases under documented instructions. | Approving, merging, or being treated as independent assurance. |
Classify the patch before choosing gates
Do not use a single test count or reviewer rule for every change. First record what the patch can affect. NIST SSDF separates code review and analysis (PW.7) from executable-code testing (PW.8); a strong policy performs both and records the results.
Risk factors to record
- Security impact: authentication, authorization, cryptography, secrets, isolation, input handling, or privacy controls deserve heightened scrutiny.
- Exposure: internet-facing services, shared libraries, privileged components, update mechanisms, and code processing untrusted data increase the blast radius.
- Change breadth: database migrations, configuration changes, dependency updates, generated files, and changes crossing service boundaries can invalidate a narrow unit test.
- Exploitability and urgency: an actively exploited issue may need an expedited path, but urgency does not remove human approval or evidence requirements.
- Observability and rollback: patches that are difficult to monitor or reverse require stronger pre-merge evidence.
- AI involvement: record whether the model wrote new code, rewrote existing code, modified tests, selected dependencies, or generated configuration.
Example internal risk bands
The following bands are an implementation aid, not a universal standard. Set your own thresholds and document why a patch belongs in a band.
| Band | Typical characteristics | Suggested human gate | Suggested evidence |
|---|---|---|---|
| Contained | Low-privilege component, narrow logic change, no external input or security boundary. | One qualified reviewer independent of the author; explicit approval. | Diff review, focused regression tests, and at least one negative case where a failure is plausible. |
| Significant | Authentication, authorization, sensitive data, network parsing, shared library, or configuration change. | Security-capable reviewer plus the owner; consider a second reviewer for unfamiliar code. | Regression suite, integration tests for the boundary, negative/adversarial cases, and recorded threat assumptions. |
| Critical | Privileged infrastructure, cryptographic or key-management logic, broad dependency impact, or a patch addressing a high-consequence exploit. | Two-person review or a designated security approver, according to your governance model. | All applicable automated tests, independent security testing, abuse-case analysis, rollback plan, and documented triage of every finding. |
Neither NIST nor OWASP publishes a universal matrix that maps every patch to a fixed reviewer count or test list. Treat these bands as a decision record, then tighten them when your threat model, contractual obligations, or incident history warrants it.
Define what the human review must examine
1. Verify the security intent
The reviewer should restate the vulnerability and the required post-fix property in concrete terms. For example, “a user without the permission must receive the same denial whether the resource identifier is valid or invalid” is testable; “fix the access bug” is not. Compare the implementation with the issue description, exploit trace, advisory, or acceptance criteria. If the intended behavior is unclear, stop the merge and resolve it.
2. Inspect the complete diff
Review more than the central code hunk. Include API contracts, feature flags, infrastructure definitions, serialization formats, database migrations, dependency manifests, generated artifacts, and documentation that changes operational behavior. AI output can introduce a security-relevant default in a file that looks unrelated to the reported vulnerability.
3. Check secure-coding properties
- Authentication and authorization decisions occur on the server-side trust boundary.
- Input is validated for type, length, encoding, and context before use.
- Output encoding matches its destination, such as HTML, SQL, shell, or structured logs.
- Secrets are not hard-coded, logged, or copied into test fixtures that can escape the test environment.
- Cryptographic algorithms, modes, key sizes, randomness, and error handling meet the organization’s approved standard.
- Errors fail safely without disclosing credentials, tokens, stack traces, or sensitive identifiers.
- Concurrency, caching, retries, and partial failures cannot bypass the security check.
- New dependencies are pinned, sourced from an approved registry, and reviewed for their transitive impact.
4. Challenge the generated explanation
An AI-generated rationale is an input to review, not proof. Ask which assumptions it made about identity, tenancy, deployment mode, data shape, and failure behavior. Trace critical claims to code, configuration, or a reproducible test. A confident explanation that does not match the runtime path is a review finding.

5. Review for regressions and bypasses
Check the old vulnerable path, the corrected path, and adjacent paths. Look for alternate endpoints, background jobs, command-line interfaces, caches, batch operations, and administrative routes that apply different checks. Verify that the patch does not merely move the vulnerability, disable a control, or make the test pass by changing the expected behavior.

Tests should find vulnerabilities that code review, static analysis, and earlier tests may have missed. NIST SSDF PW.8 calls for testing executable code to identify vulnerabilities and verify security requirements. SP 800-218A lists unit, integration, penetration, red-team, use-case, and adversarial testing in its AI-model context; apply those categories to a patch only when they fit the component and threat surface.
Baseline tests for every security patch
- A reproducible test that fails on the vulnerable behavior or demonstrates the security requirement.
- A regression test that passes with the patch and remains in the suite.
- Existing unit and integration tests for affected modules and interfaces.
- Build, type, lint, and static-analysis checks used by the project.
- At least one negative case designed to exercise denial, malformed input, missing credentials, expired state, or another failure condition relevant to the fix.
- A record of test environment, commit or artifact identifier, commands or pipeline jobs, and results.
“At least one” is a floor for a contained change, not a claim that one negative test is sufficient for every vulnerability. Choose additional cases from the attack paths and assumptions documented by the owner and reviewer.
Match test types to the threat surface
| Patch characteristic | Useful test focus | What the test should prove |
|---|---|---|
| Pure validation or parsing logic | Unit tests, malformed and boundary inputs, property or fuzz testing where practical. | Unexpected encodings, lengths, types, and parser states cannot reach unsafe behavior. |
| Authorization or tenancy | Unit tests plus integration tests using allowed, denied, cross-tenant, anonymous, and expired identities. | The decision is enforced at every relevant endpoint and cannot be bypassed by identifier changes or alternate flows. |
| Network or service boundary | Integration tests, failure injection, replay or downgrade cases, and targeted adversarial requests. | Trust boundaries, timeouts, retries, and malformed peer responses preserve the security property. |
| Cryptography or secrets | Known-answer or invariant tests, key-rotation scenarios, negative configuration tests, and specialist review. | Approved primitives and safe failure behavior are used without exposing key material. |
| Infrastructure or deployment configuration | Configuration validation, policy-as-code checks, staging deployment, and rollback verification. | The secure setting is effective in the deployed environment and does not get overwritten by defaults. |
| High-consequence or broad-impact patch | Independent penetration, red-team, abuse-case, or targeted adversarial testing as justified by risk. | An evaluator who did not generate the patch can find bypasses under realistic attack assumptions. |
Make at least one test independent of the generating agent
A model can generate a plausible implementation and a matching test that encodes the same mistaken assumption. OWASP warns: “A passing test suite generated by the same agent that produced the code provides no independent assurance.” Require a reviewer, separate test author, security tester, or independently specified test oracle to create or substantially revise negative and adversarial cases.
Treat AI-authored test changes as security-sensitive
Test files are part of the control system. Review them with the same care as production code, especially when the AI was asked to “make the tests pass.”
Red flags to inspect explicitly
- Deleted tests, skipped cases, disabled fixtures, or narrowed parameter sets.
- Assertions changed from exact security properties to broad success checks, non-null checks, or snapshot updates.
- Real dependencies replaced with mocks that no longer exercise authentication, authorization, parsing, storage, or network behavior.
- Expected errors converted into accepted results.
- Test data changed so that the malicious, unauthorized, malformed, or expired case is no longer represented.
- Tests that simply assert the generated implementation’s output rather than the intended security requirement.
- Reduced assertion counts, lower coverage in protected paths, or altered timeouts that hide failures.
Automate integrity checks
Configure CI to flag test-file deletions, skipped tests, assertion-count reductions, coverage drops in security-sensitive modules, and unexpected mock substitutions. A flag should trigger human review rather than automatically reject every legitimate refactor. Require the owner to explain approved exceptions in the change record.

Use a controlled workflow from prompt to merge
- Open or link the security issue. Record the affected component, threat, impact, intended security behavior, and any known exploit or regression.
- Declare AI involvement. Identify the tool, model or model family, version when available, and the activities it performed: code generation, editing, test generation, dependency selection, or configuration changes.
- Assign the owner. Name the developer accountable for correctness, security, maintenance, and follow-up.
- Generate in a bounded workspace. Provide the model with the relevant interfaces and constraints while withholding secrets and unnecessary production data. Keep prompts or session identifiers when your governance policy requires reproducibility.
- Build the smallest explanatory patch. Ask for a description of the vulnerability, security invariant, affected paths, assumptions, and proposed tests. Do not accept a broad rewrite merely because it is easier for the tool.
- Run automated checks. Execute build, static analysis, unit, integration, regression, and policy checks. Capture immutable results tied to the commit or artifact.
- Perform human diff review. The owner and reviewer inspect production code, configuration, dependencies, and tests against secure-coding standards and the stated security intent.
- Add independent negative or adversarial tests. Select cases from residual risk and have someone other than the producing agent specify or author them when feasible.
- Triage findings. Record each failure, vulnerability, or uncertainty in the normal issue tracker, assign severity and an owner, and document remediation or an approved exception.
- Obtain explicit approval. The qualified human reviewer confirms that the evidence supports the security behavior and that no unresolved blocker remains.
- Merge only through the protected path. Enforce required approvals and passing checks in the repository, then retain the complete record.
- Monitor after release. Watch security telemetry, error rates, and reports for regressions. If the patch is rolled back or amended, link the event to the original decision record.
Keep an auditable evidence package
NIST guidance calls for discovered issues and recommended remediations to be recorded and triaged in the development workflow or issue tracker. Your evidence should let another engineer reconstruct why the patch was trusted.
| Evidence item | Minimum content |
|---|---|
| Identity and accountability | Owner, reviewer or approvers, timestamps, team, and any required separation of duties. |
| AI provenance | Tool name, model or model family, version when available, relevant configuration, and the type of assistance provided. |
| Intent | Issue identifier, vulnerability description, security invariant, affected assets, assumptions, and risk band. |
| Review record | Standards or checklists used, important questions raised, requested changes, and final approval statement. |
| Test evidence | Commit or artifact identifier, environment, commands or pipeline jobs, logs or links retained by your system, and results for regression and security-focused tests. |
| Findings and triage | Failures, vulnerabilities, severity, disposition, remediation owner, due date, and approved exceptions with an expiry or review date. |
| Release and recovery | Deployment scope, monitoring plan, rollback method, and any post-release observations. |
Store evidence where access controls, retention, and tamper resistance match the sensitivity of the project. Do not place secrets, customer data, or proprietary prompts in a ticket merely to make the record appear complete.
Design CI and pull-request gates that enforce the policy
Required gates
- A pull request cannot merge without a named owner and a human approval.
- The approval must come from a reviewer who is not the generating agent and meets the project’s competence rule.
- Required automated checks must run on the exact commit being merged.
- Security patches must include a linked issue and a stated security invariant.
- Test deletions, skipped tests, assertion reductions, and suspicious mock changes must create a review flag.
- Negative or adversarial tests must be present when the recorded risk assessment says they are applicable.
- Any failed check, unresolved review comment, or untriaged security finding blocks merge.
Make exceptions visible
Emergency changes may use an expedited reviewer path, but the exception should name the approving authority, explain which normal gate was bypassed, state compensating controls, and set a deadline for retrospective testing or review. An emergency label is not a permanent exemption and should not erase AI provenance.
Common failure modes and corrective actions
“The vulnerability scanner is clean, so approval is unnecessary.”
Static and dependency tools can miss logic flaws, deployment-specific behavior, and incorrect security requirements. Keep the human approval gate and use scanner results as evidence within the review.
“The model wrote a regression test, so the bug is proven fixed.”
Verify that the test would fail on the vulnerable revision and that its oracle expresses the intended behavior. Add an independently designed negative or adversarial case.
“The patch is only a few lines.”
Line count does not measure blast radius. A one-line authorization or configuration change can affect every request. Classify the boundary and impact, not just the diff size.

“Two approvals guarantee independence.”
Two people can repeat the same assumption, especially when both rely on the model’s explanation. Require at least one reviewer to challenge the threat model and inspect an independently specified test path.
“The issue was fixed, so triage can wait.”
Record and triage findings while the context is fresh. Deferred vulnerabilities, flaky tests, and accepted exceptions need owners and dates or they disappear from the operational workflow.
Policy language you can adapt
AI-assisted security changes may merge only when all of the following are true:
1. A named developer accepts responsibility for correctness, security, and maintenance.
2. A qualified human reviewer has examined the complete diff, related configuration, dependencies, and tests.
3. The reviewer has explicitly approved the change; AI output cannot approve or bypass review.
4. Automated regression and security-focused tests selected for the patch's residual risk pass.
5. AI-authored or AI-modified tests have been checked for deleted cases, weakened assertions, substituted mocks, and tests that encode generated behavior instead of intended behavior.
6. Owner, approver, AI tool/model details, review notes, test evidence, and issue triage are retained.
7. Exceptions identify the authority, compensating controls, and deadline for follow-up.
Adapt terminology to your repository and regulatory environment. The controls above are an implementation template derived from NIST and OWASP guidance, not a claim that every organization must use identical fields or approval counts.
Build reviewer capability over time
Human review works only when reviewers understand both the code and the security property. Train reviewers to read threat models, identify trust boundaries, recognize insecure defaults, and challenge test oracles. Maintain a list of components that require specialist knowledge, such as cryptographic services or privileged deployment code, and route those changes accordingly.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteUse incidents, escaped vulnerabilities, flaky security tests, and recurring review findings to refine your risk bands and test catalog. Track process measures such as time to triage, proportion of patches with independent tests, and frequency of test-integrity flags, but do not turn a single metric into proof that patches are secure. The objective is defensible evidence and sound decisions, not a cosmetic approval rate.
Best Value
What this policy can and cannot promise
These controls reduce the chance that an AI-generated security patch merges with an unnoticed flaw, weakened test, or untraceable decision. They cannot guarantee defect-free code, and the cited guidance does not provide a published vulnerability rate or a universal pass/fail threshold for AI-generated patches. Your organization remains responsible for selecting tests and reviewer competence that fit its systems, users, and threat model.
Frequently Asked Questions
Can an AI tool approve or merge its own security patch?
No. Require explicit approval from a named, qualified human reviewer. Automation may enforce checks, but it cannot provide accountability or independent judgment.
How many tests should every AI-generated security patch include?
There is no universal number. Require a regression test for the vulnerable behavior, applicable existing tests, and security-focused negative or adversarial cases selected from the patch’s residual risk.
Who should write the independent security test?
A reviewer, security tester, or separate engineer should specify or substantially author at least one negative or adversarial case when feasible, rather than relying solely on tests generated by the same agent.
What must be recorded about AI use?
Retain the tool and model identity and version when available, what work the AI performed, the owner and approver, review notes, test evidence, and issue-triage decisions.
Should emergency patches skip human review?
Use an expedited human path if necessary, but document the exception, compensating controls, and deadline for retrospective testing or review. Do not allow the AI to self-approve.
