If an AI assistant gives a false tip, unsafe instruction, offensive reply, or potentially unlawful content, preserve the exact exchange first, then use the service’s response-level feedback control. Select the closest accuracy, safety, or legal category, explain the specific problem, and attach only the context needed to verify it. For a legal concern, use the provider’s separate legal or content-reporting route rather than assuming a thumbs-down is a formal notice.
Do not wait for a platform review when someone faces immediate danger. Contact appropriate local emergency, medical, legal, or other professional help, and independently verify any consequential medical, legal, financial, safety, or civic information.

What to do before you report anything
A useful report lets a provider identify one response, understand why it is wrong or harmful, and assess the surrounding context without receiving unnecessary private data. Work through these steps in order.

If somebody may be injured, threatened, defrauded, denied urgent care, or exposed to another time-critical danger, act on that risk first. Call local emergency services where appropriate, contact a qualified clinician or lawyer, or use the relevant crisis and safeguarding service. A provider report has no guaranteed response time and is not an emergency channel.

2. Preserve the original exchange
Before editing, deleting, or continuing the chat, save:
- The exact prompt or message that produced the problem.
- The complete response, including warnings, citations, links, images, and surrounding turns needed to understand it.
- The service name, app or website, and model or version if the interface displays one.
- The date and approximate time, including the time zone when a deadline or ongoing impact is involved.
- A screenshot or conversation/share link, if available and safe to create.
Copy text accurately. Do not paraphrase the statement you are reporting when an exact transcript is available. If the answer cites a source, preserve that URL so the reviewer can see whether the citation is missing, irrelevant, or misrepresented.
3. Remove unrelated sensitive material
Keep the context that explains the failure, but redact unrelated names, addresses, account numbers, health details, private documents, confidential business material, and information about bystanders. Do not alter the reported sentence itself in a way that changes its meaning. If the service automatically includes the whole conversation, review the privacy notice and delete or move unrelated sensitive chats before submitting when the product allows it.

Most services have two different concepts: feedback about the quality or safety of a response, and a formal channel for a legal or terms-of-service concern. The response control is normally the fastest way to flag an inaccurate or harmful answer. A legal report may require a separate form and additional information.

Do not claim that an output was intentional. Describe the observable text, instruction, or omission and its likely consequence. Providers may use different labels, and the same button can look different on web, iOS, Android, work accounts, or regional versions.
Reporting a ChatGPT or OpenAI response
For a problematic response in ChatGPT, use the control attached to the message. OpenAI’s Help Center instructs users on web and iOS/Android to tap the thumbs-down control, select “Safety or Legal concern,” and follow the prompts. Include the exact error or harm and a concise explanation of why it matters.
- Open the conversation containing the response.
- Locate the message that is inaccurate, unsafe, offensive, or otherwise concerning.
- Tap or click the thumbs-down control beneath that message.
- Choose “Safety or Legal concern.”
- Describe the specific claim or instruction, add a reliable correction when useful, and submit only the context needed for review.
For conversations, shared links, responses, custom GPTs, or forum posts that may violate OpenAI’s Terms of Use or applicable law, use OpenAI’s content-reporting webform as well as, or instead of, message feedback. A thumbs-down is not automatically a formal legal notice.

OpenAI says reported domains and other content “may be reviewed by OpenAI’s Model Quality team.” The word may matters: the statement does not promise that every report will be reviewed, that a response will be corrected or removed, or that you will receive a reply or a timetable.
Reporting a Gemini Apps response
On a computer, Google’s Gemini Apps instructions say to click “Good response” or “Bad response” below the answer. Choose the reason, add details, and decide whether to include uploaded files or images.
- Find the Gemini answer that contains the false tip or harmful material.
- Click “Bad response.”
- Select the closest reason, such as inaccurate, unsafe, or offensive content.
- Explain the exact sentence or instruction that is wrong and provide a trustworthy correction when available.
- Review the inclusion choices for files or images, then submit.
Google warns that submitting feedback adds the associated conversation, including prompts and Gemini responses, and any included uploaded content or personal content Gemini obtained from Connected Apps, subject to its privacy policy. Treat the review step as part of reporting, not as an optional detail. Feedback requires sign-in, and some work or school accounts cannot add extra feedback or attachments.
For a legal issue with a Gemini web response, choose More, then “Report legal issue.” Use that route when the concern is a legal or rights violation, not merely because an answer is inaccurate. The relevant instructions are in Google’s Gemini Apps Help: Send feedback or report a problem.

Google’s separate guidance states, “Gemini can hallucinate and present inaccurate information as factual.” It advises double-checking responses about people and not relying on Gemini for medical, legal, financial, or other professional advice. Its guidance for an inaccurate answer is to use thumbs-down feedback; see Learn about responses from Gemini Apps.
Reporting a Microsoft Copilot response
Microsoft’s Copilot privacy FAQ lists several access points. On Copilot web and mobile, use Give feedback in Settings or the flag icon beneath a response. In the Copilot mobile app, you can also long-press the response and choose Report.
- Keep the response visible and preserve the surrounding prompt and citations.
- Open Settings > Give feedback, tap the flag icon, or long-press the response and choose Report in the mobile app.
- State whether the problem is inaccurate, incomplete, unsafe, inappropriate, or another specific concern.
- Identify the exact sentence or action that creates the risk and attach only necessary evidence.
- Submit the report and retain your local record.
Microsoft says trained AI experts may review conversations to build, evaluate, and improve model accuracy and safety. Its FAQ cautions, “AI can make mistakes, and third-party content on the internet may not always be accurate or reliable.” Check cited sources independently before acting. Provider differences at a glance
The same incident can require different actions depending on the service. ChatGPT puts the primary report control under the individual message and labels the relevant category “Safety or Legal concern”; OpenAI also maintains a separate content-reporting webform. Gemini uses “Good response” or “Bad response” under the answer, requires sign-in for feedback, and lets you choose whether files or images accompany the submission. Its web interface has a distinct “Report legal issue” path. Copilot offers feedback in Settings, a flag under the response, and a mobile long-press “Report” action.

These differences matter for privacy as well as convenience. Gemini warns that the associated conversation, prompts, responses, uploaded material, and personal content obtained through Connected Apps may be sent with feedback. Other products may also collect conversation context, so do not assume that selecting one message limits transmission to that message. Work or school accounts can have additional restrictions, including inability to add attachments or extra comments. If a control is missing, check whether you are signed in, whether the account is managed, and whether the feature is available on the current web or mobile interface instead of repeatedly submitting an unrelated form.
How to write a report that a reviewer can use
A short, factual report is easier to triage than a long account of every interaction. Use this structure:
- Where: name the service, app or web interface, model/version if shown, and conversation or message link if available.
- When: give the date and approximate time; add the time zone if timing affects the harm.
- What happened: quote the prompt and output accurately or attach a readable screenshot.
- Why it is wrong or harmful: identify the false claim, unsafe instruction, targeted person, privacy exposure, or suspected policy/legal issue.
- Correction or evidence: link an authoritative source for a factual error when a suitable source exists.
- Impact and urgency: say whether anyone acted on it, lost money, disclosed data, or faces an immediate risk.
For an inaccurate medical statement, for example, identify the precise dosage or diagnosis claim, link the appropriate public-health or clinical source, and state whether anyone followed it. For a fabricated legal rule, name the jurisdiction and the official statute or court source that contradicts it. For abusive content, quote the relevant words and explain who was targeted rather than speculating about the model’s motives.
A concise report template
You can adapt this wording: “On [date and time zone], [service/interface] produced the following response to this prompt: ‘[exact prompt].’ The answer said: ‘[exact excerpt].’ This is inaccurate/harmful because [specific reason]. [Authoritative source] shows [correction]. [State whether anyone acted on it or faces a current risk]. Please review the response under your accuracy, safety, privacy, or legal process.” Keep the template factual; do not include passwords, access codes, or unrelated personal history.

Reports can contain more data than the reporter expects. A provider may attach the whole conversation, uploaded material, or information retrieved through a connected application. Before pressing Submit, check:
- Whether the form includes every prompt and response in the thread rather than only the selected message.
- Whether uploaded images, documents, audio, or connected-app content will be included.
- Whether names, contact details, health information, authentication data, or confidential work material can be removed.
- Whether the account is personal, work-managed, educational, or subject to an organization’s retention rules.
Keep an unredacted copy privately if you need it for a complaint or dispute, but send the provider the smallest complete record that proves the issue. Never paste passwords, recovery codes, payment-card numbers, or private keys into a feedback form.

Feedback is not the same as a legal complaint
A thumbs-down, flag, or “Bad response” report tells the service that the answer quality or safety needs attention. It generally does not establish that you served a legal notice, requested removal under a specific statute, preserved a formal litigation record, or notified a regulator. If the concern involves defamation, copyright, privacy rights, illegal threats, regulated advice, or another legal claim, follow the provider’s designated legal/content process and any applicable local procedure.
OpenAI’s published framework for reporting model misalignment describes a work in progress for tracking, investigating, and disclosing examples of model misalignment. It is complementary to existing legal disclosure requirements, not a universal consumer complaint route for every inaccurate answer.
What happens after submission
Save the report date, confirmation screen, case number, and a copy of what you sent. Provider guidance for these services does not promise a universal reply, correction, removal, review deadline, or individual explanation. Do not repeatedly submit identical reports in the hope of accelerating a response; add a new report only when there is new evidence, a continuing risk, or a separate affected message.
If the output caused a concrete loss or ongoing harm, preserve independent records such as receipts, notices, medical records, messages, and access logs. Those records may matter more than the platform’s internal feedback result. If a provider offers a privacy, legal, or safety escalation route, use the route that matches the issue rather than placing every concern in a generic quality form.
High-stakes answers need independent verification
AI responses can be fluent while still being incomplete, outdated, or false. Before relying on one for a consequential decision, verify it with an authoritative source or qualified person:
- Medical: a licensed clinician, pharmacist, or public-health authority.
- Legal: the applicable statute, court, regulator, or qualified lawyer in the relevant jurisdiction.
- Financial: a regulated adviser, official filing, bank, or government source.
- Safety: emergency services, the manufacturer’s safety documentation, or a competent safety professional.
- Civic or identity information: the responsible election, government, or public-record authority.
Verification is not a substitute for reporting. It prevents the next person from being harmed while the provider decides whether and how to mitigate the output.
Common mistakes to avoid
- Reporting only a paraphrase while deleting the original prompt and surrounding turns.
- Including an entire confidential file when a short excerpt proves the issue.
- Using a quality button for a legal notice and assuming the provider received a formal claim.
- Arguing about what the model “intended” instead of identifying the observable error or instruction.
- Waiting for a provider response before contacting emergency, medical, legal, or financial help.
- Assuming a citation is reliable merely because the answer displays a link; open the source and compare the claim.
- Submitting the same report repeatedly without new information.
A compact checklist
- Secure immediate human help if there is imminent danger.
- Save the exact prompt, response, citations, time, service, and model/version.
- Remove unrelated personal or confidential information.
- Use the message-level control for accuracy, safety, or offensive content.
- Use the separate legal/content route for a legal or terms-of-service concern.
- Read what conversation, uploads, or connected-app content the report will send.
- Include a precise explanation, authoritative correction, and impact.
- Keep your own copy and do not expect a guaranteed reply or correction.
- Independently verify any high-stakes claim.
