Fresh blood, dried blood, coffee, grease, and ink each call for a different first step. Ordinary stain removal is not infection-control decontamination: if a top is contaminated with blood or body substances, or is PPE, follow your facility’s handling and laundering rules.
Start with the stain and the care label
- Check whether it is safe and allowed to handle the top. If it is contaminated or functions as PPE, follow workplace procedures rather than treating it like ordinary laundry.
- Identify the stain and act promptly when appropriate. Stains are generally easier to remove when treated early, according to the American Cleaning Institute (ACI).
- Read the garment’s care label. Scrubs vary in fabric, color, finishes, and bleach restrictions. Use only treatments and wash temperatures compatible with the label.
- Use one suitable treatment at a time. Avoid aggressive rubbing and do not combine chemicals. For delicate or colored fabric, test a permitted product on an inconspicuous area first.
- Inspect before drying. If the stain remains after laundering, repeat a compatible treatment instead of putting the top in the dryer; heat may make some stains harder to remove.
Choose a first treatment by stain type
Fresh blood
For a fresh blood stain on washable clothing, ACI advises soaking in cold water and then laundering according to the care label. Avoid hot water for the initial treatment because it can set blood stains. Follow the label for the actual wash temperature and any bleach restrictions.

Dried blood
ACI advises pretreating or soaking dried blood in warm water with an enzyme-containing product before laundering. A fabric-safe enzyme laundry stain remover is one option, but check the scrub’s care instructions first and test a hidden area if the fabric may be delicate. This general clothing guidance is not a guarantee for every scrub blend, dye, or finish.

Sponge or soak the stain in cool water, pretreat with a stain remover or liquid laundry detergent, then launder according to the care label. ACI lists bleach only when it is safe for the fabric. Do not assume chlorine bleach is suitable for colored scrubs; check the label and any facility policy.

Grease or oil
For a light stain, pretreat with a prewash stain remover or liquid laundry detergent, then wash at the hottest temperature safe for the fabric. ACI’s heavy-stain method uses cleaning fluid, but compatibility with every scrub fabric and finish is not established. Check the garment instructions and test a hidden area before using any solvent.
Ink
ACI’s method uses rubbing alcohol or cleaning fluid: sponge around the ink, place the stained area face-down on absorbent paper towels, and apply the product to the back so ink can transfer into the towels. Replace the towels as needed, rinse thoroughly, and launder. Some ballpoint, felt-tip, and liquid inks may be impossible to remove. On colored or stretch scrubs, check the garment instructions and test a hidden area first; this method is not guaranteed to be compatible with every dye or fabric.

Iodine or Betadine
A reliable treatment specific to iodine or povidone-iodine stains on scrub fabrics is not established here. Consult the garment maker’s care guidance and test any product the instructions permit in an inconspicuous area. Do not treat peroxide, alcohol, baking soda, or another household chemical as a verified universal solution.
Ordinary stain removal is not decontamination
A stain disappearing does not establish that a garment contaminated with blood or body substances is decontaminated or safe to take home. The CDC’s healthcare laundry guidance distinguishes uniforms without blood or body-substance contamination from contaminated clothing and notes that facility rules vary. It says facilities that require uniforms should provide laundering or give employees cleaning and infection-control guidance based on their tasks. The CDC’s 2003 guideline says that uniforms without such contamination “presumably do not differ appreciably from street clothes in the degree and microbial nature of soilage”; that qualification does not apply to visibly or otherwise contaminated clothing.

The CDC’s 2003 guideline reports bacterial loads of 106–108 CFU/100 cm2 on textiles heavily contaminated with potentially infective body substances. This figure describes heavily contaminated textiles, not ordinary worn scrubs or the risk of laundering a normal uniform.
For garments that serve as PPE, OSHA’s enforcement directive says the employer is responsible for providing, cleaning, maintaining, repairing, replacing, or disposing of the PPE; under the directive’s interpretation, employees may not home-launder that PPE. Follow your employer’s policy for uniforms and protective clothing.

Why laundering rules differ
There is no single wash temperature or chemical recipe for all scrub tops. HSE says reusable contaminated fabric needs a laundering method that considers decontamination and handling to prevent cross-infection. It notes that high temperatures may not be practical for domestic fabrics because of heat sensitivity; for non-clinical workplaces, it recommends the highest temperature the fabric permits. Local policies can be narrower. For example, NHS Fife’s home-laundering appendix excludes theatre scrub suits, PPE, and contaminated uniforms. For uniforms covered by that appendix, it advises the highest temperature suitable for the fabric under the care label and says not to add bleach for whitening. That is a local example, not a rule for every employer.

- Ordinary uniform, not contaminated: follow the care label and your facility’s uniform policy.
- Blood- or body-substance-contaminated clothing: use the handling and laundering process set by your facility.
- Garment serving as PPE: follow the employer’s PPE procedures; do not assume home laundering is permitted.
