A review button alone is not a safeguard: the approval policy, technical controls, human role, and ongoing monitoring have to work together.
Start with the actions the AI can take
An AI system’s risk depends not only on the model but also on the tools, accounts, data, and workflows it can reach. A system that drafts a message for an employee to review has a different risk profile from one that can send the message, change customer records, approve a payment, publish a post, modify code, grant access, or operate equipment.

Make an inventory of the actions available to each AI-enabled workflow. Record the action, the systems and people it could affect, whether it reaches outside the organization, how much harm an error could cause, and whether the action can reliably be undone. Include chained actions: an agent might read a request, retrieve data, change a record, and send a notification in one sequence. Reviewing only the final step can miss how earlier steps shaped it.

This is a practical way to apply the context-mapping and lifecycle approach in NIST’s AI Risk Management Framework (AI RMF) 1.0. The framework is voluntary and use-case agnostic; it is a risk-management resource, not a replacement for applicable law or sector rules. NIST says the framework is being revised.

Describe the real permission, not the feature name
Translate broad labels such as “assistant,” “copilot,” or “agent” into specific capabilities. “Can use email” is too vague for an inventory. Separate reading messages, drafting replies, sending to internal recipients, sending externally, and changing mailbox settings. Apply the same distinction to finance, code repositories, customer records, calendars, and physical systems.
For each capability, note which identity or credentials the system uses, what data it can see, which limits apply, and who owns the connected account. This makes it possible to compare the authority actually granted with the authority the business intends to grant.
Choose when approval is required
There is no universal rule that every AI output needs human review. NIST describes human-AI arrangements ranging from fully autonomous to fully manual, and says the appropriate level of oversight depends on the application and its context. Set approval requirements according to the consequences of an action, not simply because a model produced it.

They are recommendations, not a threshold scheme prescribed by NIST or a rule that applies universally under law. Adapt them to the use case, applicable requirements, and the organization’s risk tolerance.
| Action tier | Typical characteristics | Possible control |
|---|---|---|
| Low impact and reversible | Limited consequences, narrow scope, and a dependable way to undo the action; for example, organizing an internal draft or updating a noncritical work queue. | Allow action within explicit limits, retain an audit record, and review performance periodically. |
| Material or externally visible | Affects a customer, employee, public audience, business record, or external service; an error could be consequential even if it can later be corrected. | Require a named, authorized person to approve the proposed action before execution. |
| High impact, hard to reverse, safety-critical, or rights-affecting | Could cause serious harm, materially affect a person’s rights or access, create a significant financial or operational consequence, or be difficult to undo. | Use stronger review, a tested stop path, and, where law or risk analysis calls for it, a second independent approver. |
Do not infer that every high-impact action legally requires two approvers. The EU AI Act includes a two-person confirmation rule for certain biometric identification cases; that specific provision should not be generalized to other AI actions. The applicable law depends on the system, use, and jurisdiction.

Compare choices using consistent questions
When deciding whether to permit autonomous action, require approval, or prohibit a capability, assess the same factors for each workflow:
- Impact: Could an error affect a person’s safety, rights, finances, privacy, or access to a service, or disrupt operations?
- Reversibility: Can the organization restore the prior state reliably, and would undoing the action also undo its consequences?
- Exposure: Does the output remain an internal draft, or does it reach a customer, the public, a regulator, or an external system?
- Autonomy and access: How many tools and accounts can the system use, and can it chain actions without a new decision point?
- Human response: Is a competent, authorized person available to understand and stop the action before it takes effect?
- Evidence and monitoring: Can the organization reconstruct what was proposed, approved, and executed, and detect changes in behavior?
These are decision axes synthesized from NIST’s risk-mapping approach and official oversight guidance, not a regulator-issued scoring rubric. The assessment should lead to a clear control decision, not just a score. A low score in one area does not erase a serious consequence in another.

Enforce limits outside the model
A system should not be the sole judge of whether it is allowed to act. Put boundaries in the surrounding software and operational process so the model cannot simply decide to bypass them. This is an engineering recommendation based on the EU AI Act’s discussion of operational limits that a high-risk AI system cannot override, the need for effective human oversight, and NIST’s attention to AI security. The cited guidance does not prescribe one exact implementation list.
Grant only the access the task needs
- Use scoped credentials and least privilege. Give the system access only to the accounts, records, and operations required for its defined task.
- Separate read access from write access. Where possible, begin with read-only permissions and add write capability only when the workflow requires it.
- Use explicit action allowlists. Define which operations, destinations, recipients, or record types are allowed instead of relying on broad access and a prompt telling the model to behave.
- Set transaction, rate, or volume limits when appropriate. Such limits can contain a mistake or misuse, but should be chosen for the workflow rather than treated as a universal safety threshold.
- Keep test and production environments separate so experimentation cannot affect live users or records.
Put approval in the execution path
For an action that requires human authorization, the system should present the proposed action to the reviewer and wait for an approval decision before the operation is executed. The gate must be enforced by the application or service that performs the operation, rather than depending only on the model’s willingness to pause. Define what happens if the reviewer rejects the request, the approval expires, or the approval service is unavailable.
Make sure the stop control is available to an authorized operator even while the system is active. Define who can pause execution, revoke credentials, or disable a capability, and test that those measures take effect in the connected systems. A stop instruction that the AI can ignore, or a person cannot activate in time, is not a dependable control.

Make human approval meaningful
Human oversight works only when the assigned person can understand the proposed action, has the competence to judge it, and has real authority to approve, reject, pause, or stop it. A reviewer who sees only an “Approve” button, lacks time, or cannot prevent execution is not providing meaningful oversight.
For each approval request, present enough information to make a decision without asking the reviewer to reconstruct the system’s work from scratch:
- What the AI proposes to do, including the specific data or records it will change.
- Why it proposes the action and which instructions or workflow triggered it.
- The information it relied on, including relevant source material and uncertainty or missing information when available.
- Who or what will be affected, what happens after approval, and whether the action can be reversed.
- How to reject, request changes, pause execution, or escalate the request.
Do not treat a persuasive explanation as proof that a proposal is correct. The reviewer needs enough underlying context to check the action, not just a fluent summary. NIST cautions that presenting information from AI systems to people is complex, and says roles and responsibilities should be clearly defined.
Assign an accountable role
Name the role responsible for approving each category of action and identify a backup for absences or urgent cases. The person needs relevant competence and training, and the authority to refuse a proposal or stop the workflow without being pressured to approve it automatically. Explain the system’s known limitations and the consequences of approving an incorrect action.
Set response expectations that match the risk and urgency. If a reviewer cannot respond within the time available before an action takes effect, the workflow should wait, expire, or fail safely rather than silently proceed. Define who can revoke access and how the organization will reach the appropriate operator outside ordinary business hours where that matters.

Test controls before increasing autonomy
Begin with simulations, a sandbox, or read-only access where practical. Expand permissions in stages only after the organization has evidence that the action boundaries and approval flow work for the intended setting. This staged plan is an operational recommendation, not a benchmark or a guarantee of safety.
Test both ordinary tasks and conditions that could expose weaknesses, including:
- Ambiguous, incomplete, or conflicting instructions.
- Untrusted text or data that attempts to redirect the system.
- A proposed action that exceeds its allowlist, transaction limit, or access rights.
- A permission change, expired credential, or account with unexpectedly broad access.
- An outage affecting a connected service, approval queue, or monitoring system.
- Repeated attempts to get the system to take a prohibited action.
- A reviewer who rejects a request, is unavailable, or needs to stop an action already underway.
Evaluate whether disallowed operations are actually blocked, whether the approval request gives a reviewer enough information, and whether an authorized person can stop execution in time. Record failures and near misses, fix the control that failed, and rerun the test before expanding the system’s scope. NIST’s AI RMF calls for continuous lifecycle risk management; NIST also identifies agent systems as an area for ongoing AI-security control work.
Log activity and respond to exceptions
Keep records that allow the organization to trace a proposed action through to its result. As appropriate to the workflow, capture the proposal, the authorization decision, the approver’s identity, the action actually performed, relevant overrides, and errors. Protect the records according to the sensitivity of the underlying data and applicable retention rules.

If behavior exceeds the approved scope, reduce permissions or disable the relevant capability while the cause is investigated.
Best Value
Under applicable EU AI Act requirements and timelines, official Commission materials address logging, human oversight, monitoring, and incident reporting for high-risk AI systems. Which obligations apply depends on the system’s classification, role, and use; an organization should check the relevant legal provisions rather than assume that the same requirements cover every AI deployment.
Keep risk management active through the lifecycle
Controls can become inadequate when a model, connected tool, workflow, data source, user group, or deployment setting changes. Review the action inventory and approval policy when those conditions change, and periodically check whether the system still operates within its intended scope. Reassess access and oversight when an organization adds a new tool, changes an account’s permissions, or moves a workflow from internal use to customer-facing use.
NIST AI RMF 1.0 organizes risk-management work into four functions: Govern, Map, Measure, and Manage. Governance applies across the other functions, and NIST says risk management should continue across the AI system lifecycle. In practice, this means assigning responsibility, understanding the context, evaluating risks and controls, then responding as evidence or conditions change.
Recommended Free Tools
“Risk management should be continuous, timely, and performed throughout the AI system lifecycle dimensions.”
That statement appears in NIST’s AI Risk Management Framework 1.0 (2023). It is a useful operating principle, not a claim that continuous review by itself prevents every unauthorized action.
Understand the legal scope before deployment
Legal duties depend on jurisdiction and use; the EU AI Act should not be read as a universal rule for all AI systems. As of 13 October 2026, the European Commission’s AI Act overview says transparency rules apply from August 2026. It gives 2 December 2027 for high-risk rules concerning specified sensitive areas and 2 August 2028 for rules concerning high-risk systems embedded in regulated products. These are implementation dates, not performance statistics. Confirm the current legal text and the provision that applies to a particular product or deployment before relying on a date.
The AI Act Service Desk FAQ explains that an AI agent may be subject to transparency rules depending on its use, and that a high-risk classification can bring additional requirements on the applicable timeline. It also says autonomy or tool use can matter to systemic-risk assessments for some general-purpose models, while describing agent-specific regulatory considerations as preliminary. Do not assume that calling a system an “agent” automatically determines its legal classification.
NIST’s AI RMF is a voluntary resource. It can help an organization structure risk management, but it does not replace legal advice, sector-specific obligations, or requirements that apply to a particular use. The same distinction applies to the technical controls in this guide: they are practical recommendations, not a guarantee of compliance or safety.
