Reduce hallucinations by giving an enterprise AI agent a narrow job, grounding factual answers in authoritative and current data, and limiting its tools and permissions to what that job requires. Require evidence for factual claims, define when the agent must abstain or hand off, and evaluate retrieval, answers, and actions separately. Grounding makes unsupported invention less likely; it does not guarantee that the retrieved information is correct.

What grounding and narrow workflows actually do

Grounding connects an agent’s output to information that can be checked. Google Cloud describes it as connecting model output to verifiable sources. Depending on where trusted information lives, its documented approaches include enterprise document retrieval, managed RAG, existing Elasticsearch indexes, search APIs, and web search. The right choice depends on source authority, freshness, and required controls—not on the feature label alone.

A narrow workflow reduces the number of decisions the agent must make and the damage it can cause when it is wrong. Microsoft’s agent guidance treats clear instructions as boundaries against scope creep, while noting that agents dynamically select knowledge and tools. That flexibility makes testing and governance essential.

Neither control is a truth guarantee. A retrieved document can be outdated, incorrectly ranked, or maliciously changed. An agent may also complete a run without a technical error and still produce a wrong, incomplete, or policy-breaking result, as AWS cautions in its agent evaluation guidance.

Build the workflow around one bounded job

Choose a task with a clear owner and source of truth

Start with a recurring task that has a business owner, a stable way to verify success, and a limited set of permitted actions. Information retrieval and synthesis, ticket creation, and system monitoring are examples in Microsoft’s agent guidance. Define what the agent may do, what is out of scope, and which cases require a clarifying question, refusal, or human handoff.

Keep read-only answers separate from write actions while validating the workflow. A request to explain a policy and a request to change a customer’s account are not equivalent risks, even if they concern the same subject.

Write boundaries as behavior, not aspirations

Specify which users, records, topics, and actions are in scope. Make hard business rules explicit and enforce consequential state changes in application logic where possible. Use the model for language understanding and bounded ambiguity, not as the sole enforcement mechanism for policy.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make the grounding data trustworthy and current

Map authoritative sources and their owners

Inventory the systems and documents the agent may use, who owns them, how often they change, and how access restrictions apply. Decide how updates, deletions, and permission changes reach the retrieval layer. Prefer sources that can be identified in the final answer and inspected later.

Static documents are not appropriate for every fact. For volatile information such as account state, entitlements, or order status, retrieve the current value from an authorized system or API rather than relying on a copied document that may have gone stale.

Treat retrieved content as evidence, not authority

Retrieved text should not override the agent’s governing instructions or business rules. Validate source ownership and ingestion paths, restrict who can edit content and indexes, and keep source or index version history. Monitor for unusual retrieval patterns, including sudden changes in top results or repeated retrieval of low-trust material.

Microsoft’s guidance on grounding-data compromise describes risks involving public pages, internal wikis, vector databases, embeddings, and index metadata. The practical implication is that a grounded answer can still be confidently wrong if its evidence has been corrupted or misrepresented.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Limit tools, permissions, and action authority

Give the agent access only to retrieval collections and tools needed for its assigned workflow. Tool schemas should make allowed inputs and outputs clear; permissions should be no broader than the task requires. For consequential actions, require explicit confirmation or human review rather than relying on the model to decide whether an action is safe.

For multi-stage tasks, use explicit stages with typed inputs and outputs—for example, retrieve a policy, verify the relevant clause, draft a response, then submit only after approval. Define stage-specific failures and fallbacks so a retrieval failure does not silently become an invented answer or an unintended action. AWS’s agent guidance recommends modular reasoning stages and fallback strategies for this reason.

Require evidence-aware answers and safe abstention

For factual responses, instruct the agent to answer from retrieved evidence and provide source references users can inspect. Keep source facts distinct from inference: a statement directly supported by a policy is different from a conclusion drawn by combining several passages.

When sources are missing, conflicting, stale, or outside the agent’s authorized scope, the expected behavior should be explicit: say what could not be verified and offer a safe next step, such as asking a clarifying question or routing the case to a person. Do not reward plausible completion when the evidence does not support it.

Evaluate retrieval, answers, and actions separately

Build a repeatable test set

Create realistic cases for ordinary requests and difficult conditions. Include ambiguous inputs, stale or conflicting documents, permission boundaries, tool failures, no-result queries, and requests outside the workflow. For each case, specify the grounding data, expected behavior, and assertions that can be checked.

  • Did retrieval find the correct authoritative source?
  • Is each required factual value accurate and supported by the cited evidence?
  • Did the answer avoid exposing data the user is not allowed to see?
  • Did the agent abstain or escalate when evidence was insufficient?
  • Was a write tool avoided unless the required approval was present?

Use atomic assertions—for example, a required value is correct, the policy source is cited, or no write tool is called without approval. Score retrieval quality, answer groundedness, and task completion as separate outcomes. A good final response can mask poor retrieval if it guessed correctly; a correct retrieval can still be misrepresented by the answer.

Run regression checks after changes

Keep known failures as test cases. Rerun the same suite when prompts, models, tools, or knowledge sources change, and compare configurations before deployment. Microsoft’s evaluation guidance emphasizes that without evaluation, teams cannot reliably tell whether an agent change improves or degrades quality. Evaluation is not a substitute for separate security testing.

Evaluate production behavior, not just uptime

Sample production traces and assess them against task-specific quality criteria. AWS documents a development loop based on curating traces, scoring them with evaluators, and comparing versions, as well as online evaluation of sampled live traffic. Feed failures into the regression set and track quality changes over time; a run that returns successfully is not necessarily a successful answer.

Make failures observable and traceable

Capture enough information to determine where a failure occurred: the user input, retrieved references or context, tool calls, agent output, and evaluation signals. Apply the organization’s access and retention rules to those records. AWS recommends modular stages, fallbacks, and telemetry for stage health and retrieval quality so teams can distinguish a retrieval problem from a reasoning or action problem.

Monitor whether the correct sources appear among top retrieval results, whether source distributions shift unexpectedly, and whether ranking or embedding behavior drifts. Review new or low-trust content and audit ingestion. Test prompt injection and adversarial inputs as a distinct security practice rather than assuming ordinary answer-quality evaluation will catch them.

Choose an implementation by controls, not feature names

Compare candidate designs against the needs of the specific workflow. Vendor documentation provides implementation guidance, not independent head-to-head evidence that one option reduces hallucinations more than another.

  • Source authority and freshness: Is the source an owned enterprise corpus, a live system, or public web content? Who maintains it, and how quickly are changes reflected?
  • Retrieval behavior: Does the system find and rank the right documents, respect permissions, and fail safely when there are no useful results?
  • Traceability: Can users see source references, and can operators inspect retrieval steps, tool calls, and replayable traces?
  • Workflow control: Are tools allowlisted, permissions scoped, consequential actions gated by approval, and deterministic rules enforced in application logic?
  • Evaluation and operations: Can the team run grounded regression cases, compare versions, sample production behavior, and monitor retrieval quality?
  • Governance fit: Do access controls, retention, regional requirements, latency, cost, and platform compatibility meet the organization’s needs?

There is no established universal hallucination-reduction percentage or best model and retrieval configuration in the cited vendor guidance. Measure a baseline on the organization’s own tasks and corpus, then report results with the task, dataset, model and configuration, date, and evaluation method.
