To reduce hallucinations in an AI-generated document summary, treat the supplied document as the evidence boundary, check each claim against a passage in that document, and remove or correct anything the source does not support. Review omissions as well as additions: a summary can be faithful but too incomplete to serve its purpose.
What counts as a hallucination in a document summary?
In summarization, faithfulness is measured against the document being summarized. A statement may be true in the wider world and still be a hallucination in the summary if the source does not support it. The key test is not simply whether a claim sounds plausible, but whether a reader can trace it to evidence in the input.

Human evaluation of abstractive summarization has found outputs that are unfaithful to their inputs, irrelevant, or incoherent. That is why fluent writing alone is not a reliable quality check. See Maynez and coauthors’ 2020 study, On Faithfulness and Factuality in Abstractive Summarization.
A practical workflow for checking a summary
1. Define the purpose and scope
Before generating a summary, decide who will use it, what they need to know, and how much detail is appropriate. A meeting brief, an executive overview, and a summary used to support a consequential decision need different levels of coverage. Concision is not an objective by itself: the right length depends on the task.

- Memory: 32GB DDR4 Ram
- Storage: 1TB Solid State Drive (SSD) Fast Boot + Storage
- Operating System: Windows 11 Pro (64-bit)
2. Keep the source as the evidence boundary
Provide the document to the summarizer and ask it to rely on that document for factual claims. Instruct it to flag missing, ambiguous, or conflicting support rather than filling gaps from general knowledge. This reduces the risk of mixing outside facts into a source-based summary, but it cannot guarantee that every generated claim will be supported.
3. Split the summary into checkable claims
Review each sentence, then break sentences containing multiple ideas into individual clauses or atomic claims. For every claim, locate the passage that supports it. Mark claims that are unsupported, stronger than the source, or that change a qualification, number, attribution, or relationship. Correct them using the source or remove them.
For example, if a document says a method “may improve” an outcome in one study, a summary that says the method “improves” the outcome generally overstates the evidence. Check qualifiers and scope as carefully as the central claim.

LongEval surveyed 162 papers on long-form summarization and reported that 73% did not conduct human evaluation. The authors also examined finer-grained review: in their study, moving from coarser to clause-level judgments reduced the standard deviation of faithfulness scores from 18.5 to 6.8. These are findings about that study, not guaranteed improvements for every document or review team. The paper discusses the evaluation challenges in LongEval: Guidelines for Human Evaluation of Faithfulness in Long-form Summarization.
4. Make claims traceable to source passages
Record the supporting passage beside each claim, or use a summarization system that produces source-linked claims. A citation is useful only if it points to relevant evidence that a reviewer can inspect; a citation that merely accompanies a fluent sentence does not establish that the sentence is supported.
NIST’s 2024 paper on machine-generated reports states: “Additionally, evaluation of citations that map claims made in the report to their source documents ensures verifiability.” This describes the value of evaluating claim-to-source mappings, not a guarantee that any particular citation system will be accurate. See Toward Evaluating the Verifiability of Machine-Generated Reports.

5. Use an automated critique, then verify it
A second AI pass can flag claims that may not follow from the source. Ask the reviewer to list each questionable claim and identify the passage it believes supports or contradicts it. Then check the original document yourself. The critique is a way to direct attention, not an independent authority: it can miss a flaw or introduce a mistaken one.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsIn an OpenAI study published in 2023, evaluators who saw model-written critiques found 50% more flaws in model-written summaries than a control group. For deliberately misleading summaries in that study, detection increased from 27% to 45%. Those results apply to the study’s task and setup; they do not predict the improvement a different team will get from another tool. Details are in Finding GPT-4’s mistakes with GPT-4.
6. Check important omissions
Compare the summary with the purpose you defined and the source’s material points. Ask whether it leaves out a key finding, exception, limitation, disagreement, or condition that changes how a claim should be understood. A summary that avoids unsupported additions by dropping necessary information is not useful enough.

Faithfulness and coverage are related but distinct checks. OpenAI’s work on summarization with human feedback describes the trade-off between concision and coverage, and notes that preferred length depends on purpose. A 2025 study proposes a question-answer-based process for checking claims against a source, prioritizing checks, and revising and reevaluating summaries; its reported benchmark gains are specific to its method and evaluation. See Summarizing books with human feedback and the 2025 Scientific Reports study.
7. Escalate consequential claims to a person
Have a qualified human verify claims when a false statement or a missing detail could materially affect a decision. For long documents, use the same claim-level rubric across reviewers and preserve the source passages used to resolve disagreements. Consistency makes the review easier to reproduce; it does not remove the need for judgment.
How to choose a review method
No single evaluation method is established as universally best. Choose a method based on how much detail the summary needs, how readily claims can be traced, and what a reviewer can realistically check.

| Review approach | What it checks | Best use | Limit to account for |
|---|---|---|---|
| Sentence-level review | Whether each sentence is supported by the source | Short summaries with simple claims | A sentence can combine multiple claims, making a partly unsupported sentence harder to diagnose. |
| Clause- or claim-level review | Whether each smaller factual unit is supported | Long, dense, or consequential summaries | Requires more review effort; LongEval’s reported results are specific to its study. |
| Source-linked citations | Whether a reviewer can trace a claim to an evidence passage | Summaries that need an audit trail | A link is not proof by itself; the cited passage must actually support the claim. |
| Automated critique | Potential unsupported claims or inconsistencies to inspect | A second-pass aid when a human will verify flagged issues | The critique can miss problems or be wrong, so it cannot replace source review. |
| Atomic content-unit evaluation | Whether discrete summary units are supported by the source | Structured evaluation or comparison of summaries | ACUEval’s reported correlation results are tied to its benchmarks and study design. |
ACUEval evaluates summaries by breaking them into atomic content units and validating those units against the source. Its authors report stronger correlation with human faithfulness judgments than the next-best evaluated metric on three benchmarks. That is evidence for the approach in those evaluations, not proof that it will outperform every alternative on every document. See ACUEval: Fine-grained Hallucination Evaluation for Summarization.
A reusable claim-checking rubric
For each claim, record the summary wording, the source passage, and a decision. Use consistent categories so reviewers distinguish different problems rather than treating every concern as a vague “hallucination.”
- Supported: The source passage entails the claim, including its scope and qualifications.
- Unsupported: No passage in the supplied document supports the claim.
- Overstated or altered: The source supports a narrower or differently qualified version of the claim.
- Unclear: The source is ambiguous, incomplete, or conflicting; preserve that uncertainty rather than resolving it by guesswork.
- Omitted: A point required by the summary’s purpose is absent or lacks a necessary qualification.
For unsupported or overstated claims, either revise the wording to match the source or delete it. For unclear claims, say what the document leaves unresolved. For omissions, add the missing point only if it is supported and relevant to the intended reader.

Research supports finer-grained, source-based evaluation as a useful way to make faithfulness checks more consistent. LongEval found that, in its study, using 50% of fine-grained judgments correlated with full-workload scores at Kendall’s tau 0.89. This is a result from that evaluation, not a general rule that half of any review will be sufficient.
Methods and metrics also have boundaries. LongEval addresses human evaluation of long-form summaries; ACUEval tests atomic content units; the 2025 question-answer-based study evaluates a particular checking and revision method. Their findings do not identify a foolproof prompt, metric, model setting, or product that guarantees a hallucination-free summary.
