It does not guarantee that an answer is true: search can miss or select the wrong evidence, source documents can be outdated, and the model can still overstate what its sources say.
What RAG does
A language model usually answers from patterns learned during training and the context in its current prompt. RAG adds a search step: the system finds potentially useful passages in a maintained collection of documents, then supplies selected passages alongside the user’s question. The model can use that material to produce an answer that is more specific or current than one based only on its learned knowledge.

OpenAI defines RAG as “the process of Retrieving content to Augment your LLM’s prompt before Generating an answer.” In plain terms, the model gets notes for this question; it does not acquire new learned knowledge or have its underlying weights changed. OpenAI’s accuracy guide and Anthropic’s September 19, 2024 explanation describe this general approach.

How a RAG chatbot finds and uses evidence
A typical system has a preparation phase and a question-answering phase. The quality of both affects the final response.

Prepare the knowledge collection
- Collect, clean, and update the documents the chatbot is allowed to use.
- Split long documents into smaller passages, or chunks, so search can return relevant sections rather than entire files. Preserve useful context such as a title, date, or source identifier.
- Create embeddings—numerical representations that help find passages with similar meaning—and place them in a search index. Depending on the system, search may also use exact-word matching.
Anthropic describes chunks of a few hundred tokens as common in its 2024 discussion, not as a universal rule. Chunk size and overlap need to be tested: a broad chunk can bury the useful detail, while a tiny one can lose who or what a statement refers to.
Answer a question
- The user asks a question.
- The system searches the indexed material for candidate passages. It may combine semantic search with lexical search, which matches exact words or identifiers.
- The system ranks and selects passages, then adds them to the prompt with the question.
- The language model writes a response. A well-designed system may also show references to the underlying sources.
RAG therefore has at least two places to fail: retrieval may provide poor evidence, or generation may use good evidence badly. Microsoft’s Copilot Studio guidance describes RAG as best suited to factual questions and answers, rather than deep document analysis.
Why RAG does not stop a chatbot from making things up
RAG makes evidence available; it does not verify that every sentence in the answer follows from that evidence. OpenAI describes hallucinations as “plausible but false statements generated by language models.” The model can still fill gaps with a plausible-sounding claim, misunderstand a passage, or state a conclusion more confidently than the sources justify.
Rank #2
- The answer is absent. If the knowledge collection does not contain the needed fact, retrieval cannot supply it.
- The right passage is not found. Search terms, embeddings, chunk boundaries, metadata, or ranking may keep useful evidence out of the selected results.
- The passage is incomplete or stale. A chunk separated from its surrounding context may be misleading, and an outdated source can produce an outdated answer.
- The model overreaches. Even when relevant passages are present, the model may combine them incorrectly or add details they do not support.
For those reasons, “RAG-enabled” is not the same as “fact-checked.” A citation is useful only if it points to a relevant source that actually supports the claim.
How to improve a chatbot that gives unsupported answers
Debug the evidence pipeline before assuming a prompt change will fix the problem. For each failed answer, check whether the source was available, whether search found it, and whether the response stayed within what it said.
1. Check source coverage and freshness
Confirm that the answer exists in the documents, that the chatbot has access to the relevant document, and that the indexed copy is current. A missing or unindexed source cannot be retrieved.

2. Inspect the retrieved passages
Save the passages returned for representative questions, including questions that previously produced bad answers. If the needed evidence is missing, investigate retrieval. If it is present but the answer is unsupported, investigate how the model uses the context.

3. Test chunk boundaries and context
Try different chunk sizes and overlap, and check whether passages retain important surrounding details such as the subject, company, or time period. Google Cloud recommends evaluating chunking choices rather than assuming one setting works for every corpus. See its RAG evaluation guidance.
4. Match the search method to the question
Semantic search can find conceptually related passages. Lexical search, including BM25-style methods, can be better for exact strings such as error codes, product names, or identifiers. Hybrid search combines the two. Compare the options on the kinds of questions your chatbot actually receives; no search method is best for every collection.

5. Tune ranking and the amount of context
Test how many passages to retrieve, how to rank them, whether metadata helps, and whether a relevance threshold should exclude weak matches. More passages are not automatically better: irrelevant context can distract the model. In an example discussed in OpenAI’s accuracy guide, adding RAG context reduced accuracy for a task the model already handled because the extra context introduced noise.
6. Teach and test appropriate uncertainty
Instruct the chatbot to base factual claims on the supplied evidence and to say when it does not have enough information. Then test that behavior with questions whose answers are deliberately absent from the collection. A system that guesses on unanswerable questions can appear helpful while inventing facts. OpenAI discusses this evaluation problem in its explanation of why language models hallucinate.
How to tell whether RAG is helping
Use a set of questions representative of the real task, with trusted answers or source passages for comparison. Evaluate retrieval and final answers separately: a correct answer may be a lucky guess, while a good retrieved passage may still be mishandled by the model.

- Retrieval relevance: Did the results include passages that address the question?
- Evidence coverage: Were the necessary facts present, with enough context to interpret them?
- Answer correctness and grounding: Is the response accurate, and can its factual claims be supported by retrieved material?
- Unanswerable-question behavior: Does the chatbot acknowledge missing evidence rather than guess?
- Operational fit: Is the system’s latency, access control, and maintenance burden acceptable for the use case?
Establish a repeatable baseline and change one component at a time—such as chunking, search method, ranking, or prompt instructions—so you can identify what improved or worsened results. Google Cloud’s evaluation guidance recommends this kind of systematic testing. A single successful demo is not enough to establish performance across real questions.
When RAG is a good fit—and when it is not
RAG is a natural fit when a chatbot needs to answer factual questions from specialized, private, external, or frequently updated material. The documents must be available to the system, kept current, and searchable. RAG also adds work: indexing and refreshes, retrieval tuning, access-control checks, and regression testing as the system changes.
For a small collection, putting the material directly in the prompt may be simpler. Anthropic’s September 2024 article suggests that a knowledge base below 200,000 tokens—about 500 pages in its rule of thumb—may be small enough to include without RAG. That is not a universal limit; the practical choice depends on the model, task, and collection. OpenAI’s example of context adding noise is another reason to compare the simplest workable approach with RAG on the intended task.

RAG is also not a substitute for full-document comparison, policy-compliance evaluation, or complex reasoning across long unstructured documents. Microsoft’s Copilot Studio guidance distinguishes those tasks from factual question answering. For more involved retrieval, Azure AI Search documents both classic RAG and newer agentic retrieval approaches; its RAG overview describes trade-offs, but availability and performance depend on the specific service and configuration.
What the published RAG numbers do—and do not—show
Anthropic reported that its Contextual Retrieval method reduced failed retrievals by 49% in its experiments, and by 67% when combined with reranking. These are vendor-reported results for the method and experiments in its September 2024 article, not a general benchmark for all RAG systems and not evidence that hallucinations fall by the same amount. The figures illustrate that retrieval techniques can improve retrieval in a tested setup; they do not establish a universal success rate.
There is no guarantee of zero hallucinations from adding a search index, changing a prompt, or lowering a model’s temperature. The meaningful question is whether the complete system—sources, indexing, retrieval, ranking, answer behavior, and evaluation—performs reliably on the questions it is meant to answer.
