Progress in Last Asylum: Plague means advancing your Sanctuary, clinic, research, and hero squad—not leveling a hero called Plague Doctor. Start by checking the next Sanctuary upgrade’s requirements, then focus resources on the systems that unlock that upgrade or strengthen the team you actually use.
Use the Sanctuary as your progression map
The Sanctuary is the account’s main progression gate: its upgrades unlock buildings and systems and raise the hero level ceiling. The official game site describes gathering timber, herbs, food, and supplies for upgrades; the Google Play listing also describes production, warehouses, residences, and territory expansion.

Community guides identify notable unlocks around Sanctuary levels 6, 7, 8, 10, 15, and 20. Among the examples they give are core systems around level 6; the Lab and Raven Nest around level 7; a second squad at level 8; Alliance Duel at level 10; Raven growth systems at level 15; and a third squad at level 20. These are guide-reported milestones, not a guarantee of identical requirements in every version. Check your own upgrade screen before planning around them.
Choose upgrades by the next requirement
Before spending materials or speedups, open the Sanctuary upgrade screen and note every prerequisite. Community upgrade advice recommends keeping required buildings—and, in particular, the Lab, Training Grounds, and Alliance Hall—close enough to avoid blocking the next Sanctuary level. Costs and timings in community tables are estimates; your in-game values are the ones to follow.
#1 Best Overall
When several upgrades are available, weigh their unlock value, whether they remove a current prerequisite, their resource and time cost, and whether they help your active squad. As Sanctuary costs rise, maintaining every building at the same level can consume resources without solving the immediate bottleneck.
Keep the clinic moving
The clinic is a progression system, not just a side activity. A community clinic guide says clinic stars contribute to Sanctuary progression and describes a fixed sequence of doctor upgrades covering movement, healing, radius, and carry capacity. Since the sequence is fixed, there is no custom doctor build to optimize: keep playing the clinic so its progress does not become a later blocker.

Invest in a focused hero squad
Hero growth uses separate materials and tracks. A community upgrade guide describes Antitoxin for levels, hero-specific or omni shards for stars, and Skill Badges for skills; some skill levels require the hero to reach a star threshold first. That makes spending evenly across every unlocked hero an easy way to dilute scarce materials.

Choose research for compounding gains
Research can improve more than a single battle. A community research guide recommends prioritizing development technologies early because they affect construction, research, training, and resource pace. It then suggests Alliance Duel technologies and research suited to the heroes or soldiers the player relies on.
Use that as a decision framework rather than a rigid queue: favor technologies that improve systems you are actively using or remove a recurring constraint. A combat-focused branch may make more sense when the account’s immediate need is squad strength; a development technology can have wider value when building and research queues are the bottleneck. Branch benefits and tradeoffs depend on your stage and goals.

What to do when progress stalls
A stalled Sanctuary upgrade usually points to a prerequisite, resource shortage, or supporting system that has fallen behind. Diagnose the specific blocker instead of upgrading unrelated buildings or spreading hero materials thinly.
- Missing building level: Upgrade the named prerequisite and check whether another building is required first.
- Clinic stars: Return to clinic play and continue the doctor’s fixed upgrade progression.
- Insufficient materials or time: Review production and queues, then reserve resources for the upgrades that unlock your next goal.
- Squad too weak: Strengthen the heroes and troops you actually use rather than investing across the full roster.
- Unsure which system matters most: Compare unlock value, bottleneck relief, cost, lineup impact, and long-term gains from research.
The right next action depends on your Sanctuary level, unlocked research, clinic stars, lineup, and current stockpiles. Without those account details, no single upgrade can be called the correct choice for every player.
