To prioritize vulnerabilities beyond CVSS, combine technical severity with evidence of exploitation, predictive threat signals, the affected asset’s exposure and business impact, and a defined response policy. CVSS helps describe a flaw; it does not decide what your organization should patch first.
What CVSS tells you—and what it leaves out
CVSS is useful for understanding a vulnerability’s technical severity and potential impact under the conditions represented by its score and vector. Keep the CVSS version, score, and vector with the finding where available: the vector helps reviewers see the assumptions behind the number.

A severity-sorted queue still cannot tell you whether the vulnerable software is present in your environment, whether the affected system is reachable, whether exploitation is occurring, or what a compromise would mean for your organization. Those are separate questions. A high score is important technical evidence, but it is not, on its own, a complete risk assessment or an automatic patch order.

Which signals should you combine?
These inputs describe different kinds of evidence. Use them together, and record their source and freshness so a reviewer can distinguish an observation from a prediction or a local judgment.
| Input | Question it helps answer | How to use it | Limit to keep in view |
|---|---|---|---|
| CVSS | How severe is the vulnerability’s technical impact under the scored conditions? | Use the score and vector as technical severity and impact evidence. | It does not by itself establish local exposure, asset criticality, or observed exploitation. |
| CISA KEV | Does CISA list the vulnerability in its catalog of known exploited vulnerabilities? | Use inclusion as a strong reason to elevate remediation attention. | The catalog is not a complete census of exploitation; absence is not proof that a flaw is safe or unexploitable. |
| EPSS | What does a predictive exploitation signal contribute to prioritization? | Consider it alongside observed threat evidence and local context. | Do not set thresholds or interpret a particular value without checking FIRST’s current definitions, score interpretation, and prediction horizon. |
| SSVC | Which response decision follows from defined vulnerability factors? | Use a documented decision process that can account for factors such as exploitation, safety impact, and affected-product prevalence. | Check the current official guide before adopting a particular tree or its thresholds. |
| Asset and mission context | What would exploitation mean for this organization and this asset? | Assess exposure, criticality, data, safety, and mission or business impact. | Useful decisions depend on accurate asset, ownership, and business-impact information. |
| Remediation policy | What action follows from the evidence? | Assign tiers, owners, target times, exceptions, and review points. | Thresholds are local policy choices, not universal rules. |
Use KEV as observed-threat evidence
CISA describes its KEV catalog as an authoritative source for vulnerabilities exploited in the wild and recommends using it as an input to prioritization. CISA’s guidance says: “Organizations should use the KEV catalog as an input to their vulnerability management prioritization framework.” Check the CISA KEV catalog and guidance as part of triage. A match is an escalation signal; a non-match should not lower a finding to “safe.”
Keep prediction distinct from observation
KEV records known exploitation; EPSS is a predictive signal. They answer different questions, so neither should be treated as a substitute for the other. Without consulting FIRST’s current primary documentation, do not attach a numeric meaning, time horizon, or universal cutoff to an EPSS value.

SSVC is decision-oriented: it helps turn vulnerability factors into a response choice rather than producing only another severity number. CISA’s SSVC announcement identifies factors including exploitation status, safety impact, and affected-product prevalence. If you adopt a specific decision tree, document the version and verify its current decision points before applying thresholds.

How does the affected asset change priority?
First establish that the vulnerable product and version are actually present. Then determine who owns the asset, where it runs, whether it is exposed or reachable, and what depends on it. A vulnerability record with no confirmed affected asset is not yet a meaningful remediation assignment.
Assess the consequences of compromise in your organization’s terms: for example, whether the asset supports a mission-essential service, holds sensitive data, affects safety, or provides access to other systems. Consider compensating controls and operational constraints too, but record them as context for a decision—not as a reason to quietly discard an unresolved vulnerability.
NIST IR 8179 frames criticality around how programs, systems, and components support organizational goals, and the impact if their operation is inadequate or lost. That makes local asset and mission context essential to a defensible priority.

Use a documented workflow that preserves both the evidence and the decision. A transparent risk matrix or a documented SSVC process can work; the important thing is to retain the factors and rationale rather than publishing an unexplained composite score.
Best Value
- Confirm applicability. Verify the affected product and version, then record the asset, owner, environment, exposure, and business or mission role.
- Capture technical severity. Record the CVSS version, score, and vector where available, along with relevant component details.
- Check exploitation evidence. Look for KEV inclusion and relevant trusted threat intelligence. Note the source and when it was checked.
- Add predictive evidence carefully. If using EPSS, consult FIRST’s current primary documentation before assigning meaning to a value or setting a cutoff.
- Assess local consequences. Evaluate reachability, asset criticality, sensitive data, safety or mission impact, and relevant compensating controls.
- Choose and record the decision. Apply your risk matrix or documented SSVC process; save the rationale and the factors that changed the outcome.
- Assign an action. Map the decision to a remediation tier, responsible owner, target date, and escalation path. If patching is not currently feasible, document interim controls, the exception approver, and an expiry or review date.
- Reassess when conditions change. Revisit the decision when exploitation evidence, KEV status, asset exposure, business criticality, available fixes, or operational constraints change.
Set tier names and target times to fit your organization’s risk tolerance, capacity, and obligations. A deadline is useful only when it has a clear owner and an exception process; a generic threshold should not be presented as binding outside the policy that established it.
What should you patch first when everything looks critical?
Consider two hypothetical findings with similar CVSS severity. One affects a confirmed, internet-reachable system supporting an essential service and has a KEV match. The other affects a system whose exposure and business role are not yet established, with no KEV match identified. The first has more evidence supporting immediate escalation. The second is not thereby safe: confirm its applicability and context, then prioritize it under the same policy. This example illustrates how evidence changes a decision; it does not represent a real vulnerability or measured outcome.
What does the 2026 CISA directive mean outside federal agencies?
CISA announced BOD 26-04 in June 2026. The directive’s federal prioritization structure considers asset exposure, KEV status, exploit automation, and post-exploitation technical impact. Its requirements apply to the federal agencies covered by the directive, not to every organization. Other organizations may find those factors useful, but should not treat the directive’s obligations or deadlines as their own. See the CISA announcement and its implementation guidance for the applicable federal details.
