A brace or tape may add protection, particularly for players at higher risk. Appropriate shoes matter, but current guidance does not show that high-tops prevent sprains better than low-tops.
Why basketball players sprain ankles
Sprains often happen during a landing. A player may come down on another player’s foot, causing the ankle to roll inward. That risk is one reason prevention should cover balance, coordination and control during movement—not just ankle strength.

A previous sprain is an important risk factor. In a pooled analysis of 2,265 Alberta youth soccer and basketball players aged 11–18, a history of ankle sprain was associated with higher risk of another ankle-sprain injury (IRR 1.98; 95% CI 1.38–2.81). Basketball participation was also associated with higher risk than soccer in that analysis (IRR 1.83; 95% CI 1.18–2.85). These are associations in a youth sample, not individual predictions. Owoeye et al., Clinical Journal of Sport Medicine, 2018.

Build a warm-up around strength, agility and balance
Use a consistent neuromuscular warm-up before practices and games. The youth soccer and basketball programs evaluated by Owoeye and colleagues combined aerobic activity, strength, agility and balance exercises. Across five studies, ankle-sprain risk was lower among players who did neuromuscular training (IRR 0.68; 95% CI 0.46–0.99). Because the analysis pooled two sports, it does not provide a basketball-only effect estimate. Owoeye et al., 2018.

For young players, the American Academy of Pediatrics (AAP) also identifies preseason ankle-strengthening and plyometric exercises as useful prevention measures. The AAP does not specify a single exercise dose that suits every player. AAP guidance on basketball and volleyball.
Balance work has a particular role after a sprain
Balance and coordination training are especially well supported for preventing recurrent sprains. The 2021 clinical practice guideline strongly supports balance training for recurrence, while noting that evidence is less established for preventing a first sprain. It also says the ideal exercise type and dose need further clarification. Exercises can be done without special equipment; a balance board is optional, not a proven requirement. Consider a brace or tape, especially after a previous sprain
An ankle brace or tape can be a reasonable additional layer of protection, particularly for players with a prior sprain or other risk factors. A 2018 guideline update reported lower risk of recurrent sprains with bracing or taping (RR 0.30; 95% CI 0.21–0.43) and lower risk of first-time sprains (RR 0.69; 95% CI 0.49–0.96) among sports participants. The evidence covers sports generally, not basketball alone. The guideline describes the choice as a matter of preference and practical considerations. British Journal of Sports Medicine guideline update, 2018.

A brace is not a substitute for rehabilitation after an injury; ask a clinician or athletic trainer about fit and use.

Choose appropriate footwear, but do not rely on high-tops
Wear appropriate footwear for basketball, as the AAP recommends. However, the clinical practice guideline reports no difference in protective effect between high-fitted and low-fitted shoes. There is not enough support in these sources to claim that high-tops alone prevent ankle sprains or to identify one shoe model or court surface as the best prevention choice. AAP guidance; British Journal of Sports Medicine guideline update, 2018.

NBA youth guidelines recommend at least one day each week away from organized basketball and caution against high-density competition schedules. Treat this as general youth workload and health guidance, not as a quantified ankle-sprain prevention measure. The guidelines also say basketball-specific neuromuscular injury-prevention programs need further evaluation. These recommendations are for youth players, not adult training schedules. If an ankle injury happens
Seek prompt medical evaluation if a player cannot walk or bear weight on the ankle, or has severe pain; an X-ray may be needed to check for a fracture. Returning to play involves restoring range of motion, strength and balance, including through weight-bearing exercise as appropriate. A clinician should guide the return-to-play decision, particularly after a significant injury or when symptoms persist. AAP guidance on basketball and volleyball.
