If your clinician or lab has told you to fast, follow the test-specific instructions; fasting is not required for every blood test. When it is required, the usual window is 8 to 12 hours, but confirm the exact duration. Plain water is generally allowed. Ask what to do about medicines and supplements, and tell the lab if you accidentally eat or drink anything else before the draw.
First, confirm whether your test requires fasting
Do not assume that a routine blood test requires fasting. Glucose tests, lipid panels, and basic metabolic panels are examples that may call for it, but the requirement depends on the specific test and your clinical situation. Follow the instructions from the clinician who ordered the test or the lab’s written directions. MedlinePlus advises not fasting unless your provider tells you to do so (MedlinePlus: Fasting for a Blood Test; MedlinePlus: How to Prepare for a Lab Test).

When fasting is required, MedlinePlus gives 8 to 12 hours as the usual range and says your provider should specify the exact interval. For a fasting blood glucose test, MedlinePlus says to fast for at least 8 hours (MedlinePlus: Blood Glucose Test). These are not universal instructions for every blood test: use the duration on your test order or provided by your clinician or lab.

Once you know the required duration, count backward from your appointment to choose when to stop eating. An early appointment may let much of the fast happen overnight, but do not start earlier than necessary or extend the fast beyond the instructions.
What you can drink—and what to avoid
Plain water is generally allowed during a fast and can make it easier to stay hydrated. Unless your test-specific instructions say otherwise, do not have food or drinks other than plain water. That includes black coffee, tea, juice, soda, and flavored beverages. MedlinePlus and Labcorp also advise avoiding gum, smoking, and exercise while fasting (MedlinePlus: Fasting for a Blood Test; Labcorp: Fasting for Blood Work). If your lab gave you more specific directions, follow those.

Medicines, vitamins, and supplements
Ask the ordering provider whether to take each medicine and when. Do not stop or skip a prescription on your own just because you are fasting. Mention over-the-counter medicines, vitamins, and supplements too; they may matter for some tests. Tell the ordering provider or collection staff before the blood draw. Be specific about what you had and when. They can decide whether to proceed or reschedule based on the test and its instructions; do not conceal it or decide on your own that the result will be usable (MedlinePlus: Fasting for a Blood Test; Labcorp: Preparing for your Labcorp diagnostic test).

After the blood draw
