Try: “I care about you. How are you really doing today? You don’t have to talk if you’d rather not.” Then give them room to answer—or not. The aim is to show care, not to get a medical update or find a perfect line.
Choose a moment that leaves room for an honest answer
If you can, ask when neither of you is rushed or exhausted. A quiet, private moment may make it easier to talk, but avoid making the conversation feel like an appointment or an interrogation. Their willingness to talk can change from day to day, so take your cue from them.

Ask simply, and make talking optional
Use words that sound natural coming from you. The American Cancer Society suggests saying, “I’m not sure what to say, but I want you to know I care. I’m here for you.” You could also ask:
- “I’ve been thinking about you. How are you really doing today?”
- “Would you like to talk about how things are going, or would you rather talk about something else?”
- “I’m here to listen if you want to talk.”
- “You don’t have to answer now, but I’m here when you want to.”
These are possible ways to open the conversation, not lines you need to deliver word for word. For more examples, see the American Cancer Society’s guidance on supporting someone with cancer.

Listen before you try to help
Pay attention to what they say instead of planning your reply. Let them finish, and resist the urge to judge, minimize their feelings, or steer them toward a more comfortable subject. You can reflect back what you heard to check your understanding: “It sounds like the waiting has been the hardest part. Did I get that right?”
If they pause or cry, you do not have to fill the silence. Macmillan Cancer Support notes that silence can give both people time to think. If it would help, ask whether they want you to listen, offer advice, or simply keep them company; then follow their preference. The National Cancer Institute’s guidance on talking with family and friends also recognizes that some people want others to listen rather than solve every problem.

If they share fear, anger, or sadness, acknowledge it
You might say, “That sounds frightening,” or “I can see why you’re exhausted.” You do not need to explain the feeling away or make a prediction about what will happen. Macmillan advises against promising that everything will be fine; a caring response can make room for difficult feelings without trying to fix them.

Offer practical help without making them manage it
“Let me know if you need anything” is kind, but it leaves the other person to think of a task and ask for help. If you can genuinely do so, offer one manageable action: “Could I bring dinner Tuesday?” or “Would a ride to your appointment help?” The American Cancer Society lists meals, errands, rides, childcare, and dog walking among ways to help.
Make the offer easy to accept, decline, or adjust. They may prefer a different kind of help—or none right now. Practical support is an invitation, not an obligation.

If they do not want to talk, respect that
Accept a no without asking them to justify it. You could say, “Of course. We can talk about something else, or just sit together.” The American Cancer Society recommends letting someone know you are open to talking later while making clear that they do not have to talk now.

“How are you?” should not feel like a demand for a medical update. A check-in can be about their day, interests, or ordinary life—not only their illness. They may welcome everyday conversation, an invitation, quiet company, or practical help even when they do not want a serious discussion. Keep plans flexible if their energy or preferences change.

Respect what they choose to share
The person with cancer decides what to disclose, when to discuss it, and whether to answer. Do not press for details they have not offered. The American Cancer Society’s guidance on telling others about cancer addresses the person’s choice about what to share; the same boundary matters when you are the one asking.
Also avoid comments about appearance that could embarrass them, such as remarks about weight loss or looking pale. If you are unsure what language they prefer, ask before using words such as “fight,” “battle,” “win,” or “lose.” People differ in how those terms feel to them.
Stay in touch in ways that fit your relationship
A single conversation does not have to carry everything. Keep including them in ordinary conversation and plans when appropriate, and adapt if they are tired or need to change plans. Taking cues and respecting quiet are part of staying supportive; the American Cancer Society offers more advice in Being a Friend to Someone With Cancer and How to Communicate as a Caregiver.
